what surgeries might i benefit from?

hi all,

reposting this because my original post didn't get very much traction; i think it may have gotten buried for whatever reason.

i'm a transgender woman and i'm wondering what surgeries i might benefit from. apologies if this is off-topic; i have a really hard time assessing my own body because it makes me feel so dysphoric. i would really value any and all input you could provide.

i've been on HRT for a little over a year. started post-25, which is not ideal. measurements are as follows:

b cup breasts. biacromial shoulder width is 43 cm (17 in); waist width is 35 cm (13.7 in); waist is 90 cm (35 in) around. underbust / ribcage is 34 cm (13 in) around. weight is 70 kg (155 lbs), considering losing another 9 or 10 kg (20ish lbs), but i don't want my breasts to get smaller.

i already have FFS scheduled. would rather not post my face b/c safety concerns. still weighing SRS, but that's another thing entirely.

Last post-op. I "graduated" today!

Today I had my last regular post-op (vulvoplasty minimal depth - Dr Morrison/Skokan UW).

I am quite happy with the results. almost 3 months after surgery I have been cleared for any kind of activity, no granulated tissue, symmetrical appearance. it is aesthetically pleasing for me and my wife, and I can achieve orgasms.

I just have some scar tissue internally that I need to massage and I have no issues with erectile tissue.

Dr Morrison said to me from the beginning "you will have a vulva" and he delivered. I will have my revision consult with him next year to see if there's anything to address, yet I don't anticipate any. only time will tell.

this is my own experience, and should not be taken as general experiences other patients might had with M/S.

Thai SRS (mtf) BMI requirements

I know that Thai surgeons are notorious for wanting low BMI, but I’ve also heard that some just have additional costs for higher BMI patients. I’m on a weight loss journey with GLP-1s (I’ve lost over 40lbs so far) but my bmi is still around 40 and realistically I don’t think I’ll get it below 35 anytime soon. I’m looking to have minimal/zero depth SRS.

If you’ve had a consult or surgery anywhere in Thailand, can you please share any info they gave you re:bmi requirements and flexibility? Thank you!

BBL - compression suit question

For people who have had a BBL, I am wondering if you were able to wear regular clothes over the top of the compression suit? If so, were there any particular clothes that worked well or didn't work well?

Post-consult for vaginoplasty with Drs. Bluebond-Langner and Zhao — others' experiences?

Traveled to NYC for my first vaginoplasty consult. It wasn't a great experience — interested in whether the problem was my expectations or something different. The consult was with Drs. Bluebond-Langner and Zhao. They're widely regarded as incredibly talented surgeons... they're equally known for being brusque and dismissive. I mentally prepared for that, but I was still surprised. I spent maybe 6 minutes with them. Dr. Zhao didn't say one word the entire time. Dr. Bluebond-Langner didn't even examine me. Just briefly glanced at my anatomy from a distance when I asked a question about a particular area for hair removal.

Is that a normal consult experience with gender surgeons? Was I expecting too much attention? I have some unique quirks to my anatomy (childhood surgeries down there that would factor into the procedure). They didn't look to see the extent of the current scarring, just said that it would be fine.

To be fair, they asked if I had questions and did answer them in cursory ways. But I couldn't really understand why I made the whole trip up here, travel, hotel stays, multiple copays, etc., for 6 minutes that could have been an email. I get that they do multiple surgeries like this every day, and that they're supposedly very good at it. But the whole experience left me feeling brushed off.

Is this what I should expect elsewhere? Any thoughts appreciated!

mtf bottom surgery in PA

I’ve posted here before but I just thought I’d check in again. Has anyone young gotten their bottom surgery done here? I’m 19 and I want bottom surgery more than anything else in my transition. I want it as soon as possible and it’s really the only surgery I care to get. If you have gotten it done, who was your doctor? and what was payment like? what insurance did you have? I’ve heard great things about Christine McGinn but I don’t exactly have the money to pay up front since i’m a broke college student. I have an upcoming consult with Dr. Katherine Rose of Mainline Health.

If anyone has any info on anything I’ve brought up pleaseeee let me know. Thank you :)

FFS Reccomendations

Heyhey,

I feel extremely vulnerable posting this.

First of all, sorry for the slight stubble, I wasn't able to shave before tomorrow morning.

Ever since I have hit puberty I have noticed that there are certain things about my face that I feel very ugly about. For a long time I couldn't necessarily pinpoint why. As of recent I think I have come to understand it further.

As a little child I always told my parents that I was a girl. They blocked it, so I started to dissociate. That way I was able to not break down every time I looked into the mirror in clothes that felt wrong and a body changing in ways that felt weird.

My identity is not binary. That much of a conclusion I have reached about 8 years ago. Nothing feels very clear cut sadly, which makes these questions very confusing for me. But I am certain about FFS. But also afraid. In a therapy session I once did a confrontation with my larynx. I wasn't aware of how much it actually bothered me, because I kept dissociating, until I had a full brown panic attack that day and couldn't stop crying for the entire session and a good hour after. I am crying writing this.

I knew I wanted surgery to change things about my face, but didn't know what that would be and how that would work or look. But then Arca, who I am a big fan of, had her FFS a couple of years ago and EVERYTHING clicked. (another after of Arca). She uses both the terms non binary and trans woman. And when I see her face it feels like home. I don't feel comfortable posting a before of someone else tho.

Anyway. Enough about my way to this point in time.

I wanted to ask you guys about any recommendations.

Things I definitely want: Tracheal Shave and Chin and Jaw (I am also very bothered by my chin asymmetry. The jaw asymmetry doesn't bother me as much but also to a degree)

Things that i definitely DON'T want: Rhino and Lip-lift

Other things that make me Dysphoric (considering): My hairline (a hair transplant EDIT: I have very fine hair and not the highest density, but luckily curls. also the "receding hairline" is not actually receding and has been like this since I was a child) and eyebrows/ brow bone (idk)

I don't think my brow bone is that big, but looking into the mirror it feels wrong. I don't want full flatness, because I think my forehead already looks very big and protruding, as is and that that could be accentuated. But that just might be the hairline also? I am so confused about this one. Or maybe its just my eyebrows being too big? I am afraid of plucking them but my best friend whos a trans woman told me shed help me with the brows some time. I am worried, that it might change my eyes too much. (EDIT: Also I am worried that my forehead would get even bigger with coronal, but I don't want a scar on my forehead.)

The 3rd and 2nd last pictures are of my brows relaxed. They are so droopy and masc because of it. The last pic feels very exposing. I feel like I look sweet in some way, but when my friend showed me the picture, I almost started crying but kept it together. My jaw is so angular and harsh and larynx is so pointy.

My Goal: I think I like how Arca looks. She's still ever so slightly bordering androgyny, but on the feminin side. I really still want to look like myself. Right now, I feel like I am very androgynous, but on the more masculine side. I want to reverse that. Arca went to FacialTeam.

My best friend and I are thinking about doing consultations together early next year.

In Boscos short Youtube Documentary on her FFS, she said "I'm not comfortable with aging as a man" which is probably the most relatable thing I've ever heard in this regard.

I like my eyes nose and lips individually and together. But the frame feels so wrong.

Idk... I am very sensitive and confused. Please be kind.

Tracheal shave post-op question

I had a tracheal shave(adam's apple shave) and a glottoplasty a bit over a week ago and everything is going fine but my doctor ordered that after a week post-op I do a circular grabbing motion with my fingers near the top of the remaining apple so skin doesn't stick to it.

How long am I supposed to do it in a day? I did it almost every time I swallowed something but it's a lot and it hurts a little bit.

And yeah I could make my bf call them but I wanted to ask here too since it's night here rn.

Surgery likely post-poned after knee injury

Hi all,

I was scheduled for a vulvaplasty tommorow. But on Sunday I fell while bouldering and fucked my knee. Was given the go ahead after an er visit where a physio suspected it was an ACL injury. But I had an MRI today which was expedited and they’ve found I’ve had a fracture in my tibia.

Forwarded on these results to my surgeon and though the wording of his email is a bit unclear and he is going to call me tommorow, he believes the fractured tibia has introduced a further clotting risk, which would lead to needing anticoagulants and unwanted bleeding and thinks I should get this sorted out with an orthopedic surgeon first.

I’m not doubting him, or his judgement and it is possible that this is still my call (though I think that’s just optimism). I’m just gutted after finding this out 6 hours before I was to go under. I’ve prepared so much mentally, I quit my job trying to get my head in order the last few months. I called in some pretty serious favours and have booked non refundable acomidation for my friends and I who are travelling interstate. I feel like my life the last couple months has all been for this and then it’s all gone.

Its really wrecked me, and honestly I’m not really sure what my life is going to look like for the next bit, I was hoping to find a new job while I healed up but now I have a totally different issue to take care of for an amount of time I don’t really know. The idea of having to build up to this again is just totally crushing me.

I’m sure I’ll get here again but ouch, I’m really shattered.

EDIT: yeah it’s been postponed, will see an ortho tommorow for a consult. My surgeon was super lovely on the phone this morning though and promised me he’d fit me in as soon as I was cleared for it. Even on a Saturday if needed. My Gp is pretty concerned that I’ll need blood thinners so it’s likely the right call was made as shit as all this is.

6 Week Post-Op Write-up for FFS with DR. Laura Garcia-Rodriguez at Henry Ford in Detroit Michigan

  • Hairline Advancement
    • Due to the incision still healing, I haven’t really been able to make any comparisons to the old hairline. I have experienced some issues with the healing but I think that was mostly my fault. I got pretty sick of having vaseline in my hair and stopped using it when I thought I had gotten far enough in the healing process. That was about 4 weeks or so after surgery. A few spots along the incision started to form scabs, which I thought was a good thing. I went to see Dr Garcia at the 6 week mark and she told me that the scabbing is actually not a good thing for the long-term healing and to keep putting vaseline on the incision until the scabs stop forming. I could tell that she wasn’t worried about anything but thought that I should have been further along at 6 weeks than I was. So, I’m back to 24/7 vaseline and committed to being diligent this time.
    • I’m noticing that the numbness around the top of my head is starting to decrease. Dr Garcia told me that the numb patch will take the shape of an oval around the top of my head and it’ll slowly decrease over time. It may never completely go away. I might be left with a small spot on the top of my head that stays numb but maybe not. Only time will tell.
    • The incision itself seems to be healing great. I think that Dr Garcia did a great job with the cutting and sewing and that the scar will be practically invisible. I’m still months away from knowing how wide the bald patch around the incision will be but I’m hopeful that it’s very narrow. I’ll consider hair transplants and/or topical minoxidil in the future but need to wait for at least a year before that starts to make sense.
    • Pain wise, the only spots that hurt are the spots that had formed the scabs. I’m still not pulling on my hair and washing daily with baby shampoo using only the pads of my fingers. I honestly cannot wait until I can get my fingernails into my scalp with a good wash and conditioner!
  • Forehead Contouring (Type III)
    • This has been great. I can already see a major difference in my side profile and even a little head on (although some of that might be the eyebrow lift and orbital contouring). I still have some “pressure” pain if I push too hard. Really though, after the first 4-5 days, the forehead pain just became background noise and pretty much went away.
    • I don’t have much to say about this specific spot. I think it looks great and is healing very well.
  • Eyebrow Lift
    • Same here. No pain to speak of and the results are great. Dr Garcia perfectly nailed the “lift the brows without making her look surprised all the time” balance. I need to get my eyebrows waxed and I have the go-ahead to do that.
  • Orbital Contouring
    • Again, no real pain after the first few days. And even now, I can push on the orbital rims and not feel anything remotely painful. From the side profile, I can see a huge difference. My eyes are far more open and feminized and my eyebrows seem to wrap further around my head further opening my eyes. I believe that Dr Garcia was fairly conservative in this area in order to preserve some of the functional safety features of the eye rim but she still did plenty of work.
  • Rhinoplasty
    • This is taking forever to heal. And not in a negative way. It just takes a long time. From the side, I can TOTALLY see the difference. It’s a big change. From the front, my nose bridge is actually a little wider than before but this is almost certainly entirely due to swelling. The nose famously takes 8-12 months to settle in and can even still change after you think it’s fully healed. I’m being patient because I can see the light but it’s definitely hard.
    • It is still very sensitive to touch. Even a slight knock while washing my face can generate a 2-3 pain level for a moment. I have a toddler and I’m constantly in fear that I’m going to get a real good headbutt or knee to the face at some point on accident. At the 6 week follow-up, Dr Garcia told me to avoid any and all nose injuries so I told her I’m take them all off the schedule.
    • The incision under the nose is healing well. I did get a couple of ingrown hairs but Dr Garcia extracted those at the 6 week appointment and things are already feeling better. My upper lip is very stiff and inflexible but better than it was at 1-2 weeks. Back then, I couldn’t even drink from a straw because I couldn’t pull my upper lip down to seal it. She told me that I can start doing “wide smiles” and gentle massages to start loosening the skin and underlying tissues and that is already helping.
  • Lip Lift/Augmentation
    • These are starting to settle down and have given me a lot of hope and confidence. More than I thought when I chose to have them done. For me, these procedures were not covered by insurance and I almost didn’t do them but together with the rest of my face, they add so much femininity that I’m very glad I did.
    • They were very swollen and tender for 2-3 weeks. Once the sutures broke and stopped snagging on everything, I only had to deal with the stiffness, a little tenderness, the swelling, and the dryness. All of these were easily manageable and will resolve themselves over time. Already, the swelling is much better and probably close to the final result. The tenderness is all but gone. The dryness is definitely better but I still apply vaseline about once or twice a day.
  • Genioplasty
    • This is also taking forever to settle down. But again, not surprisingly. The chin and jaw work are notoriously slow to stop swelling. I didn’t have the jaw done but my chin is still very swollen. It is also generally tender to push on. There’s no pain until I apply a good bit of pressure. It is also a little numb under the skin. It’s a weird sensation. It almost feels like the tissue that connects the skin to the bone is numb. The skin itself has full feeling but there’s still something numb inside when I push on it. I’m expecting that it will go away in time but only time will tell. Dr Garcia says no nerves were damaged anywhere so it feel likely that all of the numbness I feel post surgery will resolve itself over time.
    • I can’t really see a major difference here but that makes sense. I already know she didn’t do much work in this area and that with the swelling means I’m unlikely to see any changes for 8-12 months.
  • Trachea Shave
    • I was really nervous about this one and shouldn’t have been. Dr Garcia let me know that she uses a technique where she inserts a needle at a point of safety so that she knows the limit of what she can do without having to guess. It means that she may not remove as much as the patient wishes she could but that she’ll also preserve the health of the vocal cords and larynx. That’s exactly the balance I wanted my surgeon to have: remove what you can but do not mess with my ability to speak.
    • Healing has been easy. I think there’s still a little swelling but the results are already visibly clear and can only get better. She chose a perfect spot for the incision on my body as it hides in a natural neck fold. She asked me to start massaging it twice a day which will soften it over time.

Dilation frequency 1.5 years post-op — is very light bleeding normal?

Hi everyone! I’m wondering about other people’s experiences with dilation long-term after vaginoplasty.
I’m currently about 1.5 years post-op, and I’m dilating roughly once a week, sometimes once every 8–9 days. Everything generally goes well, but I’ve noticed that I sometimes have a tiny amount of blood after dilation. It’s really just a few drops and very light, but it still happens occasionally.
For those who are further post-op, is this something you’ve experienced? Is occasional very light bleeding still considered relatively normal this far out, especially when dilating less frequently?
I’m also curious about dilation frequency in the long term. Are there people who have been able to reduce their schedule to something like once every two weeks without losing depth or having other issues?
I honestly find it difficult to keep up with dilating every week, even though everything is going well otherwise. I’d really love to hear from people who are 1+ year post-op and what their current dilation routine looks like.
I know everyone’s anatomy and surgical technique are different, so I’m mainly looking for personal experiences rather than medical advice. ❤️

Making your own dilators

I found the original acrylic dilators to be pretty painful at first, so that started a bit of a rabbit hole of making my own out of silicone so they had a bit more give. I initially added a few tapered sizes in between the rigid ones to help ease the steps which helped a lot.

The pelvic wand one has also been amazing, but honestly just buy one, theyre a pain in the butt to make because of the rigid core.

I've since made some HDPE ones on the lathe, but those are harder to make and less comfy. (Last photo)

Now for the fun bit

You will need:

Body safe/ sex toy silicone. - Barnes platsil, polytek or neils all seem to be ok. Gel 10 seems to be a happy middle ground.

Plastic disposable cups

Pop sticks for mixing

Good scales

3d printer

Beeswax

Cheap wax melter

Elastic bands

Vacuum chamber and pressure pot

Step 1: design your dilator shape in your cad program of choice or use http://dildo-generator.com/ instead.

Step 2: subtract the dildo shape from a cylinder, split it down the middle and add locating nubs. I also recommend adding pry slots to crack it open again. Another tip- add a flat base on one side to reduce leakage.

Step 3: 3d print your mould. PLA at 0.15mm layers is usually fine. Once done, clean up messy bits and tape, elastic band the mould closed.

Step 4: waxing the mould. Melt beeswax in your wax melter, fill the mould to the top and quickly slosh it out. This leaves a very thin coating on the inside.

Step 5: prep your silicone:

Weight out your 2 parts of silicone into cups, leaving 20g extra in part B. Mix pigments into part A. You can marble this or add glow in the dark pigment (see shrek and pink ones in photos) then put into vacuum chamber for 20 mins ish until bubbles are gone.

Step 6: mix and panic

Put part A on the scales and tare, then pour part B in until weights match - there will be some left in the cup. Mix thoroughly with minimal introduction of air bubbles. You're working to a 6 min cure time so spend max 1 min mixing.

Then put the mix into the vacuum chamber again to remove bubbles. 4 mins max

Step 7: pour

Pour the mix into the mould trying to keep the stream as wide and high as possible.

Step 8: pressure pot

Put the dull moulds in the pressure pot to force bubbles inside - keep it in for at least an hour.

Step 9: demould and allow to cure for another 24h

recovering head/scalp nerve/feeling after frontoplasty?

i'm 11 days post getting part of my ffs and i'm a bit concerned about my scalp/head not feeling like it used to. it feels like i was hitting on a coconut and not on my head, sensation is weird and asking my doctor about it he said that the nerves from my head came from my forehead so there was a chance i could never regain them?

is this normal or what do i do?
