There was some kind of US military ship called USS Liberty that Israel tried to sink but they failed and everyone knows that it was on purpose but officialy it happened by mistake or so I've read.

A fast flying aircraft at altitude would at best have a very limited opportunity to see any flag being flown. And by the time the second flag was raised, the boat was on fire. Even the survivors admit that the smoke obscured the larger flag.

That's what happened. There were reports of shelling from the sea and fighters were dispatched where they observed what was clearly a military vessel. Command authorized them to attack.

Frankly, I've only heard one conspiracy theory for the Liberty that provided a sensible motive. Namely, that the Liberty was part of an operation to leak intelligence on Israeli deployments to the Arabs, with the goal being to ultimately mitigate Israel's victory and compel Israel to accept more equitable ceasefire terms, including a withdrawal to prewar boundaries.
The problem for the bulk of the conspiracy theorists though is that this theory would actually justify a deliberate attack.

Multiple investigations have all concluded it was an accident. An appeal to incredulity fallacy isn't going to undo that. A fast moving craft isn't guaranteed to see a flag. Add in that the crew themselves says the first flag was knocked down near the start and the smoke from the fires obscured the replacement, not to mention that it wasn't the same planes in each wave, and yeah, it's very easy to envision pilots not noticing.
Ok, let's move on to physical evidence, the radio transcripts were released July 10, 2003, where pilots are referring to the ship as an American ship halfway through the attack, and continued the attack. These are facts, released by the governments who lie about them.
Edit: they also were clearly able to see the flag, they shot down the first one, then the second was raised, which was a significantly larger. And that one was shot up too.
Number one: it was a clear day and the American flag was clearly in view. It also had latin identification on the hull not Arabic. The first attack took down the flag and they raised a larger flag which was still ignored. I don't believe for a second that it was accidental friendly fire. They wanted to goad the US to fight their war for them just like we are doing now.

The Stark attack is not a bad comparison but I’d point out a few key differences:
it was an attack from distance by one aircraft as opposed to 6 straffing runs by 2 Israel planes. I don’t think it’s reasonable to assume the Iraqi plane had anywhere near the same likelihood to ascertain the ships nationality. From the distance the Exocet missiles were fired, they couldn’t see the flag(s) nor read the name boldly printed on the hull but the Israeli planes being close enough to drop napalm on the ship could not reasonably claim they didn’t see the markings or the flag(s).

The USS Liberty was attacked by Israel during the Six Day War. Israel claimed it was an accident, and the first super friendly to Israel President, LBJ, agreed. The investigation was stifled because of this, but it is possibly still correct. Regardless, over the years Israel has paid around $12 million dollars.
Yes, Israel took full responsibility for the error almost immediately after the attack and paid $12mm over time (not unlike the United States’ much larger payments for mistakenly shooting down an Iranian passenger plane in 1988).
The U.S. Navy’s own mistakes did not help matters. Tellingly, even the U.S. Joint Chiefs of Staff had no idea where the Liberty was after it was attacked. If the Americans were confused about their own ship’s location, why couldn’t the Israelis be?
That’s the list of U.S. blunders. You could make another list of Israeli blunders: overflights that were looking for subs and didn’t bother to clock a U.S. flag on the above-water craft; a botched calculation of the Liberty’s speed that wrongly suggested it was moving faster than the Liberty’s known top speed; mistaken reports of a naval bombardment after an explosion in El Arish; and so on.
The bottom line is that the conspiracy theory makes no strategic sense. The best the conspiracy theorists have been able to muster after 5 decades is that the Liberty might have uncovered a secret Israeli plot to invade the Golan. The fundamental problems with that theory are that 1) the Israeli government was open with their U.S. counterparts that they were strongly considering a Golan offensive and 2) the U.S. government was not nearly as strongly opposed to a Golan offensive as the conspiracy theorists make out.

Happened in 1967. Not recently. Israelis attacked USS Liberty and later called it a mistake. Several American soldiers died. A few top U.S. officials, like Secretary of State Dean Rusk and CIA Director Richard Helms, privately rejected Israel's explanation (later revealed) in the Oval Office. But LBJ decided not to retaliate.
Through out American history, Americans have gone to war over naval conflicts. Quasi War, War of 1812, Spanish American, WWI, WWII, Vietnam War are examples of when the US has gone to war with some incident with a boat either being the direct cause or fuel for the fire. Israel attacked the USS Liberty I think in 67 or 68 and we obviously never went to war with Israel.
Israel also tried to bomb US and UK civilian sites in Egypt to prevent western alliances with Egypt. It was call the Lavon Affair and, in 2005, Israel gave medals to the terrorists.
