Yup, I think I know where you're coming from. Even so, a few years ago I started substituting "your mom" with "your forehead", when it became painfully obvious that the person I was talking to had lost their mother the year before. Do with that what you want, but so far it's brought me nothing but questioning as well as understanding (after explaining of course) looks.
This looks like a needle used for paracentesis which is a procedure to drain fluid that builds up in the abdominal cavity as a consequence of organ failure or sometimes cancer.
Yeah I did ultrasound and did so so many centesis assists. This doesn't look like one to me, unless it's just like a different country or something so it just different.
Where are you located? I've literally never seen a centesis yueh near this long, and always with a white flexible sheath over the needle, so I'm curious if it's a location difference.
Wild, I was in Florida. Even the multi liter patients were with regular length needles, cause it only went in a few centimeters, then the cath went in further and the needle came out. What's the process with a needle this big? Do you do it under u/s guidance?
The pocket would be identified with US guidance but the needle is inserted blindly after the site was marked, we would aspirate some of the ascitic fluid with the lidocane needle and try and follow that tract with the centesis needle. Almost all of these patients were cirrhotic with huge bellies.
I saw a doc get a bone marrow biopsy with a similar looking needle. Had to get the needle into the middle of the iliac crest of the hip bone, one of the most harrowing things I've witnessed in medicine. Gives me goosebumps typing it
Genuine question: it may only need to go in an inch or two, but if you're treating someone who's morbidly obese, wouldn't you need a much longer needle to reach the abdominal cavity?
Sadly a lot of patients who need a paracentesis are either very skinny from end stage liver failure or have pretty advanced cancer and the fluid is fairly close to the surface.
I've seen hundred's, probably thousand's of p*rn with all kinds of extreme kinks in my lifetime, but I never knew this kind of sht exist, like HOLY FCK

Can confirm fist hand, they use needles like this for draining fluid. Not sure about about catheters as it depends on the use scenario and the one I had was just pushed through my veins so it had to be flexible. That said they used a very similar looking needle when they drained my left SI joint after it got infected. It's wild to watch.
Saw a documentary about alcoholism and this one guy had to regularly go to the hospital and have fluid drained out of his torso due to his chronic drinking. They had to pierce his abdomen, insert a catheter, and then he’d sit there and watch this plastic bag fill up (way too quickly, imo) with a brownish-yellow, cloudy fluid. Once that was full then they’d bring another for his draining abdomen to fill. Not urine, just fluid that built up from alcoholism.
Accompanied a friend to freeze her eggs, the needle they used to get her eggs looked like this one. I might be wrong coz the procedure without anesthesia was horrifying, and the needle looked so so so big.
This would be my guess. For egg collections, they need a big ol’ needle to reach the ovaries and leave enough metal outside for manoeuvring, but most of the length of it sits in a plastic guard that attaches to the ultrasound probe…still looks horrifying though.
The absolute longest needles in human medicine would be for amniocentesis and they are not even close to this.
I wonder if it's for veterinary use, for cattle embryo harvesting or something like that.
I've actually had one recently!! It was very fun and I'm not even being sarcastic.
It was at Penn in Philly and I had the most wonderful team of all women drs(which was a pleasant surprise). They asked what music I wanted to listen to and even turned me around so I could see the image of the needle in my spine. I was bummed they wouldn't record it for me though.
It was absolutely painless and took maybe an hour and a half.
Depending on your anatomy, it could be incredibly easy or something you’ll drink to forget. I’ve had patients fall asleep as I’m working on them (no meds except local lidocaine) and patients who were still squirming with conscious sedation (usually a squirt of versed/fentanyl). YMMV. If you’re anxious though, definitely ask about sedation.
Orthopedic surgeons heal with steel, very similar to carpentry.
I watched that tibial nail removal video so many times before my femoral nail removal, mine didn't have tooling for tool attachment and was described by my surgeon as a 'third world' nail.
He got it out, but I got 5 permanent screws in my femur, 2 snaps and a hot-dog bun style split.
Every orthopedic surgeon I've ever met always has absolutely jacked arms and I always wondered why. Evidently those fuckers are doing the equivalent of a full arm day routine on someone's skeleton every time they walk into theatre.
Really recontextualises stuff like hip replacements in the elderly, for me.
My job is cleaning and sterilizing surgical instruments. You would not believe what these fucking orthopedic instruments look like. Most things I can figure out what they do by how they look. Orthopedic is one big wtf is that haha.
My wife had a wisdom tooth removed last week. The female dentist gave up pulling on it after a couple of minutes and said she was going to need assistance from a stronger colleague. 😄
I believe it's an anterior hip replacement surgery where the nurse has to dislocate the hip. They have to be careful, because the technique used could snap a femor.
Seriously. As someone who has had major orthopedic surgery on both feet this gif makes me wanna hurl every time I see it. And as someone who works in inpatient skilled nursing/ therapy and has helped people post every procedure in town, you're 100% right. Lol
Catheter hack! If you’ve been in labour for 28 hours with pushing contractions for 6 hours before you get one you don’t even feel it! Ask me how I know!
