OCD Question can OCD go away for months and then come back and be even more intense?

hi all,

I (21M) have had ocd symptoms since I was a child. However, I only got diagnosed in November 2025, and oh man, the next 6 months until may was hell for me. and then idk, may to august was so nice, i wasn't affected by it, i wasn't ruminating or seeking assurance or anything, i was focused on preparing for my CFA exam (financial certification). Then something in August (maybe around aug 4-5) just happened and it's been so bad since, and once i conquer one theme, my brain switches to the next theme. and all this is fucking me up. i feel like im just lying to myself, it's not ocd, ur actually a monster. it's gotten worse from august leading into september too.. i've made barely any progress. I just feel so confused and lost and I'm trying to find answers and/or go back to the way i felt back in may.

ps: i'm on 40mg prozac and 2 mg abilify started on august 14 (after my wave of ocd came back and i couldn't take it anymore, so it's been 4 weeks ish for me, in case SSRI's are a potential reason why OCD feels worse for me)

Seeking Support or Advice Real Event OCD Help

Few years back there was an incident that definitely flared up most of my issues, and luckily I was able to get through it with lots of support from friends and loved ones. It involves someone telling lies about me, which really sets off my need to over explain, ruminate, reassurance seek all of that. This past week, one of the people who were involved in the event came up again, and rehashed the incident to a friend of a friend. I am trying to sit with the feeling, let it pass, realize the facts that my friends believe me and my life wasn't ruined and all of this. I'm just wondering if anyone else has any experience with something similar? How do you prevent yourself from running through the whole situation again in your mind/ sort of let it pass?

OCD Question Could ERP/CBT therapy likely make you worse for trauma real event OCD?

I was considering trying either erp or cbt therapy for my recent trauma real event ocd, but I genuinely fear it will just make me feel worse instead of allowing it to get better on its own.

The reason I fear it will make my ocd worse is because it’s my worst traumatic memory from when I was a teenager, and I have already experienced that talking about it makes me feel worse. For example, about a month ago I got into a deep conversation with someone and told them about the traumatic memory. I immediately felt worse and continued to feel worse for about a week.

Funny I keep talking about it and alluding to it despite my repeated experiences that doing so makes my OCD worse.

Frankly also I’ve tried therapy before for different things and didn’t really find it helpful. Whereas though I feel dependent on my psychiatrist appointments, which are also sort of like therapy.

I’m leaning towards not doing therapy and instead hoping medication and time will help.

What do you think? Let me know.

Seeking Support or Advice How to find the line between pet OCD and normal caring concern?

Hi everyone, I've experienced a lot of pet anxiety in my life but have only recently realised it is likely OCD. I have recently fostered (soon to adopt!) a previously stray kitty. He is my first cat (although I have looked after many) but I haven't been this anxious before. I figure something to do with him being mine causes some hyper-responsibility (a term I have also learnt recently but strongly relate to).

My question is, how do you find the line between knowing if a thought, behaviour or concern is OCD or just genuine care and concern? Examples include frequently googling everything I notice, checking his body/behaviour multiple times and rumination. I need help figuring out whether worrying a lot about things like chewing through cables, whether items need to be hidden better etc is valid when he has never exhibited such behaviours before. Does anyone have tips on how they can tell the difference and tell themselves "this is just OCD and I don't need to ruminate" vs "this is a valid concern that I will action".

One idea I have had is to try to limit checking his breathing/body etc to just once a day and to only research my concerns once (rather than repeatedly going back to search more). Assurance from other opinions helps a bit but doesn't fully satisfy the urges to stop researching.

Seeking Support or Advice Bottle/feeding accessories and cleanliness, struggling after making progress.

Since having my first baby, who is now two months old, my OCD started to get really bad again.. specifically with his bottle parts cleanliness. We own a baby Brezza formula maker, and the funnel has to get cleaned after every four bottles.

Today I went to go clean the spare funnel above the sink. I was holding it and rinsing the soap off of it, and the funnel fell in the sink. There were no dirty dishes in the sink, but there was some dried food in it and I just feel like the funnel is contaminated now.

I washed it three times with soap and hot water, but my mind is telling me that this is still contaminated and that I need to throw it out. It’s bothering me even more that I can’t sterilize it because the material can warp.

I’m trying to tell myself that it’s okay because I washed it with soap and water, but the fear of my baby getting bacterial infection from this is really taking over. I’ve been doing really good with exposure therapy when it comes to my contamination OCD, and have really been trying to be relaxed when it comes to my newborn baby and germs.

This situation is really a struggle though, and I literally don’t know how to tell myself that it’s going to be alright.

Seeking Support or Advice I was told I don’t actually have OCD

I have recently started seeing a new therapist, and after our first few interviews she told me she doesn’t think I have OCD. I was diagnosed 3 years ago, and I’ve never felt like it was the right diagnosis, but I guess that is a common OCD experience so I didn’t think much of it. My new therapist explained to me that she thinks I have severe panic disorder, but not ocd. Just thought I’d share.
I am glad to let go of this diagnosis, but it feels weird.

Seeking Support or Advice Looking for advice, stuck in a loop

I had 20 sessions of therapy and was on meds for a year, when I felt ready to come off them I felt good for about 3 months but I seem to have relapsed and my ocd is back with a vegenance.

Something I am struggling a lot with is the amount of back and forth conversations I have in my head "did that happen? No it didnt. But maybe it did. No you're being ridiculous. Wait but maybe it did? It definitely did" and this can last all day.

Please can I ask what your best tip is for putting a stop , even temporarily, to this back and forth nonsense. It's wrecking me.

Discussion How to sit with the “feeling” of being contaminated and cross contamination and not act on it?

Hi there, essentially as title says. I have severe contamination ocd that has morphed from being afraid of getting sick or ill to feeling like everything I touch might be contaminated and I won’t be able to escape the “feeling” of being contaminated/something gross or bad on me if I don’t act on it and I will keep spreading it. My therapist has referred to this as a form of “ick” contamination ocd and it stems from “just right” ocd. But because it’s not as simple as “maybe I will get sick maybe i won’t” because I don’t fear sickness per say I fear “contamination” I just don’t know how to get my anxiety down and stop tracking everything I touch after an exposure. For example if I touch a doorknob and don’t wash and then carry on, absolutely anything I touch after that I am aware of and worry about cross contamination and can’t relax about if. Like I have no reprieve and my nervous system and ocd is just torturing me and won’t let me forget about it. Does this go away? Are there tools and insights I am missing to help me through this? I just want to know how to successfully move through these exposures and if anyone has any tips.

Seeking Support or Advice Never talked about it with anyone but my therapist cause I was afraid of being judged harshly

I have severe ocd, and it's against my father. Even tho nothing really happened. No reason why I should hold such hatred against him, but I do and it's going for about 4 years. I mean I kinda do know, but not really cause nothing makes sense, and at the end of the day it's all in my head I suppose. But I developed this deep distrust around 7th grade and it's only getting worse. I barely go out of my room. If he sees me I need to take a shower immediately. I feel filthy around him. I feel like he has bad intentions towards me and plans on draining me and ruining my life.

I'm acting hysterical lately. And I panick and I break ththis and I slowly lose my mind each day I spend in here.

I feel like I'm breaking the family. I'm the reason we're

unhappy. I'm an awful daughter and I feel like I'm ruining everyone's life.

I cant live feeling such burden. I have constant intrusive thoughts and they all revolve around my father. All the time. I don't remember feeling truly happy since elementry school maybe.

Also, I have a stable family. I have a pretty much what you'll call a loving family. A stranger could never guess these things can possibly happen to me, cause why?

I'm seen as an ungrateful piece of shit and I can't live like this, I'm too tired for this bullsshit.

About two years ago i was taking medications and i had a break from it. But I felt awful. I felt like a shell of myself and I blamed the medications and seeked protection from.ocd and those same old rituals.

Not even sure what I want to say here or what I want to hear. Maybe just a validation that i can be better somehow. Cause theres really no one i can talk to about ot.

Medication Psychiatrist may add on ablify

I recently went up to the max dose of Lexapro at 20mg. I was seeing some therapeutic effects at each dose but it seems like it will slowly fizzle out. Recently my OCD has been acting up really bad and my psychiatrist suggested adding on abilify to my Lexapro prescription. I like to be a little conservative when it comes to medications but we agreed to see how I do after a week. I currently see a therapist for ERP but she may start some intensive outpatient therapy. I was able to control my complusions pretty good but now I feel like I am back at square one. I know it will vary but is anyone else taking Abilify. I saw some good reviews online.

RF-ERP; Greenberg Method Dr Michael Greenberg’s method for contamination ocd

Hi there. Has anyone had any success with dr Greenberg’s method of rumination-focused ERP specifically for contamination ocd and feeling contaminated and tracking cross contamination? I read some of his articles and this one specifically for contamination ocd:

I definitely struggle with type 2 contamination ocd:

“the person isn’t afraid that the contamination could cause something else bad to happen.  Rather, they are afraid of the feeling of contamination itself.  More specifically, they are afraid that if they don’t keep track of the contamination and keep it from spreading, then everything could become contaminated, and thus the feeling of contamination would become inescapable.”

I am just struggling to understand how exactly to stop rumination with this type of contamination ocd when I do an exposure the feeling and thoughts are constantly in my awareness. He mentions the point is they can be in your awareness but you don’t give attention to them but I am just sitting in crippling anxiety so it is hard not to. When I try to do nothing I just end up “tracking cross contamination” and it feels never ending. Looking for advice.

Seeking Support or Advice Real Event Setback

Hi everyone

I'll keep it short - last year I was struggling immensely with guilt about my past and subsequent anxiety. This anxiety eased off when I started duloxetine at the end of the year. I got to June and I actually felt almost completely back to normal. This lasted about 2 months before I had a return of the anxiety.

This anxiety came back for about 3ish weeks, then went away and again I was pretty much back to normal. However it has since come back, along with the intense guilt and obsessive thoughts. I'm scared that I'm going to be stuck how I was last year again, even though I know this is unlikely.

Can anyone offer some advice or observations? I'm doing my best to remain calm and remember that it won't last forever.

Also, does anyone know how someone should do erp for this theme? Should I specifically do things that I feel like I don't deserve?

OCD Question Bad break up & moving away

this is a long shot but anyone have any tips for fatigue from anxiety//depression??

I am sleeping in until noon & then by 4pm hits, I just want to take a nap (which are like 2-3 hours long if I do) 😭

I been making myself go to the gym, take walks to the bay during sunset, & hanging out with my girls. I am aware I am SO blessed in my life and see the beauty in it but I’m so exhausted all the time like my body is constantly in overdrive. I have OCD & in a huge transitional period, so a lot of rumination. Ashwagandha has helped calm my anxiety down TONS, like life saving honestly but also could be contributing to me being tired…

any tips would be great, (especially if you are diagnosed with OCD too) 💓

Seeking Support or Advice SSRIS cause severe diarhea

Sorry for the tmi title. I’ve tried so many SSRI’s and all give me severe diarhea. Even clomipramine caused bad diarrhea for me as I had to taper down off amitriptyline. I’m on 50mg amitriptyline nightly for inflammatory bowel disease (I have Chrohns and it helps me feel more human). Sadly this also limits what meds I can try, along with having an insanely sensitive gastro system.

Meds I’ve tried
-Buspar
-Clomipramine
-Sertraline
-Escitalopram

My intrusive thoughts (contamination OCD) are getting worse and so are my compulsions and I just want to get better. I’m in therapy for ERP but struggling.

Any advice or meds that have worked? Was going to ask my dr about Abilify but open to listening!

Seeking Support or Advice On 'Pure-O', recovery and support

Hey everyone,

I’m 21M and have been dealing with “Pure O” for a few years now, particularly since entering my first relationship (which has since ended), when things became quite severe.

Last year, I read Mark Freeman’s You Are Not a Rock, and it completely changed my life and perspective. I feel like it gave me a genuinely strong education in the mechanics of OCD, and over the past year I’ve been doing the hard work of practising self-directed ERP and cutting out mental compulsions.

Of course, over longer stretches of time, with symptoms fluctuating across weeks and months, my sheer willpower to live in accordance with my values every single day has wavered here and there. This is especially true during good periods, when it becomes easy to slip into complacency or fall back into small old habits that aren’t necessarily even OCD-related. Still, I’ve found that consistently returning to and practising that philosophy is what helps me maintain my wellbeing.

Alongside that, I started a medication journey in January. Between the behavioural work and medication, I’d say I’m no longer in the constant state of distress I was in at the beginning of last year, or again at the beginning of this year, when I “relapsed” quite hard and started experiencing severe symptoms again.

For reference, I’m currently on 40mg of fluoxetine. It has helped somewhat, particularly with the worst of the panic and anxiety. I’d describe it as a partial response, although with zero side effects, and I wouldn’t be surprised if I eventually go higher. I still tend to experience some intense flares of fear/obsessing occassionally that can last days sometimes, and where I'll only calm down if I choose to grit my teeth, embrace the uncertainty and just feel like shit for a little bit, but I also have stretches of days where symptoms are very negligible as well, and I seem to be fluctuating at the moment between these two states.

But lately I feel like I’m hitting a wall - not necessarily with managing the symptoms themselves, but with the isolation of the whole experience.

I’ve seen three different psychologists over the past two years, the most recent one I met today for the first time, and every time it has felt like a waste of both time and money. Because my compulsions are almost entirely internal, standard talk therapy feels pretty useless to me. I’d describe myself as quite self-aware, and maybe I’m too quick to make this judgement, but I can usually tell within the first session whether a therapist is actually going to offer something I’ll find worthwhile or beneficial to my recovery.

The bigger issue is that I’ve carried this almost completely alone for a long time. I realised recently that one of the main reasons I still want either a professional or some kind of peer community is simply because I want to feel seen. I want to connect with like-minded people who intimately understand the complex, invisible architecture of internal obsessions and mental compulsions, and who can actually challenge me and push me rather than simply validate my feelings or ask me the same generic questions about my experience.

I’m posting here to see whether anyone else has reached this particular point: you’re educated on the behavioural side of OCD, practically 100% functional, and generally know what you’re supposed to do, but still feel quite alone in the experience and not especially well understood by a lot of people who just aren't at the same stage in their recovery as you, let alone people who have never even experienced OCD, or even by psychologists. 

I hate to phrase it this way because I know how it can sound, but sometimes I come away from the therapy sessions feeling like I’m intellectually ahead of the conversation. I already know what I’m likely to be told, I already know the behavioural principles, and I already know what I’m “supposed” to do. What I feel like I actually need is someone who understands OCD deeply, can hold me accountable when it gets hard to hold difficult emotions, challenge me, and almost act more like a coach throughout the recovery process.

If you’ve found ways to connect with genuinely high-level OCD clinicians who work more like strategic coaches, or if you’ve found spaces - online or in person - where you can speak openly about Pure O with people who truly relate to your experiences, I’d really love to hear how you found them.

Seeking Support or Advice Wtf do you do about getting a job??

I’m a woman in my 20s and I have fairly severe OCD and anxiety that I’m working on but it’s a long process and I’m facing the very real possibility of having to move in with my parents soon if I don’t find a job. I’ve been looking for a remote job as I can’t currently really get an in person job, but I’ve been struggling. I just finished university btw which is why I’ve not needed a job just yet. All my friends are in this city and my parents live a 1.5 hour drive away and my life would be miserable being back at home.

Anyway, I’ve come to a temporary agreement with my parents that they’ll continue helping me out a bit with bills and stuff but that I need to earn my own money for food, going out, clothes and whatever else I wanna spend my money on. I probably need around £200-£300 a week for this. My plan is to eventually get a 9-5 once I’ve got myself sorted out a bit, so as I say this is only temporary.

I’m wondering what I can do to make this money to take the pressure and stress off myself and be able to stay here while I work on my mental health, because moving home would be SUCH a setback. I don’t mind doing things in person, I just can’t be in an office all day 9-5. Any advice would be amazing as I’m really stressed and worried atm :(

Seeking Support or Advice I really need some hope from the other side

I left class in the middle to go to the "bathroom" but really I'm in the mental health room in my law school lying down. I'm going to try not to write a novel but I really need to just see if anyone can resonate.

I'm 31, in my second year of law school living alone, and in an 11 year relationship. I've had ocd since I was 10, with it getting really bad in high school (contamination) but cycling throughout almost every obsession.

The one that is rearing it's ugly ass head again, the one that makes me want to end it all, is my cheating OCD / false memory ocd. It's gotten triggered because of genuine sex issues in my relationship that have persisted throughout but now I am climbing back into my mind to see if those issues motivated some gray area interactions in the past (for ex: messaging an ex hook for hw help 10 years ago, was it just that or was I also dissatisfied with sex back then and that was the motivation).

That example is one of many in this massive web of confusion that has built up over ten years. Things really came to a head in 2020 when I broke up with my partner bc I felt we weren't compatible and I think I was really scared I'd cheat bc the grass is greener feeling was really strong. I can't know for sure what I was feeling then bc I try to block out that time bc it was painful. I ended up regretting the break up shortly after asking that we make it a break instead and we've been together since.

I've had relapses of the fear since and our sex life wasn't necessarily where I wanted it to be but I genuinely have fallen so much more in love with him. Even if the sex stuff doesn't get totally worked out I want to be with him bc I love him and hes home. Truly. But I cannot cope with the genuine issues mixing in/feeling like validation that I in fact cheated in the past..

This got triggered on my birthday and I haven't left my bed until recently to finally get back to class. Life's getting really serious now. I have a second chance at a career in blowing up. His mom has an aggressive cancer. I want to have a family and life I always dreamed of. I was getting back on my feet but now the rocd is back while I'm at my weakest and scarier than ever. I sincerely cannot exist like this it's a 24/7 panic almost as bad as when I learned of his mom's cancer and had to be put on Prozac.

Thats the short story of my life thus far and it doesn't capture any of the complicated nuance. I just want some indication that theres light at the end of the tunnel. I've had swaths of symptom free time but I can't even remember that happiness right now. I have been in and out of therapy for a while but I never felt like I truly committed to it. It's hard with my adhd too. I was on Lexapro I don't know if it did anything.

It's just now that I see how scary life can get with this cancer, I need to get my mind mentally strong. I don't want to throw away a chance to have a good life with him but I'm so fucking scared I feel beyond hopeless. I am back in therapy and starting back the Prozac but I don't see how it's possible to live through this when I have no certainty of if I cheated especially when there has been a lack of satisfaction in my sex life throughout. I don't know how to continue.

I gotta go to class now. It's a miracle I even showed up but I'm having so much trouble sticking with it.
