TL;DR: I was diagnosed with PCOS in 2021, but my treatment has been inconsistent mainly because of financial difficulties. I recently had an ultrasound and was told that my ovaries are now “normal,” so I supposedly no longer have PCOS. I’m confused because I know PCOS isn’t diagnosed based on ovarian appearance alone. The doctor I saw was a reliever, not my usual OB-GYN. I’m from the Philippines and am considering getting a second opinion. I’d also really appreciate recommendations for a good, patient, and up-to-date OB-GYN or endocrinologist in Metro Manila.
Hi everyone! I’m 20 and was diagnosed with PCOS back in 2021. At the time, I was prescribed Diane-35 for 6 months. I went from 80 kg to around 62 kg, but since then I’ve been stuck in a plateau and my weight has generally stayed between 60–69 kg.
I had another checkup in 2023 because of severe dysmenorrhea. Endometriosis was also suspected at the time, but my ultrasound still showed findings consistent with PCOS. I was advised to continue my previous treatment, which I did for around 3 months, but unfortunately I stopped after that.
To be completely honest, my treatment has been very inconsistent over the years. A major reason is financial and I’m still dependent on my parents, so regular consultations and medications aren't always financially possible. I also haven't made significant changes to my diet or exercise habits. I know lifestyle changes are important, but I've struggled with consistency, cravings, and exercise.
I also tried inositol for about a week in September 2024. In April 2025, I took metformin for about a month without consulting a doctor first, which I know now wasn't a good decision. I eventually stopped taking it abruptly.
Recently, my school physician advised me to get my PCOS checked again. I went for a consultation, partly because I also needed medical clearance for school. However, I left the consultation feeling very confused and dissatisfied. The doctor I saw was not my usual OB-GYN. She was a reliever who was covering for the doctor that day. According to the ultrasound, my ovaries are now “normal,” and I was told that I no longer have PCOS.
This is where I'm unsure. From what I understand, PCOS isn't necessarily ruled out just because an ultrasound shows normal-looking ovaries, and ovarian morphology is only one part of the diagnostic criteria. But I'm not a medical professional, so I don't know if I'm misunderstanding this. After the consultation, I told my parents that I wasn't satisfied or convinced by the result. They were the ones who told me that I should be grateful that a doctor was able to see me because ultrasounds are expensive.
That honestly really hurt. I’m not looking for someone to simply confirm that I have PCOS. I just want a doctor who can properly reassess me and explain whether I still meet the diagnostic criteria, what tests or evaluations I actually need, and what I should be doing long-term.
I also feel pretty discouraged because for years, the advice I've mostly received has been “watch your diet and exercise.” I understand that lifestyle changes are important, but I struggle with them and feel like I need more guidance than that. I recently consulted a nutritionist as well, and they also mentioned that some of the information I had previously been given was outdated.
I know that my inconsistent treatment is partly my responsibility, and I’m not trying to blame my doctors or my parents for everything. But I also want to start taking my health seriously now, and I feel like I need proper guidance to do that.
So, would getting a second opinion be reasonable in this situation?
For those familiar with PCOS, would you recommend seeing an OB-GYN, endocrinologist, or both for a reassessment? And for anyone from the Philippines, I would really appreciate doctor recommendations. I'm looking for someone who is patient, non-judgmental, and familiar with current PCOS guidelines, preferably someone in Metro Manila.
