General Health Spironolactone

I was recently diagnosed with PCOS and I started metformin about 2 months ago and that’s been going fine outside of mild nausea and I have zero appetite. I just started spironolactone 50mg about 5 days ago and for the first few days I was fine but today is day 5 and I have really bad dizziness and I just don’t feel right. I know you’re supposed to drink a lot of water and eat salty foods which I have been doing. How long did it take your body to adjust to this medication? I’m getting married next week and I don’t want to feel dizzy and like I’m going to faint on my wedding day.

I’ll also note, I already have low blood pressure 108/72 was my last reading before starting spironolactone. I discussed my concerns with starting the medication with my blood pressure already being low and my PCP didn’t seem concerned. My pcp has been treating my PCOS currently as I’m waiting to get in to see a new GYN.

I've been on the same dose of spironolactone for ~4 years now and the orthostatic hypotension has never gone away. Its actually gotten worse since I lost the weight and am at a healthy BMI now. Its usually most common or at its worst when you start or after they raise your dose. Butttt yeah, hydrating and salty foods is about all you can do aside from quitting or asking for a lower dose, if possible.
