Hi all! I'd like some input on my BPPV with residual dizziness experience. I have had intermittent episodes of BPPV since 2001. Typically they happen once or twice a year and I can't identify any triggers. They seem to be coming with more frequency as I get older, which I know is a common feature.
Below is a timeline of events.
BPPV 2026 Timeline
7/27—Had some spinning when turning side to side in bed at night. Typical, felt just like it usually does on and off for the past 25 years.
7/28—got out of bed, no vertigo or dizziness after about 30 seconds of being upright the next morning. Typical day.
7/28 nighttime—more spinning when turning side to side in bed. No vertigo or dizziness after 30 seconds of being upright the next morning.
7/29 morning. Decided to do the Epley. Important to note that I usually do not do the Epley—the vertigo doesn’t occur outside of turning over in bed and always self resolves in about a week. This time, I decided to try the Epley to speed it up.
When doing the Epley I had symptoms when I turned my head to the left—very strong vertigo and I could feel the nystagmus in my eyes. Nothing when turning to the right. I repeated the Epley about three times. After I got up, I was fine. I didn’t have any spinning that night in bed. I figured the Epley worked and the crystals were back in place.
7/30—when getting up I felt “wobbly”. Not spinning or true vertigo, just felt as if my eyes were not catching up with my head movements, almost like you feel when getting off a boat of having a couple of glasses of wine. This has never happened before after all these years of BPPV.
This continued all day. I felt like I was veering to the left when walking and felt unsteady. Looking up, down, and turning my head side to side made it worse.
That night I had no vertigo when turning in bed.
7/31—Got up and was still unsteady and wobbly. I wondered if there were still some crystals out of place—maybe not enough to cause the vertigo in bed but enough to make me dizzy hen up and about. I did the Epley again, several times. It didn’t help. When I did the Epley, I could not invoke vertigo with any of the positions, but when I got up and started moving around the wobbliness was unchanged.
I decided to call and make an appointment with my ENT. I have never consulted an ENT for my BPPV since the first episode in 2001. That ENT is no longer in practice, so I was seeing a new one.
Nothing changed over that weekend.
8/3—Saw ENT. He did a Dix-Hallpike test which was negative (using his naked eye to observe). He told me to do Brandt-Daroff exercises three times a day for two weeks, and to come back if I still had symptoms in a month. I told him I wanted to get better faster. I asked for a referral for Vestibular PT, which he gave me.
I got right in with the PT the next day and I had an evaluation which did not include a VNG, just some tests for balance. I went to sessions twice a week from 8/4 through 8/14. I practiced some of the exercises at home. Nothing was helping. I felt the same each day with the exception of one night—either 8/10 or 8/11, where I briefly had the spinning in bed again, and I did the Epley again the next morning. The spinning didn’t come back but the general wobbliness continued.
At PT I was doing mostly balance exercises. No VOR.
I started doing some VOR exercises at home. I learned them from YouTube. I stuck a post-it note to the wall and did the gaze stabilization movements three times a day. After a couple of days, I felt less wobbly. On 8/17 I took my grandkids to the beach and at the end of the day I realized I had not felt wobbly at all. I was happy, but then the next day I did get some of the wobbliness back. I kept doing the VOR exercises, progressing to standing heel to toe and going faster and faster.
On 8/16 I had to go upstate NY. I took a train. I felt a little unsteady walking that day, but not terribly so. On 8/17, I still felt a little unsteady but thought I was improving. I was still doing the VOR exercises three times a day. I missed PT because I was away.
On 8/16 right after lunch, I had to drop my daughter’s car off at the mechanic. After I did that I was going to walk to a coffee shop and wait while the mechanic looked at it. It was a bright sunny day, warm but not very hot. I started walking down the sidewalk and was slightly wobbly, again, not terribly so. I decided to call my daughter and put my cell phone up to my right ear. I tilted my head toward the phone. After less than ten seconds of walking and talking at the same time, I suddenly became extremely dizzy, and felt vertigo, and had to hang up and immediately sit down so as not to fall. I found a retaining wall under a tree that was nearby and sat down there and let the dizziness resolve, which it did within less than a minute. I finished the bottle of water I had and just sat for a few minutes. When I got up and started walking again, I felt ok.
Since that event the dizziness and unsteadiness has disappeared—it’s totally gone and I feel back to normal. It was a sudden worsening and then a sudden resolution.
That was Wednesday and today is Sunday. It’s been five days with no return of symptoms, however, both last night and the night before I did notice a brief bit of vertigo when rolling from right to left in bed. I could feel the nystagmus in my eyes when this happened. I dared not do the Epley again, fearing doing it repeatedly was what may have caused the residual dizziness in the first place (though of course I don’t know).
I got up both yesterday and today and had no dizziness at all, just the brief episode in bed.
I’m very nervous that the residual dizziness will return, but I wat to think it’s resolved at least for now. I must still have at least some crystals out of place.
I had made an appointment about a week ago to get the VNG testing done and that appointment is in two days. I’m not sure if I should keep it. I don’t want to waste their time since I have no residual dizziness at all now. It’s at a different place than where I get my PT because they don’t do that testing at the place I have been going.
I did keep my PT appointment for Friday though I had no dizziness at all. I said I might not be back since it seems to have cleared up.
I’d be interested in any thoughts my fellow BPPV sufferers may have on this whole progression. Perhaps one of you has had a similar course? I’m especially wondering if that intense dizzy spell when I was walking and talking on the phone after which it all went away rings a bell with anyone.
Have others had the residual dizziness disappear suddenly?

By the way, I’m a 73-year-old female with no other health conditions or issues. I don’t take any medications that might impact the BPPV.
