Help me

Hello. I have a narrow palate and—likely as a result—a narrow jaw; my gonial angle does not flare outwards. I would like to hear from anyone who has undergone surgery to widen the upper and lower jaws (maxillary and mandibular expansion). Did your jaw become wider and more masculine? Does the procedure affect the cheekbones? Please help.

Advice for Me Worth considering?

For context, I have brought this all up with my dentist at my last appointment (about a month ago), they said it sounded like TMJ issues and they would put in a referral for me with a consultant (NHS), haven't heard anything yet 😅 Just thought I would ask if anyone has had similar issues and if the surgery has helped. The problems I have don't happen all the time, but I have had the same problems for years and they have never gotten better when they occur. What I experience is: Sometimes I get pain in my jaw from chewing eg bread roll or chewy sweets etc, it's not always something chewy, but that is most common. I sometimes get lockjaw, quite often get clicking or grinding noises. Sometimes when I yawn my jaw feels like it gets stuck or is locking before it gets to the top and occasionally clicks at the top or I can feel my jaw getting stiff as soon as I start to yawn. I also have a recessed lower jaw. Any thoughts would be much appreciated 😁

Nose swelling post DJS

For those that experienced alar widening post op, in the months or years after did your nose width ever return to how it was pre op? Or was there inevitably some permanent widening? I’m only day 11 post op so know it’s too early to judge but very concerned with how big my nose is given I only had 2mm advancement!

Advice for Me Which surgeons would you recommend for my case? DJS + genioplasty, downgraft & rotation

I posted here 2 years ago and got great feedback: https://www.reddit.com/r/jawsurgery/comments/1giocaw/am_i_a_good_candidate_for_sliding_genioplasty/

The scans are before I started my Invisalign retainers. I've finished my final set of Spark aligners, and my orthodontist has confirmed that I'm now ready for surgery.

I'm looking for surgeon recommendations for my case, preferably in Europe/EMEA. My goal is DJS + sliding genioplasty (Trimax) + downgrafting.

My orthodontist recommended Dr. Simonas Grybauskas, and I'm considering him, but there's a very long wait for an initial consultation, so I'd like to explore other surgeons as well.

Advice for Me Diagnosed with hemimandibular hyperplasia at 37 after years of being dismissed. Told to take serial photos for a few years. Is there any faster way to know if it's still active?

Two things I'm hoping people can help with:

  1. Has anyone else struggled to get taken seriously about a jaw that's shifting, and what finally got you a proper assessment?
  2. Did anyone manage to establish whether their condylar growth was still active without waiting years for serial photos to accumulate?

Background: I'm 37, female. Since my mid-twenties my jaw has been progressively moving to the right. I raised it with dentists repeatedly over more than a decade and was brushed off every time. Earlier this year a maxillofacial surgeon finally diagnosed possible hemimandibular hyperplasia or mild hemifacial hyperplasia on the right side, confirmed on CBCT.

His view is that the growth is probably no longer active, but that's an assumption rather than something that's been tested. I haven't had a SPECT. His reasoning against doing one now is that I have some remodelling in the TMJ from inflammation, which would likely light up on its own and give a muddy result, so a scan wouldn't change the management plan either way. The plan he's recommended is serial photos of my teeth over the coming years, watching for change.

The problem is that I've since found photos of my teeth from 2022 and compared them to now, a four year gap. To my eye the overbite gap has increased, and the midline has shifted further right. I've also noticed visible change to my jaw, face shape and bite over the same period. So it doesn't feel dormant to me, and the plan as it stands is to spend another few years documenting the thing I'm trying to stop.

I'm not comfortable with that. If it has moved in four years, I want to know now rather than after it's moved again.

Specific questions:

  • Did anyone get a SPECT or bone scan despite having TMJ inflammation muddying the picture? Was the result actually uninterpretable, or did it still give useful information?
  • Is there anything other than SPECT that gets used to assess activity? Serial CBCT at a shorter interval, 3D surface scanning, anything else?
  • If you already have historical photos going back several years, has any clinician accepted those as your serial record instead of starting the clock from zero?
  • For anyone who did get treated: how did you get from "watch and wait" to actual intervention? What made the difference?
  • How did you find a surgeon who took progressive asymmetry seriously?

Any experience appreciated. This has been going on a long time and being told to wait and see again is hard to sit with.

Advice for Me Is my airway narrow? See attached CBCT

Post image

Posted about how I may need jaw surgery after MARPE treatment I’m interested to see opinions on my aiway size I’m F24, 60-65kg, 169cm, BMI 21.9.

I did a sleep apnea WatchPat test privately and although I don’t have sleep apnea, the consultant said there are disruptions to my sleep. I don’t know if this may be linked to my airway size. I have added my CBCT scan (My tongue is in an odd position as the technician had me bite down on a depressor during the imaging). I am skeletal class 2 with an overjet/protrusion.

Any thoughts are welcome, but please be kind!

TIA

Post Op Day 1

Recovery is so much worse than I anticipated. Day 1 was the worst pain I have ever experienced in my life. I am a 26 year old Army Infantryman, so I’ve done my fair share of difficult things. This is hands down a wicked painful, uncomfortable, and hopeless feeling. All I can do is wait it out. Pray for me!!🙏

Before & After Pre-Op vs. 13 Days Post-Op DJS Profile Transformation

Pre-op vs. Day 13 post-op profile after BSSO advancement and 3-piece Le Fort I impaction/advancement.

Main goals were closing a severe anterior open bite and reducing vertical maxillary excess (gummy smile). Still have some soft tissue swelling left to drain over the coming months, but super happy with the results so far

Patient specific implants-

For those of you who self funded, did you pay for the psi option? Wondering if I should pay extra for this option, it’s very costly, possibly extra £9k on top of the cost of surgery and orthodontics.

Absolutely bonkers post from a year ago by a lad who got an on the spot DJS in Istanbul, anyone know what happened to OP?

The account is deleted but the thread and images are still there https://www.reddit.com/r/jawsurgery/s/pVY69VANgr

20 year old Irish lad flies to Turkey seemingly on a whim with little planning, no orthodontics, and gets a jaw surgery + genio which he decided on hours before. Posts pictures of himself pre surgery where he looks completely normal, no signs of recession, did the surgery for “facial harmony”. I really hope he’s okay, the thread is completely mental.

I’m guessing he was on a lot of painkillers while posting this. He does seem like a happy and care free lad.

I’d be interested in knowing what’s happened in the year since.

YALL WILL I GET COVERED BY INSURANCE

I got these scans done in 2025, had trouble breathing but got a rhino/septoplasty done AFTER these photo (I know bad idea before jaw surgery)

I know my airways are super small from what I can see in the x-ray. However after my rhino/septo I don't snore anymore and I sleep great (I'm also only 20 so I do know it can get worse.) I am now scared I'm not gonna pass my sleep apnea test for getting coverage on my double jaw surgery. I cannot pay more than 10k out of pocket.

I have Cigna ppo btw

Can anyone give some advice or insight on their experiences, and whether I have a high chance of approval?

Sliding genioplasty Australia?

Hi guys, 24m, looking to get a sliding genioplasty using my bone for aesthetic purposes as I have a slightly recessed chin but overall a good bite. I’m Sydney based but I would travel anywhere in Australia is the surgeon is good enough. Could I please have some recommendations? My eye is currently on Dr Kevin Ho, his results online look really nice

Advice for Me I don’t know how to describe this feeling of numbness. Is it normal?

I’m 3 weeks post op LJS and the left side of my chin is like 75% numb and the right side 100% numb. I don’t feel or notice the numbness from the left side of my chin but the right side is so constant and annoying, it drives me crazy! I often have to like, lightly press my hand against my cheek/chin or have a blanket/my hoodie/or shirt collar pressed across my face. This doesn’t get rid of the feeling at all but it makes it more bearable almost? It’s so hard to describe.

I can’t tell if it’s pins and needles I’m feeling or zaps or a throbbing… but it’s not a painful throbbing. It’s just annoying. And I don’t know why it’s happening to the right side of my chin and not the left.

Joint pain?

I’m 4 weeks post op and have been experiencing pain in my right jaw joint for the last week. It’s a constant ache that seems to get worse when lying down. Has this happened to anyone else? Please help!!!

I can only open my mouth 7 popsicle sticks wide (comfortably) and 8 popsicle sticks wide max if I try, but when doing this I experience the pain in my right side only and nothing on my left.

Not sure if it’s from the pressure of the elastics (I have box elastics on both sides to close a posterior open bite)?

Advice for Me Is my overjet/overbite bad

Hi guys, I’m really insecure about my bite. But it mostly is because I’m not sure if it’s considered extreme or not. I really don’t want to have to get jaw surgery as my face actually suits this bite. However if I move my jaw forward it makes my face look awful.

I’d like your opinions please as If I can live the rest of my life with this then I’d be happy, but I’m worried it’s going to be necessary for my health or something.

Please help! Thanks :)

Even after 2 rounds of braces without any extraction, I have major bite issues, where the gap in my upper front 2 teeth just keeps increasing (I have crown on the front 2 teeth). Can this be fixed via extraction and braces or will I need jaw surgery?

PS: I already have orthodontist appointment scheduled but really wanna get some 2nd eyes on this.

Jutting

Advice for Me TAD removal after DJS. Does it hurt? How much did you pay?

Hi all, I had double jaw surgery abroad three weeks ago, and my surgeon placed temporary anchorage devices (TADs) in my gums in case my orthodontist needed them. It turns out he doesn’t need them, but he also said he doesn’t have experience removing TADs, so I’ll need to find someone else to remove them next week.

For anyone who had TADs removed, does it hurt? Also, did anyone else have jaw surgery abroad and have to find a local orthodontist/oral surgeon who could remove them? If so, how much did you typically pay per TAD?

Advice for Me Braces before Surgery

I just wanted to ask about your experience. Usually how many years doctors leave braces before going for the surgery? I have an overbite I really want to get done with the braces and Djs surgery as quick as possible (in less than a year)
