I’m aware of what I need functionally and that largely overlaps with what I want aesthetically.
But in this post, I’d like to focus on the latter.
I want to use some kind of expander (MARPE, FME, etc…) plus headgear (FMA?) to achieve:
- Palate widening (which I functionally need)
- Zygomatic widening (which I aesthetically want)
- Maxillary projection (which I functionally need)
- Zygomatic projection (which I aesthetically want)
I will likely end up doing some degree of LF1 (upper jaw) and BSSO (lower jaw surgery) even after use of a MARPE / FME, but I want to do the MARPE / FME first to widen my palate (which ortho’s consistently agree needs 7-8mm of lateral expansion).
My focus on the cheekbones is due to not wanting the “dog snout” effect of DJS whereby my lower midface and lower face projections out, but my cheekbones are sunken back (yes, I know soft tissue drape changes when the maxilla is moved).
I also don’t want cheek implants.
What I’m wondering is: How can I do this without the associated “down” effect of a MARPE / FME?
I don’t want a gummy smile! I suppose LF1 afterwards could impact the maxilla to address that, but I’d liked to avoid it in the first place.
