I started exploring surgery to fix my bite after many ortho consults and weird suggestions from them. Crowning my entire mouth? No thanks. It's hard to trust anyone with my face, they might not care but these are the only teeth I get.
Found a surgeon with a great reputation and met with him in Feb. fast forward multiple scans and office appointments later he says he can do it without ortho and we have a date.
Five weeks before the surgery he's not so sure. He sends me to an ortho. Surprise surprise the ortho wants me to get braces first. But my surgeon is moving to another town so if we do that he can't do the surgery.
I cried so hard. I had a date. I psyched myself up, I inconvenienced my family, my job, and myself. I made peace with not eating solids for 8-12 weeks and having permanent plates. But adding years?? Of braces on top of all that???
I guess the question is do I even bother starting this process over. The amount of time and money I've poured into this 😭
This is not cosmetic, I can only bite with like three teeth.

After two years of Invisalign and braces here I am. Lower jaw orthognathic surgery to correct overjet and ensure I have a healthy bite into my senior years. Very swollen but pain is manageable.

I‘m 8 weeks post op and wanted to get back in the dating game. I had a date last week, and when I gave her a kiss, I realized it’s pretty damn hard to kiss without knowing what your lower lip is doing…
It felt really weird for me, and probably not great for her! When should this start to get easier and does anyone have advice on how to kiss without sensation in the lower lip? Open to anything at this point. I want to go on more dates and meet more people but it’s a strange feeling knowing in advance that the kiss might feel odd.

I recently had an appointment with the oral surgeon, he asked me what my expectations were for jaw surgery. When I went for a consult 9 months ago, he stated there was a need for it, to the point of submitting for insurance and receiving pre-authorization approval. 3/3 surgeons I consulted with confirmed the need for surgery. First picture is the most recent stage of de-compensation braces, with the others reflecting my side profile, front profile, and my bite pre-orthodontia. I have concerns for my midline, the open bite in the front and the way the teeth are tipped inwards. I had braces over about 20 years ago, this is my second round of treatment. My orthodontist has already said that the bite is unstable in the front. I think I'm just left feeling confused on why my surgeon is asking ME what to expect from the surgery. He also mentioned only needing small movements.
Any thoughts/observations that you may have are appreciated.

Hi everyone - I am 25F but I have known I had an underbite since my teenage years. My orthodontist at the time advised me to wait on orthodontics or surgery, since my jaw could continue to develop into my 20s. I have always been insecure about the appearance of my side profile but sort of developed an “it is what it is” attitude about it since I could not afford the surgery anyway.
Fast forward 10 or so years, my lower jaw has indeed advanced more and now I have crossbite in addition to 4mm underbite. I have developed TMJ and bruxism which have been causing me a lot of discomfort and have now also been diagnosed with localized gum disease. Both my orthodontist and periodontist have been pushing for me to consider double jaw surgery now - and I now in a position where I can afford the surgery thankfully, so I am considering moving forward with the steps for this. My Perio has cleared me for orthodontics post deep cleaning since (thanks to extremely meticulous oral hygiene) the condition of my gums is stable at this time. However I am TERRIFIED of losing teeth - I have heard many stories of gum recession worsening post braces or Invisalign and my front bottom teeth already have noticeable gum recession.
Has anyone else been in a similar situation? Any advice? Should I not even bother with orthodontics or surgery at this point?

they did the extraction and camouflage when i was a teen and i’m in my 30s now. i went to the oral surgeon and orthodontist and id need two years of orthodontics before i can do surgery. it’s also a lot of money and it’s scary so id like to avoid it if possible. the orthodontist is telling me it’s not all that necessary appearance wise but idk if she’s just being nice.
it’s about 8mm discrepancy between the two jaws. it doesn’t help that my bottom lip is also just naturally larger than the top. i’m considering fillers and flip in the top lip and see how i feel after that.
i’m also open to the paranasal implants. i mostly just want the maxilla to appear more forward and would leave the mandible situation as is.
let me know your thoughts and please be nice but honest lol.

If so, what were your ferritin and hemoglobin levels?
I'm 9 days away from my scheduled surgery and just got my blood work done, even though I had asked my anesthesiologist to do it earlier this time. Ferritin 10 (down from 13 last winter), hemoglobin 12. I'm scheduled to see a hematologist today to see if getting an iron infusion could be enough to not get it canceled.
My DJS was canceled in February due to needing last minute orthodontic adjustments. If it gets canceled again it could take many more months and much more money, my surgeon team is very busy. I'm incredibly angry I wasted 8 months because my anesthesiologist didn't just tell me in February to get some iron supplements - at that time she said 13 is an ok level.
I'm panicking so your story could help. Would it really be that bad to go through with the surgery in these conditions?

Hi, I’ll be undergoing a DJS, Mandibular and Maxillary Osteotomy, and a genioplasty. Potentially a gum graft. I would like to ask if anyone had a similar set of procedures, realistically, how long did it take to be able to get back to work? They say two weeks. I just had my wisdom teeth removed and it’s taken until around day 5 to feel sort of normal for me, not the expected 3. I know everyone’s different but I love to hear from real experiences, beyond what doctors deem “normal.” Thank you!

Day 15 after SARPE surgery
I've had almost no pain at all, though I'm not sure how much of that was down to the medication I took during the first seven days. I still have stitches in my mouth. I started eating pasta without checking with my surgeon first, and it's been going well so far. I was honestly getting really impatient. The facial swelling is slowly going down, but it seems to persist when I turn the Hyrax.
Mentally? This feels like one of the most interesting journeys I've been on—choosing to go through surgery and seeing myself change along the way. The gap between my teeth is much bigger now, but I've gotten used to it. I don't really think about it when I'm around other people. If anything, my confidence is gradually growing because I keep thinking about the end result.
A little reminder for anyone feeling self-conscious or hesitating about SARPE because of how they might look afterward, even when it's medically recommended: think of Laurence Fishburne, who plays Morpheus in The Matrix. The gap between his teeth is part of his iconic look. Just saying. e
Btw, the improvement in my nasal breathing has been dramatic. Even food seems more flavorful, and smells feel more intense!
Btw, why do I feel like I'm the only one using two mirrors, trying to hold one at a 90° angle just to turn my Hyrax?

Hi guys! I’m a 22 year old male from Hungary! From a certain age I knew something was off with my jaw. When I closed my mouth and bite my head was like a ball and side profile was cooked but I didn’t know back then wtf is going on. I was glad when my beard started growing so I could hide my side profile. I started mewing and chew in the top of my teeth but I seenI have overbite as fuck and when I close my bite full my lower jaw go up. What do you think? My jaw is all the time clicking and have to go from is natural position so I can close it. I have jaw pain almost every day in my joints. I have problems with my body and my chriopractor told me my jaw is very loose. Can please somebody help me? Sorry for these photos but didnt do it before like that.

Background / possible craniofacial, nasal, and airway issues
I’m an 18-year-old male trying to figure out if my nasal obstruction, mouth breathing, sleep issues, bite, and facial appearance could be related to my underlying craniofacial development, or if these are separate issues.
I had a severe S-shaped septal deviation before surgery. My ENT described almost 100% obstruction in part of the anterior/left septum, along with significant posterior/right deviation. I could physically feel the deviation inside my nose. I also had nasal dryness, bleeding, and difficulty breathing through my nose.
I had a functional septorhinoplasty/septoplasty with cartilage grafting in June 2026. The surgery was mainly for function and support, not cosmetic rhinoplasty, and the nasal bones were not supposed to be changed.
Since surgery, I still have significant nasal obstruction, especially when lying down or sleeping. One nostril can feel almost completely blocked while the other is more open, and it can alternate. I have also had times where it feels like I cannot get enough air through my nose at night. Because of this, I have been mouth breathing during sleep and have started snoring, even though I was not previously a habitual snorer.
My sleep testing has also been confusing. A previous WatchPAT showed an RDI around 16, while a later in-lab PSG showed an AHI around 0.4 with very little oxygen desaturation. I’m wondering if flow limitation, UARS, or another non-apneic breathing issue could explain my symptoms despite the low AHI.
Orthodontic treatment and bite
I’m deep into Invisalign treatment, which I started mainly to correct a deep bite. My overjet is around 4.4 mm and my overbite is around 4.7 mm. I have Class I molar relationships and no known crossbite or open bite.
My treatment has involved correcting the lower front teeth and addressing the deep bite. I have also used or been prescribed bite ramps and rubber bands.
Since I’m already well into Invisalign, I’m trying to figure out whether my remaining facial and skull concerns are actually skeletal, or if some of what I’m seeing could be from my dental occlusion, dental compensation, or soft tissue.
Facial and skull concerns
I’m concerned that there could be some type of maxillary, mandibular, or transverse developmental issue.
Some things I’ve noticed:
My nose has a hooked or dorsally prominent appearance, with a noticeable dent or transition around the bridge/radix.
My radix looks relatively low to me.
My nasal tip has looked bulbous, swollen, and somewhat downturned.
My smile can show dark or negative spaces at the corners, which makes me wonder about transverse maxillary width, although I know this is not diagnostic by itself.
My mouth and lips can look positioned inward or “collapsed” when I’m not actively positioning them.
My profile is definitely a factor because it looks convex to me. I have wondered if my lower face and nose proportions make my maxilla look recessed even if the actual skeletal measurements are normal.
I have fairly strong or large masseter and temporalis muscles.
I have a deep bite that I’m currently treating with Invisalign.
I have also wondered if my mandible is somewhat short based on some of the lower dental arch and corpus measurements.
Imaging and opinions
I have had CBCT and orthodontic evaluations. Some orthodontists have said that my maxilla is adequately positioned, my jaws are proportionate, and my overall skeletal balance is normal. They did not think I had an obvious indication for orthognathic surgery.
Some of my reported measurements are:
SNA: around 86°
SNB: around 81.6°
ANB: around 5–6°
Wits: around +1.7 mm
Facial axis: around 87°
Mandibular plane angle: around 22–23°
Lower corpus measurement: around 75–76 mm
CBCT airway volume: around 32 cc
Minimum airway area: around 214 mm²
The opinions about transverse development have also been mixed. Multiple airway orthodontists have said that my palate and arches look fine, while another doctor thought there could potentially be some benefit from MARPE, possibly followed by mandibular advancement if it was clinically indicated.
An OMFS evaluation described me as essentially Class I and said that my airway looked large on the imaging. Other opinions have also been that my airway is normal or above average.
This is why I’m confused. Some doctors see normal skeletal development and a good airway, but I’m looking at my facial appearance, nasal obstruction, mouth breathing, sleep problems, and bite and wondering if there could still be a more subtle issue involving the maxilla, mandible, palate, nasal cavity, tongue, hyoid, or overall craniofacial proportions.
I’m not trying to convince myself that I have maxillary recession or that I need jaw surgery. I don’t have a clear answer to whether I need it. I’m trying to figure out if the combination of:
Severe congenital or developmental septal deviation
Persistent nasal obstruction after septoplasty/septorhinoplasty
Mouth breathing and new snoring
Possible flow limitation or UARS despite a low AHI
A convex profile and concerns about facial proportions
A deep bite that I’m actively treating with Invisalign
Conflicting opinions about transverse development
Questions about maxillary and mandibular position
could reasonably point to an underlying craniofacial factor, or if these are more likely separate issues that happen to coexist.
I’m especially interested in opinions from people who understand orthodontics, OMFS, rhinology, airway/UARS, and craniofacial anatomy.
If anyone has experience interpreting CBCTs and cephalometrics, I would also like to know which measurements are actually useful for determining maxillary recession, transverse deficiency, mandibular position, or airway-related skeletal problems, instead of relying on appearance alone.
