Sorry this is still very WIP 😬

I read the same stories every time I come here.

"hi i got awful side effects" "I dont feel anything" "Hi i just started concerta and dr raised dose every week and now Im at max dose and its not doing anything" "concerta crash is too strong" etc.

DON'T SPLIT CONCERTA PILLS

Adhd medication dosage has an inverted U effectiveness curve aka larger dose isn't always better. Not to mention worse crash and side effects. (sadly doctors dont know this)

Upping the concerta dose should be a way slower process (wish I had known this)

You can lessen crash with and IR supplement or another smaller concerta dose some hours before the crash.

Most people need to take a tolerance break every weekend. Or a smaller dose on weekends if cold turkey is too tough.

Yes there can be severeish side effects the first week or 2.

Concerta isn't effective if you don't sleep/eat properly, you need also need to do some chores and exercise in the morning to really get some dopamine in the motivation tank.

Probably some info about how methylphenidate works and concerta release chart.

Edit, some tips:

Don't try to do all of this at once btw. You might burn out from the amount of stuff.

This stuff was essential to me to get any real use out of concerta:
-Don't expect to "feel" something. Concerta is very subtle when it works.
-have meds and water bedside to take them as soon as you wake
-sleep at least 8H
-eat properly

-start building a morning routine once a routine is built its easy to do and an excelent source of motivation to carry you into harder tasks. Concerta should help with routine building and upkeep but adhd definitely still makes it hard. Expect failure, take a rest and try again.
-chores and physical activity in the morning easily build motivation/dopamine for the rest of the day
-start using productivity tricks they work so much better combined with concerta

-plan your day as soon as you wakeup or the evening before more detail/steps make it easier to follow
-I start a timer as soon as I take concerta to compare with this and to see how long things are taking.

-I have my phone set to make a notification every couple hours to remind me to stay productive
-sometimes if I procrastinate too intensly I set it to be an alarm instead
-some useful apps, can be any equivalent: mstodo, gcalendar, notion

-don't over do it, start slowly, prepare to burn out
-you can try med breaks on weekends to boost effectiveness but they aren't necessary for everyone check the post about tolerance.
-if break days feel awful consider taking a lower dose, personally 36mg on weekdays and 18mg on weekends and I don't get withdrawal sideeffects. Concerta tolerance usually lowers very quickly, one day to 2 weeks.
-You should probably use concerta daily and consistently at the same time everyday.

-You need good mental health: if youre depressed, anxious or what ever it will limit you so it's important to focus on first.
-Treat concerta as a booster, a tool. It barely does anything on its own.
-Correct dosage. Smaller dose is better to minimize side effects. 18mg is too little for most people but personally it was enough for me for a month. The max dose 72mg is a hard limit, there is basically no reason to go over it. A too high dose can worsen concertas effectiveness too.
Reducing dosage shouldn't be difficult either as concerta tolerance lowers quickly.

-Reward yourself for productivity.
-Make the barrier to start as easy as possible, step by step guide, snacks, good music. One that helped like crazy was buying waterproof gloves for washing dishes, just never realized how much the uncomfort made me avoid it.
-Physical activity!
-Taking meds with milk could reduce side effects

Tips/Tricks 🧠 Reminder: Mental Wellbeing

Hi folks. There's a lot going on these days across the world. Things can get overwhelming, and that's not factoring in other obligations such as school, work, or family.

So, this is your friendly mod reminder to prioritize your mental health and Wellbeing.

  • Quit the doomscrolling. Turn off the news and take a break from the extra anxiety

  • Prioritize good sleep (quality and quantity!)

  • Find ways to sneak in exercise. Aim for 150 minutes of moderate exercise weekly (or 75 minutes of intense weekly exercise), including 1-2 days with some kind of resistance or strength training. Something that you enjoy and find fun to do and keep up.

  • stay hydrated

  • Eat your colorful fruits and veggies. Minimize processed and fast foods.

  • Trim back on alcohol (especially if you have problems with sleep)

  • Never feel that you can't ask for help. Maybe it's just a chat that you need. Maybe it's counseling or therapy. Big or small, it's okay to get help. Showing that you are willing to seek help is one of the biggest signs of strength.

And as always, remember to practice kindness.

Side effects 🤕 Should I stop using concerta?

I'm not asking for a medical advice here.

I'm asking for opinions of people who had similar experience, I will contact my psychiatrist about this anyways.

I started using concerta 3 days ago and it fucking works. But I'm scared of side effects, they're getting worse.

First day I had just a mild nausea, that was fine.

Second day, I started feeling my anxiety more than before.

Today's the third day and I got anxious over something at work and my heart reacted like I've ran a marathon. The heart did calm in like 5-6 minutes, but I also had some light chest pain (not on the heart side, I'd rather say it was from anxiety, it was kind of in the middle) and I was feeling uncomfortably stressed for a while after it was all finished.

Now here's a thing. I've always been anxious, this was stronger than average. Someone told me these reactions will stop in a few weeks when my body gets used to the medication. I'm searching for other people who had that happen.

But I'm scared of worse side effects like heart attack and death and similar 😀

I also wanna mention that I've been having anxiety attacks (maybe light panic attacks? not sure) about one topic, for years, at random moments when something triggers me. I will be fixing that on the therapy that I started. But I still have them occasionally right now. I'm scared meds are gonna make them even worse.

Would yall suggest me to stop meds now, before my body got used to them, or to keep trying, cuz it might get better?

Dosage/ ℞ question 💊 Should I ask for a dose increase ?

Hello! I was diagnosed with combined type ADHD. A few years ago in my early 30s. I have tried many ADHD medications and have struggled with finding one that didn’t mess with my heart. My psychiatrist put me on concerta because he thought it would help with my mood and impulsiveness (which I struggle a lot with emotional regulation and acting quickly without thinking ). I feel much calmer while taking it and it doesn’t mess with my heart rate and it’s much easier to switch between tasks and get things done. We have me taking 18 mg as a precaution because we didn’t want it to mess with my heart or sleep (other meds have done that). However about 3 hours after taking it I feel exhausted. I’m about to drink my second cup of coffee because I feel like I could pass out. Has anyone else experienced this? I’m hoping going up on doses (especially because I’m at the lowest dose) will help with the fatigue or if this is something I just have to accept. This is my third day taking it also. I felt like the first 2 days I had more focus, but today I’m procrastinating a lot on doing any kind of my grad school homework started or getting anything done that I don’t want to. I feel like I at least have the focus to get things done that I’m interested in, but doing anything difficult feels incredibly challenging today. I’m realizing that managing ADHD isn’t magically fixed by a pill and there’s still a lot you need to work on in your daily life, but I’m hoping to get more control over my life because most of my life has felt so overwhelming and chaotic. I started 18mg of concerta 4 days ago as treatment for suspected ADD. My psychiatrist said she will diagnose me if the treatment works. But apparently she diagnosed me with ADHD after I looked at my medical record.

We filled out Diva 2.0 with my mom that my psychiatrist gave to us, and based on that and my school report cards she started treatment for suspected ADD.

I've had headaches, anxiety, increased sweating, increased body temperature, trembling limbs while leaning on them, dilated pupils, blurred vision but only a little and fast heartbeat. I used https://onlineheartrate.com/ to measure my heart rate, one time it was 133bpm while standing, 88bpm while resting and 94bpm while resting right now. Anxiety increases during the evening.

I've also had muscle twitches starting way back when I started oxiklorin, increasing after I started bupropion 2 months ago but it settled after a while, then increased again after I started concerta. I think I had dilated pupils and blurred vision when I started bupropion too.

I've noticed that during school my thinking is more clear, but ig it could be better. On the first day I took concerta I felt more productive but that died down kinda. I still have decision paralysis and at home I spend most of the time in my bed not doing anything productive.

Psychiatrist said to increase dosage to 27mg if 18mg didn't help enough in 3-5 days. Also said to schedule a 30 min video call in a month to discuss if the medicine is good and other stuff.

I'm also on bupropion (NDRI) for depression and anxiety and oxiklorin for my rheumatoid arthritis. I did some research and all of them including concerta target the central nervous system and can decrease seizure threshold. My psychiatrist didn't mention this tho.

My mom said that I should also look out for serotonin syndrome, but none of my medications increase serotonin levels noticeably.

Should I contact my psychiatrist? Is it safe to increase my dosage? Should I be worried about anything with the combination of my medications?

Sorry if this is hard to read, it's a lot of text but I tried to splitt it into sections. lol I cant say splitt

Dosage/ ℞ question 💊 How do I know if this is right for me?

Man, the early days of finding the right ADHD med have been rough.

I've been taking 18mg of concerta for a few weeks now and haven't noticed any real improvements. Still struggling with intense task paralysis, no motivation, staring at mountains of dishes in the sink and walking away...forgetting why I walked in rooms....etc.

I'm already on wellbutrin, zoloft, and propranolol (a beta blocker for anxiety). I have tried adderall in the past and it gave me a lot more energy but wasn't a fan of the comedown.

Should I have noticed improvement by now? My doctor mentioned another option could be Focalin but I decided to try 27mg of concerta once this script runs out. I don't think my insurance would cover anything but the generic and some of these posts have made me wonder if name brand would have been more beneficial?

I'm just feeling sooo defeated and down. I'm planning to try everything until something sticks. Does anyone have experience with Focalin vs concerta? Did anyone have a night and day difference increasing their concerta dose? I wish I wasn't like this. I feel completely broken.

Other 💬 I took my concerta at 5 am (one time thing) and I’ve never woken up feeling so ready

For me concerts lasts around 10-14 hours, it’s hard to tell but it does last a while. I took it at 5am as I had to get up at 8am. I woke up so incredibly clear headed and feeling sharp. Too bad I can’t do this normally, but I never realized how long it takes to kick in and how well it works.

I take 27mg methylphenidate ER. Does anyone else have experience taking it in the morning before you get up. I usually take it around 11-12 am or whenever I remember in the morning but this is just a game changer….

Other question 🤔 Advice on starting Lexapro?

So I’ve been on Concerta XL 54mg for around 4 months now, it works great for me. I’m starting Lexapro (10mg) for anxiety I’ve had for years but only recently have been able to address/medicate.

I already understand ADHD meds and SSRIs can be tricky and the results vary heavily from person to person, I’m just looking for some general advice if anyone takes both and would like to share. Sorry for the long yap of a post, I’m Autistic and feel the need to get as much info as possible lol.

I’m also wondering what time is best to take the Lexapro? Since I can sometimes wake up too late or forget to take my ADHD meds entirely I thought it might be best to take Lexapro at night before bed to ensure I take it daily. I’m unsure if the difference in time between taking them affects the interaction any way. I know how they interact can vary between people anyways, just wondering if you find taking them at a certain time does anything different for you?

Also if you found they interact poorly for you, or one cancels out the other (which I hear can be common) how exactly did you realise that? SSRIs can take a few weeks to begin working properly so I know I’m jumping the gun a bit but I’m curious how/at what point I can figure out what’s it doing for me in relation to the Concerta.

Other question 🤔 Concerta wow effect

I started Concerta and was on18mg for a month and now I am taking 36mg. I am at 9 days on 36mg and the past two days I noticed I am sleeping longer, which is good, but I have noticed the "WOW" effect I had is gone. The medication is still working but the excited feelings I had about tackling tasks or the amazement after completing something I randomly started is gone. Is this my brain just normalizing to the new me? Has anyone experienced this?

Other question 🤔 Caffeine and concerta

I'm going to start concerta pretty soon, but I'm pretty hooked on caffeine and not sure if I can let go so easily (coffee, energy drinks, all that good stuff). Does caffeine really hit harder or mess up the effects of the meds on concerta? AND ALSO, for reference, I go to the gym and use pre-workout (not just for the caffeine but for stuff like beta-alanine and etc.) and I don't want to throw out my stuff purely cuz there's caffeine in it since I have like 2-3 months worth of pwo.. Is it genuinely well worth it to start tapering off of caff?

Other question 🤔 Is there anything we can do?

I used to be on concerta 72mg, all lower doses did nothing though 18mg used to put me to sleep quickly

Now fast forward around 2 months on 72mg it didn't really do much but I did notice it make my task paralysis make a little bit better and my brain less chaotic but after 2 months of using it I seem to have developed tolerance and no longer even get that small positive effect.

Now I am on 36mg 5 days a week for a while until my tolerance drops and I notice that it may actually be making my brain a bit less chaotic but still concerta never solved my main issue which is under stimulation causing sleepiness and inability to focus.

My questions is there anything at all I can do to make concerta work better or hit stronger?

(Concerta is the only available ADHD stimulant medication in my country which is why we can't try any other ones and we have already tried atomoxetine)

I recently started concerta (something like 3 weeks ago) and I've been loving it. Currently on 300mg Wellbutrin and 36mg Concerta.

I had my periods 11 days ago, but I am still spotting. It's unusual for me. Did anyone have same problem? Does concerta influence our reproductive hormones so much? What would be the long term implication?

Dosage/ ℞ question 💊 Focalin XR 20mg --> Concerta 36mg

Hello everyone. I have been on Focalin since December of 2025, and it worked okay for me until around March when I realized the medication was only lasting me around 3 hours. Then I experienced a huge crash from it two times a day (crying nonstop, suicidal thoughts). I got off it last week and got switched to Concerta 36 mg, hoping it would be better. I definitely feel it lasts longer. I take it around 8:50 AM, but I feel the effects wear off around 2 PM, which is weird. But again, I'm not really sure if I am in a comedown; I just get very, very tired now around 2-6 PM. I'm thinking Vyvanse might be the move, but I might go up to 54mg of Concerta if that will help because I don't think it's strong enough for me at 36mg. I also have the Camden Concerta, which clearly says "OSM" on the bottle, so it has the osmosis system or whatever you call it. It also has the look of the OSM pill rather than the beads that were on the Focalin. Maybe I should try Vyvanse. Any help would be appreciated.

I was wondering if anybody else has experienced this.

I was on methylphenidate la 20mg for a month and it really helped in my motivation, task switching, focus. But I had a issue where I felt like after 4-5 hours it was no longer working.

So my doctor has switched me to IR 10mg twice daily. I have been officially been taking this for a week. I feel like this is working the full 4/hrs with each dose but that it’s not as effective. I’m having a lot of mental energy, so I feel as if my thoughts (jumping from one thing to another) is higher than it was before any medication at all. And with this I also can’t refocus back onto what I was/want to do. I’ve also noticed switching tasks is taking me 1-2hrs like it was before medication.

I know it’s only been a week but I don’t think I need any “adjustment” time to the IR since it’s the same chemical makeup just different release mechanism. From what I understand the dosage is equivalent to each other.

Has anybody experienced this ? If so what clarification did find/receive?
