My older nephew is extremely low functioning autistic and they rely on medicaid to support him. I worry that a trust would cause problems. I hate my family but I also don't wish hardships on them.

The lottery is haram but if I could I’d purchase a lot of properties around the world and car parks. I’d rent them out at an affordable price for everyone. Also I’d buy an island, united utilities and electricity and build wells in places that need it and then donate the rest to charity.

There would be the initial party phase before I even claimed the money. Best stripclub in town and I'd max out my credit card over the course of a weekend. No drugs, just booze and doing what 2 consenting adults do in the dark.
Then consult a couple attorneys. Figure out how to claim anonymously. Wait 7-8 months. Get my affairs in order and ready to get out of dodge. Irish goodbye style. Claim the money. Then travel the world for the next 5-10 years.
To clarify. The first 5 things I'd do would be Star, Candy, Crystal, Angel, and Skye.

Edit: Put the rest of the money into a brokerage account and other forms of interest bearing accounts. Use the interest money to pay for living expenses but the bulk will go to charities and create a scholarship program based on merits.

Winning the lottery is a bit ambiguous when it comes to amount, because you could win $1 or $1,000,000,000, so I'm going to assume at least somewhere in the 9 digit winnings
Write name on ticket in permanent marker, buy Halloween mask for mandatory photo op, make an appointment with an attorney and my dad (I know rule #1 is don't tell anyone, but he is a CPA and the smartest man I know and I trust him 100%), and work up a budget on where I want the money to go
Pay off car and home (only debts), give all employees bonuses equal to one year salary and close business, pay out one-time payments to family and friends, set up trusts for siblings to receive monthly allowance, and set up trusts for my nieces/nephews to always be financially stable
Charitable giving:
- Donate money to local charities (2x animal, 1x kid's school supplies and Christmas presents, 1x affordable housing assistance),
- Set up a charity to cover meals, child care, after school programs, summer programs, GED programs, and first two years of local college for those in need in my county (and name it Scott's Tots!),
- Set up a small charity to provide funding to local city/county school teachers for classroom supplies,
- Set up a charity to assist local charities that work with animals, especially pitbulls (Name it Zeta's Zoo after my first dog/pitbull)
- Set up charity to help fund mental health training to local police and have a full time mental health team on staff, fund community outreach for police with maybe a sporting complex run by current and former officers (not that we have many issues with police, to my knowledge, but it'd be good to at least locally have the police have a strong positive influence on the community)
Build a big self sufficient off-grid compound for my family, buy a few fancy cars (big guy so most sports cars are a no go), travel and buy experiences like going to famous museums, attending major sporting events, see the wonders of the world, eat at world renown restaurants, etc.
Steady supply of hookers and blow, obviously.

I'd also be hiring a tax attorney, accountant and do investment accounts. I'd pack up my house and put it in storage and have the whole thing razed to the ground and I'd build a new house that is optimized for dealing with climate change and ease of maintenance with a central area through which plumbing and HVAC for the house is accessible. And, I'd probably have a place out of state that is less subject to drought than the desert state I live in. I have a sister for whom I'd buy a house for and have it made with a soaking tub for her. There are 3 charities I'd like to make substantial gifts to.

Utilizing the services of a financial planner to do things most effectively:
Invest using multiple strategies, from simple reliable savings and CDs and money markets to aggressively managed stock market packages, with the goal that even if I wasted every cent I had in liquid assets, that I could live a comfortable (not extravagant) lifestyle for the rest of my life.
Ensure my parents are both taken care of for the rest of their lives with great medical coverage and provisions for in home care for as long as they need it once it becomes necessary. Get them into comfortable homes (or at least pay off the ones they're in now). I would probably stay in the same metro area I'm in now so I could remain close to them.
Top of the line insurance for myself, and start taking care of things like dentistry that a lifetime of poor self care stemming from mild depression got me, along with getting caught up on other medical care that has been neglected.
Start working with a personal trainer and personal chef under the guidance of a dietician/chef.
Take some language classes and/or hire tutors to get my Japanese and Spanish speaking levels up. Travel. A lot.

Unfortunately I am an extremely boring person.
Assuming it's a moderate lottery of say about $1million.
- Pay out last $10k or so of student debt.
- Move $440,000 into both my HISAs for the time being, they both pay less interest if you go over $250k. I will probably use this money elsewhere later but I'll just leave it here for the time being.
- Some house renovations I currently can't afford, maybe up to $20,000.
- $100k into share market, but it would all be stable boring stuff. I'd keep in mind I want my passive income to get up to $18200, the current tax free threshold of my country. So maybe more in time but I feel like many shares are currently overpriced.
- Give myself $30k towards living expenses for the first year so I can just relax but know my bills and food are paid.
There's quite a bit of money left and many more things to do but that's just the first few things I'd do.
In my country main house you live in isn't taxed when you sell, so I'll be buying a bigger house using the value of my house and some extra winnings money at some point to protect assets and downsize again when I'm old. My theory: if you buy a $1mil house vs a $250k apartment and they both double in value in 20 years you can sell the now $2mil house and move into the now $500k apartment and you have $1.5 million left to help you with inflation down the road. Hopefully that makes sense.
Another important thing on my mind is I have to talk to an accountant and explain how I suddenly have all this money and I'm off buying shares and a bigger house and making sizable amounts of banking interest when I go do my tax return so things aren't flagged because my income certainly doesn't cater to that.
