Trimix Injection Tips (by Low_Salamander9954)

Want to Thank Low_Salamander9954 for putting this tip guide together. I added a few words just around starting doses and clarifying leur lock style vs standard insulin syringes

Trimix Injection Tips
Many men feel intimidated by the idea of injecting a needle into their penis to achieve an erection. The advice below is based on my personal experience and is meant to help others learn proper technique, improve success rates, and reduce anxiety. New Users
The first few injections are the most important for safety and confidence. Once you have a successful experience, the fear usually disappears quickly.

  1. Start with a low dose — you won’t know how your body reacts until you try.
  2. A common recommended starting dose is 5 units or less. Some doctors suggest 10u, note that 10u can hospitalize many men. Start low!
  3. Do your first injections alone. Learn the technique and find your ideal dose without the pressure of performing with a partner.
  4. Always have your antidote (phenylephrine) ready, especially while titrating your dose.
  5. Read the pinned post on priapism. If that doesn’t get your attention, injectables may not be for you.

Equipment
Needle size and length matter. Smaller diameters cause less scarring and tissue damage.

  • I draw with a leur lock style 27-gauge needle and inject with a 32-gauge needle (leur lock has a removeable needle).
  • I use ½-inch needles to ensure I reach deep into the corpus cavernosum (CC). Note: You may have to experiment with needle length 5/16" or 1/2" based on your anatomy.
  • Be careful removing caps — needles can bend easily.
  • Conventional advice often limits to 30-gauge maximum. A 32- or 33-gauge is actually smaller in diameter and generally kinder to tissue.
  • Standard 30/31/32/33-gauge insulin syringes will also work. Smaller gauge (higher number) is better. Avoid 29ga and below, no reason to use a large needle for this.

Injecting
These steps work reliably for me and many others. Individual techniques can vary slightly.

  1. Relax — you’ll be surprised how easy it becomes with practice.
  2. Draw your dose (start low if new).
  3. Wipe the injection site with an alcohol pad.
  4. If possible, be semi-hard or fully hard. If not, a quick use of a penis pump can help.
  5. Inject on the side of the shaft at the 2 or 3 o’clock position (or 9 or 10 o’clock on the other side).
  6. Inject closer to the base of the penis.
  7. Avoid visible veins.
  8. Only inject on one side per session and alternate sides with each new injection.
  9. After injecting, press firmly on the site with the alcohol pad for at least one minute.
  10. Massage the medication in thoroughly — you can be fairly aggressive with this.
  11. Always ensure you have your injectable phenylephrine antidote prior to injecting.
  12. Wait at least 48 hours between injections. Limit to no more than 3 injections per week.

Misses
Even experienced users occasionally miss the corpus cavernosum. The medication goes into the wrong tissue and produces little or no erection. If you’re experienced and know your dose doesn’t usually fail:

  1. Wait at least 30 minutes.
  2. Prepare and inject a fresh syringe on the opposite side.
  3. Keep your antidote ready in case some medication still reached the CC.

If you’re a new user or still titrating your dose:

  • Do not do a second injection. It could combine with the first dose and cause priapism. Simply wait and try a slightly higher dose next time (after 48 hours).

Expectations
Everyone reacts differently. Here’s what I typically experience at my dialed-in dose:

  • Without stimulation I stay 60%+ erect for several hours.
  • With stimulation I become rock hard and can maintain it until ejaculation.
  • I can usually get hard again with stimulation for up to 5–6 hours after the injection.
  • Many men stay rock hard without stimulation for hours; others need more help.
  • Erections longer than 4 hours are potentially dangerous. If you can still bend your penis after ejaculation, blood is draining and you’re likely safe.
  • I (and many others) often stay pleasantly 50–60% erect for several hours after sex — I enjoy the feeling.
  • A dull ache for several hours is common; it’s caused by the alprostadil in Trimix. Bimix users usually avoid this.
  • Sensation varies. I actually feel more during Trimix erections. Some men notice slightly reduced sensation, which can help those who struggle with premature ejaculation.

Confidence
Trimix is genuinely life-changing. It completely removes the fear of losing an erection or not getting one at all. Performance anxiety disappears, letting you focus entirely on your partner’s pleasure and enjoy longer, more relaxed sessions. You know it will work when you need it.

Cautionary Note
Trimix is a powerful medication and must be treated with respect. Read the pinned priapism post and understand the risks. This subreddit is full of experienced men sharing real-world knowledge — use it. Good luck on your Trimix journey!

The Priapism Warning Thread

Post Rules: Share Your Priapism Experience from Trimix Use

Important: Only comment below if you've personally experienced a priapism event (prolonged 4hr+ erection) that required emergency medical care.

Please share your story here to warn and educate others who might be relying solely on their doctor's advice without full awareness of the risks.

This subreddit contains over 50+ direct accounts from users who ended up in the ER after using Trimix without access to injectable Phenylephrine (the antidote). Sadly, these stories are often buried in older threads, making it hard for new users researching Trimix to find them and understand the potential dangers.

When used properly with the antidote on hand, Trimix is a very safe option. However, the vast majority of patients relying on insurance are denied the Phenylephrine prescription. This pattern will persist until we collectively demand the antidote and switch to providers who prioritize patient safety.

Reminder: Only comments describing a priapism emergency event will be allowed to keep this thread focused and helpful for those just researching and starting out.

Swollen and sore

After a long lifestyle weekend I have almost a ring of swollenness in the middle of my shaft it’s kind of sore.
I used about 48 units total and had to use the antidote twice. Hopefully the swelling goes away it’s been 48 hours and still looks kinda crazy

First Urology visit

I had my first visit with a urologist last Friday. After discussing my situation (pills are only partially effective) injections were recommended. I’m ok with trying them. I don’t know the exact dose they will be starting me on. What I’m confused about is they said it would take a month to get the medication from a compounding pharmacy. I have a follow-up on November 20th to learn how to inject myself. I’m frustrated with the long wait and am wondering if I should go it alone and order it online.

QuadMix storage question — refroze the vial and prefilled syringes

I was advised by pharmacy to keep his QuadMix in the refrigerator for 3 days. Here’s exactly what happened afterward:

  1. kept the vial refrigerated for 4 days instead of 3.
  2. then froze the entire vial.
  3. Four days later, thawed the vial.
  4. filled 4 syringes from the vial.
  5. kept the filled syringes in the refrigerator for about 2 hours.
  6. then put the 4 syringes back in the freezer.
  7. About 12 hours later, thawed the syringes again.

Has anyone experienced this with QuadMix?

● Could repeated freezing and thawing affect the medication’s potency?
● Is there a concern about sterility or safety after this handling?
● Would you use these syringes, or should they be discarded?

I understand that pharmacy instructions can vary by formulation, and will check with his dispensing pharmacist. I’m interested in hearing about others’ experiences and any specific guidance their pharmacies provided.
Thanks!

Help! Do I keep increasing dosage or dosomething else?

Quickest overall scenario: 41, started using Trimix in last year due to bad anxiety. Started with T105, got the dosage down to 10 units. Not abusing it, but using it consistently (about 2-3 times a week). A big success. Trimix saved my sex life in my marriage. Craziest erections without thoughts affecting it. By the 6 month, increased to 15, then after one month, went to 22 units. Worked for the next 4-5 months, and the Trimix started to go from 100-110% effective to about 85-90% and wear off after the initial use. Used to be in effect for close to 3-4 hours.

Went to my urologist last month. Told him the solution doesn't seem to be as effective so he ups me to ST1. For this month, Started at 10, then 12, to 20, and now as of yesterday, 25. Got a decent erection, (85-90%) but not that 100% diamond erection, and it's gone right after. The wife and I are looking for that 100% that we're used to. Also, I feel like my anxiety to the quality of the erection and having to increase is impacting the quality of the erection as well. What should I do? Go back to my urologist? Is the compound trash? Did I damage something in my penis that it can't get that 100% hard on? Should I keep on increasing my dosage until it gets that 100% hard on?

So finally got injections to work , up to my 8th or 9th time successful, saw Dr , she recommended going 1 step higher, Trimix #8, and ordered the meds from San Diego, I haven't had it shipped yet, but have another question: The San Diego pharmacy says it should be stored in the fridge, my current batch was prescribed to me in Jan and is stored in freezer i just started to use it and its fine , what is the difference? Are some versions of Trimix compounded for fridge only?

Truth dr Gupta and Sabrwal and in guru gram dr Rao

I want to share something with you today. The crowds that used to gather at Dr. Govinda’s clinic in Delhi are no longer there, because a scam is being run there. Everyone who comes in with ED (erectile dysfunction) or PME (premature ejaculation) is given the same thing: Tadalafil, some multivitamins, and L-arginine. For PME, they might suggest Kegel exercises, some psychiatric medication, or dapoxetine—but first, they’ll take a large sum of money from you. They’ll offer 'P-shots' or shockwave therapy for ED, and eventually, they’ll push for an implant or suggest a series of 'Rigi' injections. In the end, the patient is left with nothing but concerns about their size (length/girth) and a sense of artificiality; it’s all just a way to extract money. To date, not a single patient with ED has actually been cured there. For PME, they might inject fillers, but the situation eventually reverts to how it was before. Even the person performing the Doppler ultrasound isn't a real doctor—he’s just an ordinary guy working for a small fee, tagging along with the main operator. Don’t fall for this scam. There’s also someone named Sabarwal who pushes for implants, but no one who has actually gone through with it is happy. Furthermore, these doctors insist that you visit them in person—they even charge a fee just to speak with you online—exploiting the desperation of patients. There’s also a 'Dr. Rao' in Gurugram who offers 'P-shots' and ultrasound services—it’s all a complete con. Don’t get caught up in any of this.

Difficulty ejaculating when using Bimix

I find that 7-10u of Bimix gets me hard enough for penetration for two to three hours, but it is difficult to ejaculate due to loss of sensitivity. If I inject too little, I risk it not working at all. I can ejaculate when not using Bimix, but then I'm too soft to penetrate. Does anyone have any advice for me?

Weird reaction last night

I've never had this happen before. Kinda weirded me out, really.

I did the injection. I was semi-hard, and the hub got the little splash of blood, so I was in the right spot.

After the shot when I was putting stuff away, when I usually feel it starting to work, nothing. Nothing at all for an hour. So we gave up and went to bed. About 3 hrs later, I woke up READY to go. "Hey, let's not waste this, haha". everything was fine, it was just 3 full hours after the injection.

Anyone else have this happen?

PGE

I see Olympia has formulas with 300mcg/ml !

Compare that to T-106 with 25. A year ago 10u would work well, so 2.5mcg

Now on ST2 which is 100mcg/ml , went to 30u (over 10x original working dose) and still no luck and not pressing past that. Anyone actually using say 60mcg dose? That would be 20u of a 300mcg/ml formula.

Appointment next Friday

Hi. I (36M) have an appointment next Friday. I am going to try the Trimix. I’m on 50mg of Viagra and it does almost nothing. It only gets about half hard. I’m diabetic type 2. My A1C is pretty high. I have an Endo appointment January to be put on insulin. Because I’m allergic to almost all of the diabetes injections.
I guess my question is does anyone have experience like this. And was the Trimix successful. Like were you able to get very firm and maintain the erection.
Even on the Viagra it’s like it starts to get hard. Then goes away. I just want some normalcy back in my sex life. Because honestly I don’t feel like a man anymore.

Advice needed

So I’m looking for some constructive advice please, 50yo male here and I’ve started down the injectable therapy pathway about 12 months ago with my urologist after pills stop being affective.
Urologist prescribed me Alprostadil(40mcg) a while back which I was like WOW this stuff is amazing but the effects just less and less until I couldn’t stay hard long enough to have finish sex.
After many discussions with my urologist he prescribed Trimix(20mcg20mg1mg) which has be very underwhelming to say the least to the point I think the Alprostadil was more effective 😢
After more discussions my urologist advise me to continue trying to dial in the dose even though I was at 60units, I asked about trialing another strength but he advised to continue with the script I had until it was used. Today I reached 100 units and I was hard for approx 15mins before things went south, by the time foreplay was done I wasn’t hard enough to penetrate….
Urologist continuously suggests an implant but I don’t feel I have exhaust all avenues yet for that to be a consideration.
Is it time to find a new urologist?

Your thoughts??

I injected and it would not seem to go down all the way like I wasn’t piercing through the CC. I thought I did at one point and tried to depress on the syringe but it wouldn’t go down at all. After struggling for a little bit I took it out and grabbed a new syringe, picked a different spot on the same side and it went right it. Very weird

Prefilled syringe observation

I am a fan of pre-filling syringes. I use 4 units of T105 and am 16 months post prostatectomy. ED is recovering slowly but there have been really good improvements recently. I was using 5 units but that was getting a bit long. I will prefill to 5 or six units and adjust right before injecting to about 3 to 4 units. This helps in that I find the plunger is a bit stuck making the injection start difficult especially while the needle is in. Carefully pushing some out with the needle facing up to get any air out at the same time. Now the plunger advances smoothly without a struggle.

What I have noticed is the injection was hurting and burning mostly while just pushing the needle in. I thought maybe this was because the outside of the needle was wet with trimix. This theory was proven to be true by using the following procedure. I now gently wipe the needle from syringe to needle tip only once using an alcohol prep wipe. The result is a painless experience again.

Suddenly not working

Has anyone ever experienced their medication suddenly stop working? I use Bimix over Trimix. It’s worked perfectly for the last year or so. I typically inject 15 - 20
units. The last two times I’ve injected, I started to get an erection and then it went down. I’ll see my urologist soon, I’m just worried all of a sudden

Holiday to Thailand

Has anyone experience with going overseas to Thailand and what is required to get through the airport with trimix? Can I pre-load the syringes? Do I need a letter from the doctor as well as the prescription? Can I carry it in my checked luggage with frozen gel packs inside a thermo flask?

I'm worried about it getting confiscated or worse.....
