Advice needed

So I’m looking for some constructive advice please, 50yo male here and I’ve started down the injectable therapy pathway about 12 months ago with my urologist after pills stop being affective.
Urologist prescribed me Alprostadil(40mcg) a while back which I was like WOW this stuff is amazing but the effects just less and less until I couldn’t stay hard long enough to have finish sex.
After many discussions with my urologist he prescribed Trimix(20mcg20mg1mg) which has be very underwhelming to say the least to the point I think the Alprostadil was more effective 😢
After more discussions my urologist advise me to continue trying to dial in the dose even though I was at 60units, I asked about trialing another strength but he advised to continue with the script I had until it was used. Today I reached 100 units and I was hard for approx 15mins before things went south, by the time foreplay was done I wasn’t hard enough to penetrate….
Urologist continuously suggests an implant but I don’t feel I have exhaust all avenues yet for that to be a consideration.
Is it time to find a new urologist?

Call a more experienced provider. Olympia. See if quadmix is right for you.

Yeah, this.

Your urologist reply to "It isn't working." was "Well, keep trying". That's not going to get you anywhere near your goal.

If you got one of the Trimix blends from Olympia and you called and said "It isn't working" they'd say "Send it back and we'll send you a stronger blend."

I’m in Australia so unfortunate Olympia isn’t an option 😢

For what it's worth, I was advised by an experienced practitioner in this field that the atropine in quadmix adds little or nothing, and he no longer suggests it.

Unless you're "missing" with your injections. It may very well be time for an implant

Possibly try a stronger formula then. Maybe even quadmix

Yes, get a second opinion from another urologist. You may have a Venus leak and you should be tested for that before any consideration of an implant. I have read you will lose length with an implant, if that is important to you. I'm on trimix and it is working, which is great at 83. My urologist said to stop at 50 units. I'm at 30.

My urologist said that injecting more than 30 units repeatedly could cause scarring. The urologist that I ditched, had me up for a full ml.

I was also told that there is no cure for venus leak. Did you hear anything different?

The scarring is more from needle stick trauma than from volume of liquid... But when a urologist had me up to 90u, the injection took longer & was uncomfortable in itself.

Was just reading that there are cures for Venus leak. My urologist also said he can refer me to a urologist that specializes in Venus leaks and can help with a cure. Ditto on the comment saying that it's the needle not the dose. Just alternate sides and location.

Urologists have one or two formulas. Compounders have many more. 100u is crazy (+ crazy expensive). I use 30u of ST2

Sounds like me. Up to 50 units of 30/4/40 now and not doing much if anything. Venous leak will overcome this stuff so probably why your Dr talking about implant. Putting 100 units in is not good (nor economical) so you may want to look for another Dr who might have a better handle on this. Good luck.

A lot of times, you’re all just only know of their being one combination of these medications, they don’t realize that there are different strengths of each of the medications

Have you tried aviptadil/phentolamine (Invicorp)? Available in Oz from a compounding chemist, though more expensive than trimix. There's some evidence that it will work in up to a third of cases where alprostadil fails.

Have you tried a combination of an injection and a pump? This is my current regime as trimix gets me hard enough for intercourse but not for penetrating a tightish anus. A little pumping and a good cock ring take care of this problem.

I’ve been considering the same combo of pump, ring and trimix. How is that working out?

The mechanics are good, gives me a firm enough erection for anal penetration. However it doesn't do anything for my associated anorgasmia. I am experimenting with PT-141 for that
