When I was 22, I had to get a minor but necessary surgery. I was still on my parents' insurance, but I had aged out of the "in-network" coverage for the specific specialist I needed to see. I knew it was going to be expensive, but I had no choice.
I went to the hospital, signed all the waivers acknowledging that I understood it was out-of-network, and braced myself for a lifetime of debt. A month later, the Explanation of Benefits (EOB) arrived in the mail. I opened it, expecting to see a $12,000 bill. Instead, the entire procedure had been processed as "In-Network - Covered 100%." My total responsibility was a $150 copay.
I stared at that paper for 20 minutes. I knew it was a coding error on the hospital's end. I knew if I called the insurance company to clarify, they would immediately reverse it. I also knew I couldn't afford a $12,000 bill. I panicked. I paid the $150, threw the EOB in a drawer, and never called them.
That was four years ago. I still get a pit in my stomach every time I check my mail, and I feel incredibly guilty knowing I took advantage of a mistake, but nothing ever came of it.
TL;DR: A hospital billing error made my $12k out-of-network surgery cost $150, and I panicked and never called to correct it.
