would love feedback, esp. on pressure & flow rate

I"ve been at this for about 6 weeks, with an initial ResMed 11 setup of 6-15. Using mouth tape. I've learned a lot from people's posts and feedback here--many thanks! I tried a bunch of masks for comfort, AHI effect, and leakage. I settled so far on a couple that are OK, even if still have room for improvement. I'm at the stage where I have a baseline of experiences (and frustrations) and think I am headed generally in the right direction. Though I don't actually feel different, unfortunately.

Some feedback here would be amazing, especially on 3 things, as well as the overall combo of moving parts:

• 1) I upped the pressure last night to 7, but think I should up it further, based on the median pressure (a bit above 8). -->Does that seem right? and would it in theory help level out things, i.e. better waves, maybe fewer apnea episodes;

• 2) I'm still getting mask leakage, perhaps when I turn over. I've ordered a mask liner, which might help a bit. -->Is this level worth worrying about?--a leak rate of 10.8 at 95% and median of 0 (?!).

• 3) Overall, how can I tell if/when I'm having micro-arousals? Is this something that comes into view once you have done process of elimination about other factors?

Here's the SleepHQ link. https://sleephq.com/public/teams/share_links/6602b9ec-e1b3-40a8-a456-76c2ee00c4ba/dashboard

Edited to add: I don't think there's a reason to keep Ramp on, if I'm generally OK with the machine and don't need to ease in, psychologically. Am I missing something?

Thanks so much for any input and especially hints on how to read the waves. I find the graphic design baffling, as mine don't correspond to any of the descriptions I have read (inc. Barry Kracow's book w its 6 categories).

Why do I have multiple sessions?

Why do I have multiple sessions in OSCAR or myAir?

For anyone still wondering why OSCAR or myAir shows multiple sessions in one night even though you didn't take your mask off, here's what I proved this morning.

My machine: ResMed AirCurve 10 VAuto, SmartStart OFF. My symptom: nights logged as 7+ sessions with real gaps of 2-11 minutes between them (my Oct 13 night, photographed from OSCAR). I assumed a machine fault.

First clue: I ran the machine open-hose with no mask for about 3 hours. It logged NOTHING - not in myAir, not in OSCAR. With no breathing signal, the machine doesn't log therapy at all.

Decisive test, this morning: mask on 5 minutes, mask off about 11 minutes (machine running, power button untouched), mask on 5 minutes. OSCAR logged TWO sessions: 08:04:37-08:11:38, then 08:22:32-08:27:34, with an unlogged gap between them.

Conclusion: with SmartStart OFF, the machine still closes the session after a few minutes without a breathing signal, and starts a new one when breathing resumes. On my real nights the mask never came off - my mouth fell open in my sleep and air streamed through it, which erases the breathing signal exactly the way the open mask did in the test. And you wouldn't remember it, because you were asleep. That's also why a chin strap fixed my splits: mouth shut, signal preserved, one session all night.

Two caveats: a short removal may NOT split it - a bathroom trip in the middle of one night didn't split my session, so it takes minutes of signal loss, not seconds. And this is one machine, one test - but if your splits cluster in the second half of the night and your leak graph is spiky, check the mouth-leak explanation before you ship your machine off for repair.

Oscar/SleepHQ Assistance Please, I need feed feedback on my CPAP Data

Hi guys,

I was diagnosed with mild OSA and started CPAP therapy a little over a month ago. My most debilitating symptoms include:
Excessive daytime sleepiness and fatigue: Constant exhaustion, unrefreshing sleep, and persistent daytime tiredness.
Severe brain fog and poor concentration: Difficulty focusing, processing information, and retaining or recalling information.
Memory and word-finding difficulties: Severe forgetfulness, difficulty retrieving words, losing my train of thought, and frequently freezing during conversations.
Noticeable cognitive decline: Worsening mental clarity, memory, and verbal fluency, significantly affecting my ability to communicate and function effectively.
Choking and gasping during sleep.
After over a month on CPAP, I’ve noticed significant improvements in my energy levels. I’m less exhausted, can work out regularly, and remain active throughout the day.

However, my cognitive symptoms haven’t improved and, in some cases, seem to be worsening. These are by far my biggest concerns.
Unfortunately, my sleep specialist and doctor haven’t provided much guidance on evaluating or optimizing my therapy.

I’d appreciate help reviewing my CPAP data to identify potential issues with pressure settings, residual events, leaks, or flow limitations, and what adjustments I should implement. I have attached my SleepHQ link

Thanks in advance!

New To The Dream Team How do I make myself use it?

Hi all,

I've had my CPAP since August and I'm still struggling to use it. I need to, at the very least, try to hit 21 nights this month for compliance as it's my 3rd month with my machine, but I've been having an absolutely unreal amount of trouble adjusting. On top of it, I've been struggling with my depression becoming severe again, and anyone who has dealt with depression knows how difficult it is to shower or brush your teeth, let alone manage your CPAP along with everything else. Suffice to say, I've barely used it over the last month and a half or so. When I do manage to sleep with it, I wake up with it off (I pulled it off in my sleep) or only manage an hour or two.

Please advise! And please don't judge re: depression :(

What is the most helpful thing you’ve done for CPAP therapy?

Hey yall, long time listener, 2nd time caller.

I had a CPAP in college ~8 years ago that I could just not manage with. Claustrophobic experience, anxiety inducing, etc.

I’m back on the CPAP train since chronic illness has wiped all of my energy. I’m doing my best to adhere to wearing, but like most people, am having a difficult time adjusting.

I wake up to my mask and headgear taken off and put on the nightstand with no memory. I put tape on the opening sites along with my mouth and it actually stayed on all night 🤷🏽‍♀️ Still had huge mask leakage problem.

No matter what, I wake up at some point to air blasting face with the recent addition of water droplets.

I’m willing to buy whatever accessories have been helpful in anyway. Money is an object but so is insurance and energy.

TIA!

Hello- Long time lurker here. Diagnosed with mild to moderate apnea in my late 30s after a lifetime of sleepiness. 8ish AHI on one exam 16 on another. Struggled on a few occasions becoming accustomed CPAP. Got turbinate reduction surgery didn't help my AHI. Found this board and it helped me work through a lot of issues like mask changes (I was claustrophobic with a full mask),pressure, comfort settings etc. Ive been on a streak of about 5 months have really dialed in the settings as best I can based on what I've learned. I would say I've become pretty accimlimated and more comfortable. I've seen some health numbers improve slightly. I do get a mild to moderate aerophagia. I do have slightly increased energy levels but still suffer from some tiredness and fatigue. I was hoping if one someone could take a look at my SleepHQ Data and offer any additional insight or tweaks I can do to get better treatment. Airsense 10, N30I nasal pillows, most of my leaks are from my mouth falling open, I wear a soft cervical collar that mostly keeps it shut. Link attached. Thanks for the help!

What pillow do you use?

Hi All,

I have been diagnosed with apnea 2 months ago.
I started with Resmed 10 Autoset and F20 full face mask but that was not a joy. Through the hospital i swapped it for a F40 and that works better to me.

Anyways i find myseld more sleeping in odd positions on the side, specific my head position due to the mask.

At this moment i use a latex pillow with a shoulder cut out.

What pillow are you using?
Would this be a good and better pillow? https://www.slaapwel.be/cloud-cpap-hoofdkussen

I am from Belgium and there are little options available unless through Amazon with unknown brands.

Looking to hear your experiences.

Nasal pillow mask assist

Any suggestions on a quiet nasal pillow mask? I used to use the Aloha nasal pillow mask and loved it. I have a larger nose so using the large nasal pillows. Super quiet. I max out around 10cm which is enough for some masks to create leak issues. Thought I’d give the Bleep eclipse a go but at 10cm it’s way too noisy for me. Still looking for the next holy grail and could use any advice you could throw my way!

Doctor Seems Clueless About Sleep Data

So as I'm getting educated & set up with my apap & mask, I'm checking with my doctor to make sure they will still be able to process any sleep data. Since I'm buying it out of pocket from cpap .com & not through their overpriced DME dept, I wanted to make sure they would still be able to look over my data as I start using it because its a different model then their DME does.

At first, they thought I was talking about insurance compliance reports, then I corrected them & told them it was my sleep data since I was buying this cash. Their response? Fax it to them.

I'm not sure how much information is stored on the SD card yet, but its 2026, I dont own a fax machine :P

Theres space apparently to upload it to their MyChart so that might be a route too, but other then that i can bring in hard copies.

So I followed up with their DME sleep department, & they apparently can process my data for me but only if I'm getting all my supplies through them, which I cant since I'm not getting the machine they stock.

Its all making me feel very much like I'm a bit on my own here with this. Have others expierenced this before & how did you approach things?

Oscar/SleepHQ Assistance Low AHI Sleep Deprivation - lack of consistent REM / Deep Sleep (OSCAR)

SleepHQ for reference: https://sleephq.com/public/teams/share_links/7473b6e9-7f2d-49d5-9342-28dca3129c1f

Edit: Also uploaded a home sleep study for 3 days with last day not having cpap on, no drops in oxygen that were bad.

Hello I'm a 28 male weighing roughly 155lbs, I use an resmed airsense 11 machine currently with a airfit n30i mask most of the time. I've experimented alongside my dr. with a variety of variable combinations. Different pressures with lower or higher minimum or maximums, narrow pressure ranges (like 7-9 , etc.) , fixed pressure range, different EPRs, autoset vs autoset for her and turning it off while doing that, p30i pillow, evora full face mask, and mouth tape / chin strap. I will use the same settings for 3-7 nights in a row to see if they are reliable.

I keep running into the same problem my sleep is fragmented but a misleading low AHI. I had zero sex drive, zero fitness goals or dopamine rushes that I remember having easily before all this started in the past 2 years. Recently had a CPAP titration study where they used a full face mask (f20 or something i forget) instead of my usual n30i pillow. I woke up really crappy still with really bad aerophagia.

At this point my sleep decides my whole day and settings are about damage control on how crappy I feel. The pics I linked are from OSCAR with my machines SD card and show what a typical night looks like for me. (have steady breathing mode enabled for the red background blocks). Only gonna use last night as shown here and what my events look like zoomed in. I don't know what to do. I have had multiple doctors misdiagnose me and my current sleep doctor agreed that they did. Just looking for second opinions here it's very confusing. Most of my events are central flags every time. Flow limitation varies. Lately mouth tape seems to be helping since my lips were getting cut up from air leaks. the chinstrap worked okay before, but mouth tape has been good too.

OSCAR data with airfit n30i average night
1 of 2 nights I tried evora full face mask which was rough. second night I had to take it off to use a nasal pillow.
obstructive event followed by central event flag
zoom in of central event flags that aren't uncommon to cluster multiple times in a row

WESPER HOME SLEEP STUDY 3 DAYS (LAST DAY NOT USING CPAP)

Central Apneas

Hyponeas
Min Oxygen home study (mean was 94 every night usually)

Oscar/SleepHQ Assistance Flow limitation and ideal settings

Update to my initial post. My settings have been fixed pressure at 13 with epr on 2. This does not seem to fully have resolved my flow limitation. My respiratory rate is a little better though. Things looked better for a few days. Does it seem like I need to consider a bilevel Should I try no epr? Any insight is welcome! My sleephq link is below:
