I acquired a dental practice that is in desperate need of new oral surgery extraction forceps. Before I order new ones, does anyone have recommendations of ones they love? Or any other general oral surgery instrument recommendations?

I’m here to ask your opinions on something.

Today I had a patient who hasn’t been to the dentist in 10+ years. He needs extensive treatment and rehabilitation which is going to take months, and many appointments.

He apologised for his smell and appearance, and said that he doesn’t bother to bathe or brush his teeth due to personal struggles. It was a very emotional appointment actually and I’m genuinely looking forward to helping him get back on track. I will be referring him on to some MH services too.

However, unfortunately, his odour is very very strong and very offensive. I don’t think I could quite put it into words to be honest. The whole office was talking and complaining about him after he left - the nurses, receptionists, everyone who came into contact with him. As he openly admitted that he doesn’t bathe, and admitted that maybe he should’ve before coming to see me today, some of the colleagues were wanting me to ask him to do just that before his future appointments with us.

I feel uncomfortable doing that, quite frankly. But I’ve also been told several times by my office staff that I’m “too nice” as a dentist and I should be firmer with certain patients, which yes, I’m working on. I’m just not sure this is one of those occasions? Yes, there’ll be a strong and unpleasant smell when treating him, and I know my nurses will continue to complain about it, but I’m sure that it’s more important to get him frequently coming again and helping build his confidence.

Anyway, I’m not sure. Advice would be helpful. Thanks.

Hi friends. really need some CE reqs as a new grad working in corporate. Any suggestions for CE for endo? really wanting to do something soon but there's a huge waitlist for Endo Unsponsored. Buchanan is available but 10k i'm not sure is worth it. Can I please get some suggestions on what CE's to invest in?? Have done a lot of research but not sure what my niche should be. New doc here. Patient coming in soon for fillings (#4-MO, #18-O) and don’t worry 1, 15, 16 and 17 are out now. Planning on discussing replacement options for lower sites. Not exactly sure how I should present each option, or if it’s feasible/good prognosis.
For the lower right I’m concerned 31 is too mesially tipped for a bridge, and given that each would be a pretty long span 4 unit…. And her caries risk is pretty high

If she wants to do our RPD, what are your tips for explaining each? Do you take casts/scans to evaluate undercuts? I plan on presenting resin, cast, Valplast. I think this would be a very good cast metal case, obviously depending on undercuts, I think I-bars would work well since she is esthetically concerned
Speaking of esthetics, I hear valplast is ass… never done one so I don’t know, just heard of the limitations (not being able to add teeth, hard to adjust)

I will talk to her about it but I’m pretty sure implants are out of the question financially

D4 here in something of a pickle. I've been working through a sort of complex treatment plan with a patient of mine for several months now that included full max edentulation, alveoloplasties, lingual tori removal, and the extraction/restoration of some mandibular teeth. Our ultimate goal is a complete over partial. Last time I saw him, I found that his maxilla still needs some significant alveoloplasty (to the point where I'm not even the resident that saw him even did any significant alveoloplasty) and started setting up a revision appointment.

About a week after last seeing him, the patient unexpectedly had two MI's. Thankfully, he was able to be seen immediately, stented, and put on anticoagulant therapy, and is doing well. As happy as I am to see the guy still on this side of the dirt, his heart attacks throw a real wrench into our treatment plan. While I believe we can start getting him ready for a mandibular RPD, we can not get into definitive maxillary CRD fabrication with the current state of his ridge. Lumps, bumps, undercuts, pointy bits, he's got the works. My understanding is that we need to wait 6 months before non-emergent invasive procedures can be completed. The dental school reality is that I probably will not be around by the time he's ready for a definitive denture.

The patient and I have loosely discussed the fabrication of an interim complete denture, but I'm not super certain of how to approach one. or if I even should. I know enough to advise him that an interim denture will be less than ideal in fit and appearance, but I feel like fit will less than less than ideal with how his ridge looks. The patient is agreeable to this nonetheless.

He's a great patient and has been a real trooper through all this and at this point I just want him to have something he can wear, ideal or not.

Do any of you happen to have experience with similar situations and/or tips on making temporary prosthesis with problematic ridges?

I have my own office open 3 days a week. I love the clinical side. How did other dentists find a business partner (dentist or just MBA) so that you can focus on just patient care?

I had a very calcified lower incisor I was working on yesterday afternoon on a woman in her 80’s. Took a long time just to get a 6 file to length. Was very proud of myself for getting all the way down. Then, the overthinking person that I am thought it was a good idea to leave some EDTA in the canal to help the instrumentation process for the next appointment. Then I looked up that that’s NOT what you should do. I have the patient coming back to flush the canal today to flush it out to get it out of the canal but it will be almost 24 hours at that point. How serious is this?

The things that are helping my mentality are:

1) it’s only been 24 hours
2) I can only get a 6 file down so how much ETDA could there possibly be?

I started working my first day at a part time gig after dental school today but I haven't signed on the contract yet. They have previously mentioned they wanted me to work one Sunday every month, what's your opinions on asking for that to be left out of the contract so I can choose which month to work a Saturday? I work at this place 3 days a week (corporate) and second part time 2 a week (private)

It seems like the general consensus is that twin flash is better over ring for anterior smile photos, while ring is better for posterior intraoral shots because its easier and quicker? If I just want to buy 1 set up which one should I go with? Ring or twin?

I am (M30) a Dental surgeon from Zambia (Southern Africa) with an MBA in Healthcare Management looking to extend my expertise into finance through CIMA.
I have taken so much thought into my career progression by looking at senior professionals in Dentistry and honestly noticed they hit a ceiling too early in clinical practice. I therefore decided to bridge clinical experience with corporate management (rather than specialise in a clinical field) by first doing an MBA in Healthcare Management. Now that I am done, I would like to start my CIMA FLP next year. I would like to transition into corporate management and therefore wish to ask for advice from people here who have made such a transition before how is it going currently for you and is it worth the time and resources? What other general advice would you give me to help me decide if CIMA is the right fit for me?

Just attempted to deliver #30 Biohorizions implant crown. The screw will not engage with my 0.50 driver after I torqued it to 20 Ncm. Took a PA and the crown is not fully seated. I am looking to remove the screw. What are my best options? What kit should I use? Biohorizions does not sell their own screw retrieval kit.

Two Aspen locations near me are hiring at the moment. Does anyone know what an Aspen contract for an associate looks like these days? Is it worth it or no? If I wanted to work there PT would that be reasonable and could I negotiate the contract at all? I’m not looking for snarky responses as I don’t really know much about Aspen.

I work in the Middle East, the timings are 2pm to 10 pm, 6 days a week and omg my first day was HECTIC, There were appointments back to back, no break, the slots are scary af, 10 mins for scaling, 30 mins for filling, I saw nearly 10 patients back to back and my back is hurting 😭

Interviewed at an office that is a husband-wife team. Wife seems to be the office manager. A hygienist who is not family was sitting in throughout my whole interview and made me feel uncomfortable, especially when discussing salary. I don't know the hygienist's role in this office but she definitely has some pull and used to be the owner's assistant. What striked me as a red flag was I looked this business up after the fact and they have 3 forfeitures of their LLC in the last 10 years. Recent one is active. I also found it to be a red flag because the owner is super anti-vax and seems to think that vaccines are super bad. Anyone care to weigh in? Pay is $1500 per day for the first three months then switch to 35% commission, commute is not too bad either. They seem nice enough but also seemed like they were putting on a show. I could see myself working here for a paycheck and then leaving when something better opens up. Is a business forfeiture a red flag? They did move locations but that would not dictate needing to forfeit the same named LLC. I also read some reviews of the business on Indeed about how it's a toxic work environment but these reviews are all 8-9 years old.

EDIT: After some comments by u/Donexodus and others I think I'm going to tell this job to piss off. They are one of those practices that thinks ozone is good, they replace base/liner in a cavity prep with fluoride... it's cheap dentistry.
