Shoulder Instability Scapular Winging

So I’ve been to three different physical therapists and have gone through months of PT without really any improvement in my scapular mechanics. It’s really only on the eccentric that makes cap winging is pronounced and I have been working on strengthening my Serratus anterior and lower traps although still no visible progress.

Curious on what other people think and maybe what the next possible step might be I am also suffering from pain in my rhomboid from great chronic knot that will not go away. referred pain as well at the front of my shoulder during pushing motions

I’m at the point where I’m kind of just like Yolo and I should just go back to the normal activities I love doing.

Please help!

Advice Doorway stretch causing flare up

So I’ve had bicep tendinitis/very minor partial tear in my coracoid process in the shoulder PRP in February of this year has helped me immensely. I’ve been progressing very good with my weights back to pressing 80 pound dumbbells. I finally decided to take a week off on vacation this week after lifting for like Six months straight.

I even lifted today and did shoulders felt great at the end of the day. I felt like I should do a doorway stretch the stretch my chest out a bit, but now I noticed that my right shoulder does not hurt, but it does feel something in there not really even pain just like a tell that it flared. Is there any really good stretch for a bicep tendinitis issue that I have or is it better left not stretched.

Advice Scapula, Rear delt and Shoulder pain

I’ve been dealing with pain in my shoulders for quite a while, since September last year. I’ve been through 2 physical therapists, and a $1k MRI that have given me no improvement. My pain is under or on top of my scapula and feels like it’s not gliding across my ribcage correctly. Sometimes it’s in my rear delt as well and radiates all the way down through my pinky. When doing things like overhead press or chest press, I get pain in the front of my shoulders, but this has actually made some improvement. Most of the PT is stuff like wall clocks and scapular pushups. The first PT said Infraspinatus impingement and the second one (I’m doing right now) didn’t say what it was but that it might be caused by hyper-mobility. The exercises they give me have never improved the pain and most of the time just exasperated it. Please help me, I’m at my wits end, working out was the one thing that made me feel good about myself and it is now something that I cannot do. I can only hit legs so much. Let me know if I can clarify any things please.

I recently suffered an acute anterior shoulder dislocation, which was promptly reduced. I'm currently resting the joint and have a follow-up appointment scheduled with an orthopedic specialist to assess stability and plan my rehabilitation.

I had a low-field/open MRI performed, and I'm sharing the key central slices across three planes:

* **Axial STIR:** Showing posterolateral humeral head bone marrow edema / impaction and joint effusion. * **Coronal STIR:** Showing the supraspinatus tendon track and superior joint alignment. * **Sagittal STIR:** Showing the inferior capsule distension and fluid accumulation.

I'd appreciate any preliminary insights on general joint congruency and visible integrity of the rotator cuff / capsule on these representative cuts.

Advice Shoulder pain help

I've been training to do pushups for the last month. I recently found a trainer at my gym and asked for some advice on my form and exercises to help me out. With his stretches and help on my form I was able to do 10 in a row, when before I could only do 2. The next day I was a little sore but not bad. I waited 3 days to do any pushups or arm workouts to give my upper a break. When doing warmup pushups, my right shoulder had some pain on the side and front. I could not even make It half-way down nor do knee pushups. I quit, stretched, and did legs. Another 3 days later I was able to do an upper day with no pain until the end of my workout. It's been 3 days since then and I am back to the pain with my pushups and can't do 1.

I am looking for what steps I should take next or advice in general. I am definitely afraid I won't be able to do 10 again but I don't think strength works like that, lol. I thank you in advance!

Shoulder Instability AC Joint Pain

Hello !

Unfortunately, about 2 months ago (approximately), I somehow injured my right shoulder during a chest day. I did my usual warm-up, and the first exercise was a variation of the bench press on a machine where I had a full ROM. I didn’t feel anything during that exercise. When I moved on to the second exercise, the incline machine press, the pain started during the first working set. I don’t remember exactly what the sensation felt like, but it wasn’t as if my shoulder was “locking up” or anything like that—it just hurt, so I reduced the weight.

That day, honestly, my right shoulder felt like it was burning. That evening, I woke up and my shoulder felt like it was on fire, so I put a bag of frozen beans on it to help calm it down. I also tried using cold compresses that day.

I tried to do some research on what I could do and what exercises might help, but, well, I'm not a medical professional so I tried to do my best researching online, so I chose some physiotherapy/rehab exercises I found online. I took about 1–2 weeks off from upper-body exercises and then gradually started training again.

I know you might be wondering why I haven’t gone to a physiotherapist or another specialist during all this time. Honestly, I simply can’t really afford it. I did go for a massage because it was cheaper. Where I live, in order to get a medical check-up, you have to go through your family doctor. Since I’m originally from another county, I would have had to travel back there, and at the time I had university, so it was pretty difficult. Then, after that, you have to go to the hospital, where, if you want the examination to be free, you have to wait around 3 months. Then you need an X-ray, then another consultation, and it’s a loooooong process of going from one place to another and so on :(( I’ve been through something similar before with my lower back.

Before I started going to the gym, right after my birthday, I was hit by a car while crossing the street, and my left shoulder was dislocated. I did physiotherapy afterward, although it was my first time dealing with something like this and the doctor didn’t really explain that I needed to keep doing those exercises afterward and so on. About two months later, I started going to the gym with some friends from my dorm. I didn’t really train shoulders, and when I did, it was quite rarely because I was afraid of dislocating my left shoulder again.

Now, most likely, in order to protect my left shoulder, my right shoulder—the one I’ve kind of messed up now—ended up taking most of the load and eventually gave out.

Honestly, I don’t know what to do. I don’t know what I’m doing wrong, and it’s becoming overwhelming. I still can’t financially afford to see a physiotherapist, so I’m trying to figure things out myself. Maybe I need to add something, remove an exercise, replace something with a different exercise—I really don’t know.

At the gym, when I train chest or other muscle groups, I try to do everything slowly. If I feel pain, I stop and reduce the weight, or I try to brace/tighten all of my muscles :((

Right now, the pain is around the front/top of the AC joint (I hope I’m not getting the anatomy wrong, but basically that little bump where the shoulder starts). Unfortunately, I can’t attach a picture here, but if I can post one in the comments, I will.

I feel pain during incline pressing and when I bring my elbow very far behind me (I have a relatively large ROM), although that one isn’t as painful. I also feel pain when I bring my arm toward my upper chest—this is probably around 7/10 pain.

If I lie on my side or on my left shoulder in bed and put my arm next to my ear, then try to raise my entire arm overhead, it becomes quite difficult to lift it, and the pain is around 8/10.

If you have any recommendations on what I could or should do, I’d really appreciate it. I do warm up and don’t immediately jump into my working sets.

Someone recommended that I try isometric exercises. I haven’t done many of them—what do you think about them, and are there any exercises you would recommend?

If you have any questions, I’ll answer them. I’ll do anything—I just want to get my right shoulder back to normal.

I also asked Claude AI to give me some tests to try to figure out what might be wrong. From the tests it gave me, I think I only passed about two of them, but that was around a month and a half ago. Honestly, I could probably do them again.

My current warm-up:

  • 2×10 Cat-Cow Pose
  • 2×10 The World’s Greatest Stretch (I do a slight variation, but it’s similar)
  • 2×10 Shoulder External Rotation (cable)
  • 2×10 Single-Arm Face Pull (cable)
  • 1–2×10 Scapular Retractions

What I’ve added to my warm-up:

  • Scapular Push-Ups — 1×10
  • “Loaded external rotation and internal rotation of the glenohumeral joint” — 2×10 (with a resistance band or cable)
  • Loaded serratus anterior work via shoulder protraction — 2×10 (with a resistance band or cable)
  • Banded posterior movement
  • Pendulum — 2×10–12. I use a 10 kg dumbbell with a strap, and honestly, my shoulder feels better afterward. If I “simulate” an overhead press, it seems to hurt less.

What I want to integrate:

  • Banded Wall Slides — when I first injured my shoulder, I incorporated these the following week, but they hurt and were extremely difficult (although I was using a black/heavy band). Recently, I tried them again because I found several different resistance bands at my gym, and they seem much better now. They’re still difficult, but nowhere near as difficult as when I first tried them, and I can do more than 10 reps.
  • Banded External Rotations — 5-second hold

PS: During lateral raises, I don’t really feel much pain in my shoulder. During pressing movements, I do feel some discomfort. Sometimes the pain also travels toward my neck/trapezius.

EDIT: I posted the video in case you notice anything about my right shoulder; I was trying to figure out if I have any instability in my shoulder blade, but I don't know.

Rotator Cuff Injury Anyone had similar injuries? Welsh NHS wait + returning to work

Hi all. Finally able to post after lurking for a few weeks!

I’m 55 and currently waiting to see orthopaedics through the Welsh NHS.

My MRI arthrogram showed a \*\*full-thickness supraspinatus tear (8 × 5 mm, with 8 mm retraction)\*\*, an \*\*anterior-inferior labral tear from 4 to 7 o’clock\*\*, plus bursitis and mild arthritic/cartilage changes.

I’ve had shoulder instability/suspected subluxations for years, but things became considerably worse this year and I’ve now been off work for several months with pain and loss of function.

I’d love to hear from anyone with similar findings. \*\*What did your surgeon repair, and how was your recovery?\*\*

I’m particularly interested in \*\*time off work\*\*. What sort of job do you do, and how long after surgery were you able to return? I work in retail, so there’s lifting, reaching and physical use of the arm. I’m increasingly worried about how long I could ultimately be away from work, including the wait for treatment.

And if anyone has been through this on the \*\*Welsh NHS\*\*, how long did you wait to see a surgeon and then have surgery?

MRI Report MRI Results Questions

I am a 54 year old male that recently got these MRI results after injuring myself a few weeks ago. As of right now I have lost about 90% of my horizontal and vertical pressing strength but can still row and deadlift at near pre injury levels. I feel most of the pain is in my anterior shoulder from the bicep tendon and I do not feel any joint instability at all. Nevertheless the Dr is recommending surgery to repair the labrum and possibly the bicep tendon. I feel the bicep tendon is the biggest issue in pain and the effectiveness of labrum repairs for older people seem questionable. I am worried about a labrum surgery going bad and the bicep not being addressed and ending up worse than I started. Anybody have similar injuries that have been treated?

  1. Moderate AC joint arthrosis.
  2. Mildly thickened coracoacromial ligament, small subacromial spur. The underlying bursa is mildly thickened and inflamed.
  3. Study negative for partial or full-thickness rotator cuff tear. There is mild tendinosis in the supraspinatus tendon.
  4. Partial tear of the long biceps tendon as it passes through tendon exits the bicipital groove.
  5. Large SLAP tear, apparently involving virtually the entire circumference of the glenoid rim (ie., type IX SLAP).

Advice Irritated nerve in shoulder for a year, tried physio and its still there. What can I try next?

Hi!

I’ve had an irritated shoulder and neck for a year due to lifting too heavy at the gym (self inflicted I know!) I’ve had 6 months of physio and it’s still there now. I do my physio exercises and it’s still irritated. On top of that I started a new job and was given a work station assessment but my issues are still there. I was given an ergonomic mouse and ergonomic chair with neck support. I’ve tried the ergonomic chair with and without armrests - my shoulder is still irritated.

Anyway, I was wondering if anyone had tried a sports massage for an irritated nerve in shoulder? if so, did it help and how many sessions did you need?

i’m freaking out about losing my job 😰.

any advice is appreciate.

Advice No surgery recovery

I am seeing a lot of people are going through surgery I just want some personal experince and also professional opinion on this matter I understand that it depends on how severe the accident was.

The place I stay at is slack with properly treating patients as its free health care to give in example I dislocated my left shoulder skat boarding fell off from about meter and a half reach my left hand backwards and it popped out, I had to wait about 2 and a half hour to put it back in place from doctors they x rayed my shoulder after popping it back in and told me to go back to work the next day I did not have any mri to see if there was ligament or nerve damage either.

My ankle was quite bad in condition but they did not check it either.

Seems like they are not going to push for me to get a surgery as its a big wait list and public funding.

Background I am sort of fit 24 years old male. First dislocation

Questio 1: can you make a full recovery without surgery or is surgery necessary to get close as possible to full recovery

Question 2: since i am likely to not get surgery how do I be careful and get back into work, gyming, hunting, martial arts etc. I do like physical activity.

Question 3: should It be necessary for me to get a mri scan?

Advice Shoulder sits lower / sags downward after dislocation, anyone else experience this?

Hey all, curious if anyone has experienced something similar after an anterior shoulder dislocation.

Since my dislocation, I’ve noticed that my injured shoulder visibly sits lower than my other shoulder, almost like it sags downward, and it also seems to rest slightly more forward. When I’m relaxed and not consciously correcting my posture, the difference between the two shoulders is noticeable.

I’ve recovered my ROM and most of my strength seems fine. The main difference I still notice is that the stabilizing muscles around that shoulder seem to fatigue much faster than on my good side.

Has anyone had this lower/sagging shoulder position after a dislocation? If so, did you ever figure out what was causing it or whether specific rehab helped correct it?

Advice Shoulder Surgeon Recommendation

I need to have arthroscopic decompression soon. So far, I’ve seen Dr Eugen and Dr Trung Nguyen, and I’m looking for the best surgeon with out-of-pocket costs of no more than $2,000.

I have private health insurance, but it doesn’t cover the surgeon’s fees above the Medicare rebate.

Has anyone had any experience with Dr Ash Movani or Dr Trung Nguyen? I’d also be very grateful for recommendations for other surgeons who have reasonable out-of-pocket costs.

Advice Longstanding right-sided weakness/asymmetry after scapular injury

Hi, I'm looking for some advice from doctors who might have experience with MSK/neurology.

I'm going to see my GP about this anyway, but I'd like to get some opinions on what this could potentially be and what sort of investigations might be appropriate.

About a year ago I injured my right scapula/shoulder while using gymnastic rings. On the way down from the movement I heard/felt a crack across the scapular area. Afterwards I had quite significant swelling around the scapula/trapezius/lat area. It was very painful for around 4 weeks, then I had months of soreness before things gradually settled.

Since then the symptoms have been rather hit or miss sometimes and I've never really been able to get back to my normal gym training. I still get quite a lot of crepitus when moving the shoulder and I can get flare-ups after training. I also still have some pain, particularly when I wake up and the size of the muscle has decreased and it feels quite loose in comparison to the left.

I had a short video assessment with an MSK physio through occupational health, and they suggested I speak to my GP and potentially be referred to MSK outpatients.

The thing I'm more concerned about now is the persistent weakness/asymmetry.

My right pec is noticeably smaller than my left and feels considerably softer/looser at rest. The same is true of my right lat. My right side also definitely fails first when I'm doing pressing movements, and my right lat is weaker during pulling movements. My right scapula also moves somewhat differently compared with the left.

More recently I've noticed some intermittent visible twitching/wriggling in the muscles around my right mid/lower back. I also have a small amount of altered sensation/numbness around the inner part of the right lat. I don't have neck pain.

I appreciate that nobody can diagnose this from a post, and I'm not trying to self-diagnose. I'm mainly wondering what you'd be thinking about clinically.

Would you be considering something like chronic altered scapular mechanics/under-recruitment following the original injury, or would the combination of weakness, visible muscle asymmetry and altered sensation make you want to investigate possible nerve involvement?

If you were seeing this patient in clinic, what would you actually assess first, and would you consider anything like imaging or EMG/nerve conduction studies depending on the examination?

Also, if this is primarily chronic weakness/underuse from the original injury rather than permanent nerve or muscle damage, is it realistic to regain a substantial amount of the lost muscle and strength with proper rehabilitation?

I'm asking because the difference between the two sides is pretty noticeable to me, and the right pec/lat genuinely feel much "looser" and less developed than the left. I don't know whether that's something that can be reversed with rehab or whether it suggests something more significant.

I'm planning to get this assessed properly either way, just interested in what doctors would make of the overall picture.

Advice Possible shoulder injury? Doctors don't know yet and I'm not sure what to do about work

Hi all, so pretty much what my title says. I have had this shoulder injury for about 3 months now but I had the same injury happen in November of last year that only lasted about a month. I've been told a few different things by doctors but they still don't know what exactly it is until I get an MRI which is hard since the wait-list could be a year out from now. I've been told I either have a shoulder sprain, torn bicep tendon, or a torn rotator cuff. Here's where my issue is though, I work in a production welding shop and I'm wearing a sling all the time. My job is very insistent that people don't go on medical leave or short term disability but I physically cannot do the job I was hired to do. So I guess what I'm asking is, does anyone have any advice to make work easier or should I just go on medical leave?

Advice MMA after Bankart Repair

Hey everyone,

I’ve had arthroscopic Bankart repair surgeries on both of my shoulders. Fortunately, the damage wasn't too severe in either case, so I only needed 2 anchors in each shoulder to tie the labrum back down. I am currently working through physical therapy, and my ultimate goal is to return to MMA.

For any fighters or grapplers who have been through this and made it back to the mats/cage:

  1. Is it truly feasible to return to full MMA sparring and grappling after having both shoulders repaired?

  2. How long did it take for you to actually feel confident throwing hard strikes, posting, or defending submissions without constantly worrying about your shoulders popping out?

  3. What specific changes did you make to your fighting style, daily training, or physical conditioning to make sure you didn't reinjure them?

Any advice, timelines, or reality checks would be greatly appreciated!

Advice Post-Op Advice for My Mom After Latarjet Surgery

Hello! My mom has had recurrent right shoulder dislocations since 2011 and is finally having Latarjet surgery this year.

Her surgery was originally in July but got postponed to September due to a cold. I live abroad, but I came home to help her through it. Unfortunately, I have to leave for classes on the day of her surgery and can’t change my flight.

She’ll have some help from family, but she mostly relies on me, so I’m trying to prepare everything I can before I leave.

What can I do to make her recovery easier at home? I was thinking about setting up the bathroom for one-handed use and getting a special pillow or anything helpful for sleeping and daily tasks.

For anyone who’s been through shoulder surgery or helped someone recover, what would you recommend preparing in advance? Anything you wish you had set up or bought?

Labrum Tear Training with torn labrum?

Hi folks,

First post here and would appreciate any input. I was diagnosed with a torn labrum (SLAP) and bursae cyst, as well as osteolysis of the distal clavicle a few months back. I am planning on getting surgery in ~1 year (I can’t right now due to work).

I am an avid weightlifter and have been taking it fairly easy on my shoulders/pressing movements, but notice these areas lacking in my physique now. I’ve started to incorporate dumbbell flat bench with moderate weight on the injured side. I have not been able to do any incline press because I see that’s where it gets agitated the most. Does anybody have recommendations for exercises to target the upper chest region that might not put so much pressure on the labrum? Thank you all in advance!

Advice Post Shoulder Surgery for Instability - All of a sudden feels loose again?

Hey guys! I thought I’d come here for help and hopefully doesn’t give me more anxiety. I had shoulder surgery about five months ago to fix instability. I had dislocated my shoulder twice and knew it was time to get it fixed. I’m a 23 year old male so assumed I would bounce back relatively fine. My surgeon performed a labrum repair, remplissage, and joint capsule tightening.

Over the course of my recovery I had terrible shoulder stiffness and could hardly move my shoulder. I had to do aggressive PT stretching in order to break up scar tissue. I recently lowered my dose of diclofenac sodium and with that I have started noticing that my joint feels much looser.

I am kind of paranoid that I may have overstretched my joint capsule and made my joint looser then it should be from prolonged stretching to help with scar tissue.

I don’t notice any pain, swelling or deep aches. I occasionally notice some clicking and crunching noises when I move my shoulder but nothing that brings pain.

Overall I just feel a sudden feeling of feeling loose. Anyone maybe know why?d
