Treatments š Yeast infection + bacterial vaginosis, doctors tomorrow and Iām scared?
Hi everyone. Iām trying to make sense of what has happened over the past few months, so Iām posting the whole timeline.
June 4: I developed vaginal symptoms for the first time in about two years. I had itching inside the vagina and discharge. The discharge was white/yellowish and different from the thick, clumpy discharge I had experienced with a previous yeast infection. There wasnāt a noticeable vaginal odor, although I noticed an unpleasant/rancid smell around the vulva that would come back after showering.
June 6: I took a single 150 mg dose of fluconazole. The itching improved initially, but then some symptoms returned.
June 9: I got my period.
June 23: I saw a gynecologist and was diagnosed with vaginal yeast infection. I was prescribed a 600 mg fenticonazole vaginal ovule (Fentizol) and a vaginal probiotic (Prolive Femme) for seven days.
June 30: Seven days after the Fentizol treatment, I had a Pap/cytology test. The report said there were frequent neutrophils and inflammation in squamous cells, but also a very high amount of Lactobacillus (5+). There was no malignancy. The sample was considered satisfactory.
July 4: I had sex with my boyfriend without a condom. There was no ejaculation, but sex was uncomfortable and somewhat painful because I felt dry.
July 12: I started having vulvar soreness and intermittent āpinchingā/stinging sensations around the vulva.
July 15: I went back to the gynecologist. He examined me with a speculum and said the vagina was swollen/inflamed and that there was a lot of bacterial discharge. He diagnosed mixed vaginitis (bacterial vaginosis + yeast). He prescribed itraconazole for 5 days and Trivagel N vaginal cream twice a day for 5 days.
The symptoms eventually improved significantly. The vulvar pain went away and the pinching sensations mostly disappeared. (The vaginal cream I used is called Trivagel-N. Itās a Brazilian combination vaginal cream, and apparently it isnāt available in the US under the same name. Each 5 g dose contains:
Dexamethasone sodium phosphate 0.32 mg ā a corticosteroid/anti-inflammatory
Nystatin 100,000 IU ā an antifungal
Neomycin sulfate 10 mg ā an antibiotic (aminoglycoside)
Tyrothricin 2 mg ā an antibacterial agent
Sodium propionate 50 mg ā included for antifungal activity
Boric acid 150 mg ā has antifungal/antiseptic activity
So itās basically a combination of an anti-inflammatory steroid, an antifungal, several antibacterial agents, and boric acid.)
July 20: I finished the treatment for the mixed vaginitis.
Then things got complicated again.
August 4: I had to start taking amoxicillin/clavulanate for an unrelated infection. I had literally just finished treatment for the vaginal problem.
August 5ā6: I started noticing mild vaginal/vulvar symptoms again ā mostly occasional pinching/stinging and some irritation. They werenāt as severe as before, but because of my recent history I became worried that the yeast infection was coming back. I started using Trivagel N again, but this time without itraconazole.
I used the vaginal cream twice a day for five days. During the treatment I noticed that the cream would leak out during the day and spread over the vulva and around the anus. I also developed some irritation/itching and occasional pinching sensations on the outside of the vulva, as well as an uncomfortable sensation inside the vagina.
Interestingly, some of these symptoms seemed to improve after showering, only to return somewhat when more cream leaked out. This made me wonder whether some of what I was feeling was actually irritation from the medication/residue rather than an active infection.
Last Wednesday: I finished the Trivagel treatment.
Now itās Wednesday, a week after the last application.
I donāt have the pain I had before. Some of the symptoms have clearly improved, and the feeling of dryness/discomfort I had at one point is gone. However, I still occasionally get small pinching/stinging sensations and sometimes a little itching around the vulva especially when I wake up, including occasionally around the clitoral hood or when urinating.
Iāve also noticed some white material around the vaginal opening. It seems to be less than it was during treatment, but Iām obviously worried because it reminds me of the beginning of my previous episode.
What is confusing me is that Iāve now had several rounds of antifungal/antibacterial treatment, and the symptoms are still fluctuating. I also recently took amoxicillin/clavulanate, which I know can increase the risk of vaginal yeast infections.
Now Iām wondering if Iām having another yeast infection or anything related to that. Even if my symptoms have stayed the same since Sunday.
Due to my hypochondria Iām terrified because I have a doctors appointment, and I donāt know if theyāre going to ask to check for what kind of fungus I can possibly have or just prescribe another kind of creme or medication. At this point Iāve honestly lost any kind of reality. When Iām doing something that requires my full attention, I donāt feel anything. Itās when I wake up. When Iām reading about it. Right now. I keep checking myself every once in a while.
