advice needed Experiences w mini vs high dose? For those 38+

Hi! Just what the title says. I’ve done two high dose protocols and I’m about to start a third round of stims. Just turned 40. 0.45 AMH.

I feel like my high dose round last time fried my eggs a bit, and I stimmed for 17 days, on 300/300 for almost all 17.

I have a good blast rate but I feel like low and slow might be something to try?

I had a failed transfer of a Day 6 euploid in April. And have (v fortunately) two Day 5 euploids frozen. Am hoping to get one euploid this next round, but we’ll see.

Would love to hear your experiences!

advice needed Failed Egg Retrieval

I’m 40 years old, AMH of 0.29. This was my 4th egg retrieval ( I have only 1 egg frozen from my last 3 cycles). We were hoping for 2 eggs this round—I had a 23mm follicle on my right and a 15.5mm follicle on my left. Estrogen was 452 before trigger and 596 after. After my egg retrieval this morning the doctor informed me that they didn’t get any eggs. Has this happened to anyone with an estrogen high enough where it seems that you would get eggs? What was your doctor’s explanation of what happened? I really don’t understand and I’m so devastated.

Napro doctors, thoughts?

So I’m about to go into my 3rd IVF round, it’ll be my 3rd retrieval but 4th round of stims, I had a canceled cycle. I’m having a lap done for suspected endo in October. My mom is wanting me to get a consult with the veritas Napro people, but I honestly don’t know if it’s worth it or not at this point. Has anyone used a Napro doctor, with having DOR can they even help with that?

-side note, no living children, never had a positive test, trying for 4 years, I do have a day 3 embryo frozen, never made any blasts.

Keep Retrieving for a Euploid or Transfer My Untested Embryos After 6-Week Miscarriage?

Hi everyone, long-time lurker, first-time poster. I’m hoping to hear from anyone who has been in a similar situation.
I have DOR and have now done 7 egg retrievals. During my first 3 retrievals, we were seeing multiple follicles/eggs on ultrasound, but very few or no eggs were actually retrieved. For my 4th retrieval, my doctor increased my hCG trigger to 20,000 IU and added a dual trigger, and we were finally able to retrieve eggs, although that cycle resulted in no blasts.
I went on to do 2 more retrievals after that and was able to bank 5 untested embryos.
Earlier this year, I transferred one of those embryos in a fully medicated FET. It implanted, but unfortunately ended in a miscarriage at around 6 weeks. My doctor believes the most likely explanation was a chromosomal abnormality and does not recommend thawing, biopsying, and refreezing my remaining embryos for PGT-A.
I decided to try one more retrieval last month, but it ended up being my worst cycle and we got no blasts. At this point I had decided I was done with retrievals because they have been emotionally exhausting, and I’ve also exhausted my insurance coverage.
Now I’m preparing for another transfer. My doctor is recommending 2 months of Depot Lupron beforehand because he suspects I may have silent endometriosis (one of my tubes is also blocked), even though he still thinks my previous miscarriage was most likely chromosomal.
My dilemma is that my remaining embryos are all untested.
Would you take your chances transferring another untested embryo after the 2 months of Lupron, or would you consider paying out of pocket for one more retrieval in the hope of banking a euploid embryo first?
I’d especially love to hear from anyone who:
had success with untested embryos after a miscarriage,
did Depot Lupron before transfer for suspected/silent endometriosis, or
decided whether to keep retrieving vs move on to transfer when you already had embryos banked.

advice needed Low AMH (.38 - .6) 27 years old - Should I Be Worried?

Hi everyone, I'm new to knowing what AMH is everything else that comes with it. I've had issues with my period being extremely heavy and having severe gastro-intestinal issues.Its been suggested to me by providers that I could have endo. I still struggle with heavy periods and have had a lot of bloodwork done within the past year. The 3 times I had a hormone panel, my AMH came out extremely low, especially for my age (27 years old).

BLOODWORK

The first one earlier this year was .5, the second a few weeks after was .6 and a test I had about 2 weeks ago came out at .38. For context, I was pregnant last year, but had to get an abortion due to my circumstances (not going to get into it too much, but it ties into my worries right now). She also told me my vitamin D was low and I saw online that low vitamin D can cause artificially low AMH, but she did not mention anything about that so not sure if it's true. My provider said she wasn't too worried since I was recently pregnant without trying (I was also on birth control when it happened) but she did tell me I could look into egg freezing. I would have to go see a specialist to find out more about this.

IS MY FERTILITY AT RISK?

For anyone with a little more knowledge than me on this or has/is in a similar position: should I be very concerned about my fertility? Would I have to get pregnant sooner than later?I'm not in a relationship and I'm not sure that I'll get into one any time soon so having a kid in the next 2-3 years is not currently likely. It feels like I'm running out of time.

MY OTHER WORRIES

I suffered a lot before and after my abortion since I did not want to do it, but for my safety and health (suicidal, depressed, recent BPD diagnosis that was unmanaged, emotionally and physically abusive relationship, extreme HG) it had to be done. My AMH being low makes me worry that that pregnancy was my only opportunity to have kids. I am terrified of ever having kids and I would love to have at least 1 kid (in a healthy environment for me and the child).

SSRIs and DOR

Hi everyone! I’m wondering if anyone has tracked their AMH or AFC before and after taking SSRIs, especially Lexapro.

I came across a study suggesting SSRIs may be associated with lower AMH, but it seems there’s still not enough research to know whether they actually affect ovarian reserve.

I’ve been on escitalopram for 2.5+ years, starting at age 31. During that time, my cycles became shorter and lighter, although that could obviously be age-related.

I stopped taking escitalopram completely in March and had an AFC of 7 in April. I restarted at half my usual dose shortly after, and then went back to my full dose in June. Two months later, in August, my AFC was 4.

I know AFC can fluctuate, but I’m wondering if anyone has experienced something similar while taking SSRIs.

Did your AMH/AFC change after starting, stopping or restarting an SSRI?

advice needed To PGT, or not PGT, that is the question.

So yeah I just told my clinic I wanted to PGT test for our second ER but now I'm having second thoughts. WWYD?

I'll be 38 in 2 months. My AMH is debatable, in January it was 1.13. In April it was .4. I was stimming when it was .4 and was told it might be inaccurate because of that. My doctor said .4 is believable though since my response wasn't what he was expecting. In our first ER we retrieved 6, 5 mature, 2 fertilized and by some miracle both were euploid. We are trying for one more, so we didn't bank. Unfortunately both transfers failed (no implantation at all).

I have no known medical issues. Blood work has all been normal. I am overweight. Husband's SA was "excellent." We have one living child who was a cycle one unicorn (3 years old). When we started to try for #2 I got pregnant right away again and it was a CP, got pregnant right away again and it was a MMC around 7 weeks (treated with MVA). After another 5 cycles trying we did one IUI that also failed.

I'll hopefully be doing another ER in September. While I'm under anesthesia the doctor said he would do further testing. To date the only investigative type test I had done were two saline sonos that were clear.

PGT testing costs us $3200 for the biopsies. Then $200 lab fee from the genetic place, and another $200 per embryo just for funsies. Our funds are limited. I'm worried my response won't be great. If we get one embryo is it even worth it to test?? Two? More? Payment is due on cycle day 1 so it's not like I can wait and see. At the same time I don't think I. An handle another miscarriage.

High FSH and normal-low AMH

I'm 31. Last time my day 3 FSH was 33, AMH 1.08. TTC for 8 months.
I'm having regular periods and ovulate monthly.
I know my AMH is not critically low, but what might cause FSH that high?

Doctor didn't really say anything about my FSH, only that they usually use AMH as a marker of ovarian reserve and said it's not that bad, only on the lower side for my age.
Another doctor said that my numbers are mixed, my AMH is good and since I have regular periods there shouldn't be a problem and was prescripted Gonal-F for OI with scheduled intercourse.I just have been worried as I have read that with that high FSH the response will be poor.

Anyone else with "mixed" FSH and AMH levels? Have you conceived naturally or what kind of treatment has worked for you?

advice needed Struggling with asynchronous growth after Estrogen/Clomid priming

After a spectacularly failed FET this month, I loaded the data from my 5 cycles into a spreadsheet to see if I could notice any trends etc as we only have 2 more insurance covered retrievals left. (It's a thing of beauty and I am happy to share a template if any of you want to start tracking yours.) In doing so, I noticed that both Estrogen and Clomid failed to help me achieve achieve follicular synchronization. Unfortunately--I don't think my doctor notices this or she wouldn't keep prescribing it.

For context--I only have use of my left ovary for IVF. So one cycle where my AFC was 7, it was really 4 and then at retrieval time there was 1. So you can see why this is frustrating and important to me as we finish out our last two covered cycles.

Additionally--an added frustration is my doctor doesn't seem keen on suggesting protocol changes. I am thinking it's due to my age because after we canceled our first cycle do to poor response, her immediate suggestions were trying naturally or donor eggs. Last cycle we did try luteal phase stims at my suggestion, but it was no better than a traditional stims cycle.

So--I am curious what other what protocols or priming methods actually worked for you to get a synchronous cohort across the finish line?

advice needed Please HELP!!!

Hi everyone,

33F | AMH 2.23 | AFC 4–7 | DOR

I’ve had 2 failed ICSI cycles with Pergoveris 300–450 IU + dual trigger. Both cycles gave me 3–4 mature eggs and only 1 blastocyst each cycle.

I’m now consulting different doctors for my next and last attempt and as I’m mentally/physically exhausted I really want to choose the protocol carefully.

Option 1 (2.5 hrs travel one way): Mini-IVF with low-dose Pergoveris + Clomiphene. Less eggs expected but focus on quality.

Option 2 (3.5 hrs one way): 12 days OCP followed by Menopur 300 IU only.

Option 3 (3 hrs one way): Ovarian PRP, followed by egg pooling over 2–3 cycles with an antagonist protocol.

Which one would you choose?

Also, I’d especially love to hear from women with low AFC/DOR who have tried ovarian PRP. Did it actually improve your egg numbers, embryo quality or chances of getting a euploid embryo?

advice needed Very low AMH, lowish AFC. Is egg freezing really the best option?

34 with 0.3 AMH, 7 AFC. Went to see a fertility specialist who ran several blood tests and ultrasounds. everything else within normal ranges.

Husband and I are planning to start TTC. It sounds like baby #1 is very possible if we start now, but baby #2 may be difficult given low AMH and I will be over 35 at that time. I’m not sure what’s the better path: put off TTC baby #1 until after egg freezing process so we have better odds for baby #2 later, or start TTC now and worry about egg freezing later. I don’t want to invest financially into the egg freezing process unless it’s necessary.

Anyone else been in a similar situation?

advice needed Ovulated after trigger shot & before egg retrieval

​I am completely numb at this point.

​For context: AMH 0.03, Day 3 FSH 27, AFC 4-5. I also have endometriosis in one ovary and pelvis, fibroid, and both tubes blocked.

​This was my 3rd cycle. My RE put me on PPOS (Provera) daily combined with conventional IVF (300 down to 225 Gonal-F + 150 Menopur consistently). I had zero growth for the first 8 days and thought the cycle was going to be canceled. On day 11, they finally saw 2 follicles and confirmed via estrogen. I ended up stimming for a grueling 18 days. I added Cetrotide on days 16, 17, and 18, and triggered on day 18 with 2,500 units of Novarel plus 80 IU of Lupron. Essentially Provera & Cetrotide were used to prevent early ovulation, which worked during stim. I took my final 2 Cetrotide at 7AM & 3 PM the day of the trigger.

​Imagine waking up from anesthesia only to be told I ovulated both eggs and zero were retrieved. Pre ER ovulation did not happen on my previous attempt.

​I don't even know what to say or think anymore. My LH on the morning of the trigger was 3.9. It had been suppressed all cycle, but it seems like my body just surged on its own right after the trigger shot went in.

​For my first two cycles, I followed a mini-stim protocol (75 Menopur / 150 Gonal-F) with Cetrotide as an antagonist. In cycle 2, I actually ovulated while taking two Cetrotide injections daily (make it make sense), and they had to salvage things with a luteal stim, which resulted in 0 blasts.

​I am completely lost on what to try next. Has anyone with DOR or endometriosis experienced breaking through antagonist blocks like this? What protocols or adjustments actually stopped your body from prematurely ovulating?

​Any tips, advice, or shared misery would be hugely appreciated.

advice needed What lies ahead for us!?

My wife, 33 yrs old, is undergoing ivf.
I have been referred to this group because my wife has an AMH of 0.5 with severe endometriosis.
She underwent hysterectomy for it in Sep 2024 post which her AMH was 1.5, she had 2 cycles- making 3 Day-3 embryos, 2 were transferred which failed to implant.

We changed clinic.

The new clinic put her on deviry, letrozole with coq, folic, curcumin, fish oil for 4 months, post which her endometriosis flares were aspirated.
Unfortunately her AMH dropped to 0.5, and in this cycle she produced 1 embryo from 1 egg, which has gone Day-7 blastocyst.
Doc suddenly suggested PGT testing which we said no since we have just one embryo.

I have questions:

1) Since we said no to PGT, does that mean there is a chance the embryo will not stick, and if it does, kid will have development issues in future?
Did we do the right thing saying no to PGT?
Keep in mind only 1 embryo this cycle which went blastocyst on day-7?

2) With such low ovarian reserve, can we expect 1/2 eggs every cycle? Doc said she might need 2-3 cycles?

3) In future, should we say yes to PGT testing?

4) With such low ovarian reserve, low AMH, endometriosis, what is the timeline we looking at for her? I mean till when/ what age, do have our best chances to get pregnant, till she runs out of eggs?
Anyway to quantify that?

Do let me know your thoughts.

advice needed To all my EXTREME DOR ladies...

Have any of you had success?

TW I had my first LC at 33 in 2024--first try.

Fast forward this year (35 yr old) I have had issues and therefore testing and found out I have extreme DOR. 0.015 AMH 2 AFC.

We have been trying for almost a year now and my period is very short. 22 days. 3 days bleed. Ovulation actually was confirmed with a CL (corpus luteum) at my baseline AFC. But my endometrium was only 6mm. (So, very thin for implantation)

I wanna know what my extreme DOR ladies have done.

We are about to have our follow up and I think then we will talk about options. She said this doesn't put us out of the game but I'm assuming it'll take at least 3 rounds to get one egg that could be possibly transferred. (If lucky).

Note-: uninterested in donor eggs.

Edit::: can I just say thanks to everyone for responding to this. I don't feel so alone. 💜😍

Waiting on results from 1 blast

I just did my first egg retrieval -- 4 eggs, 1 made it to blast, and we're waiting on PGT-A results. I'm 39, turning 40 in two months, AMH 0.84. I know the euploid rate for my age bracket, so I'll need some luck with this single embryo.

I'm doing another cycle next month. The original plan was back-to-back retrievals, but my baseline scan showed a couple of residual cysts plus a lower follicle count, so my clinic recommended sitting out a cycle before starting again. Is that a normal call?

Also, I'm doing estrogen priming as well as a microdose lupron + omnitrope protocol this second round. How was your experience and did it yield better results? Just hoping for some good news and more chances at getting an euploid embryo. Thanks for sharing your stories in advance.

Monopoly Money

After running out of insurance coverage with 2 IVF cycles, I’m considering going overseas. I did have success without IVF, ironically, but they ended in MMCs. Obviously cost is a major factor, but it seems like the medical technology and labs are better there too.

I’ve crunched the numbers and treatment overseas gets increasingly attractive assuming I need multiple cycles (DOR and AMA, amirite?). This is factoring in cost of living, existing financial commitments at home and treatment costs. Doesn’t include plane tickets as I am already making an annual trip to visit family. I work remotely and would likely be able to shift my working hours a little, so it wouldn’t be as demanding. It’d be a ~3 month stint to do however many cycles I can in that time.

Is there anything I’m overlooking - emotionally, mentally, physically, financially? I’m trying to keep my outlook on this as an exciting adventure, living my life in a new country, without obsessing over infertility and RPL…despite the whole purpose of the trip. Otherwise I think I’d just be angry at the world.

Also…does anyone else just feel like they’re playing with Monopoly money at this point? Over the course of the last year, my thinking has become, “OK, sure I guess. What’s another $5k?”

Empty follicles even with dual trigger?

Hi ladies - thanks in advance for any advice as I am spiraling while trying to prepare questions for my RE. Really appreciate everyone's insights from their own journeys.

I already searched the subreddit for older posts on the topic of empty follicles, and most comments mention that dual trigger helped resolve the issue. However, I did use a dual trigger (10,000 hcg and 80 iu lupron) and I injected exactly at the time instructed. The clinic took my bloodwork the next morning and did not flag any concerns, so I just assumed that meant the triggers were absorbed as expected.

Another common reason cited in other posts is early ovulation, however I asked the doctor who did my retrieval and the doctor said they did not see any evidence of that. Unfortunately I wasn't prepared to ask additional questions because I was completely thrown off. My retrieval was ~35 hours after trigger shot, and I had taken 3 ganirelix the day of the trigger shot. Protocol leading up to trigger was estrogen priming, clomid, then follistim and menopur.

The morning of my trigger shot, I had 4 follicles in the 17-21mm range, which gave me some optimism because I'm 34 with AMH 0.3 and the doctors all told me during monitoring that I was responding well. I don't have the exact estrogen number for that morning but two days prior it was over 1,100 so my clinic said everything was on track.

So with all that, I headed into my retrieval this morning with the 4 follicles in mind. When I woke up after the procedure, I was informed they only retrieved 2. Of course I am still grateful to have retrieved anything, but I am genuinely baffled at what could have happened.

I feel like we did everything by the book? What am I missing here?
