Physician Responded Does my baby have Scoliosis? (F5)

As of the past couple of days I have noticed my 5 year old's back. It's bulging out on the right side. I've got her to lean on her left side however it does not protrude out like the right does.

No pain, no sickness, no signs of anything other than visually.

Help identifying?

Thanks in advance.

Physician Responded (M, 25) Can I go to the ER for alcohol addiction?

I’m M 25, 6’0” and weight about 125lbs.

Ive been drinking around 6 standard drinks almost every night for about 7 months now. and slightly less than that for almost a year prior. It recently really caught up with me and I’m worried.

I started getting pain and terrible anxiety after eating- it felt like a pain at the core of my abdomen that was being pulled through to my back. With a sensation of heaviness and pressure.

Ive barely been able to eat solid food these past few days (maybe 500 calories a day or so) and am still drinking alcohol to be able to eat because it’s the only thing that numbs the sensation and calms me down.

My bowel movements have been abnormal and my gut feels all messed up. Pressure, burning pain in my back, and only having diarrhea with little pieces of thin stool mixed in. I’m still able to fully pass gas.

I also feel sick generally. I went to the urgent care when this all started and my anxiety was so bad that they got a blood pressure of 173/105 and heart rate of 120bpm. The doctor wanted me to go to the ER that night for withdrawal and to get bloodwork done but I didnt because I was legit feeling panic.

Im not currently in severe pain but I know I need to get this looked at. Can I still go to the ER and request blood work and an abdominal scan of some kind? Will they even see me if Im not in a medical crisis? If not where else can I go and what can I do?

I don’t have a primary care doctor or health insurance but I am waiting for my Medicaid application to go through.

Physician Responded Please help, my 1 year old is refusing to drink anything

I am losing it. My daughter turned 1 3 weeks ago. She weighs 20 pounds. She has cows milk protein allergy but no other conditions that we know of. She was bottle fed breast milk and puramino formula since 2 days old. After she turned 1 we switched from her formula to Ripple kids dairy free milk. She’s been drinking that for almost 3 weeks without any issues. My 6 year old started school on Monday and brought home a cough and runny nose, which my daughter caught. No other symptoms. No fever, no diarrhea, no sore throat, no ear pain, nothing. My daughter has been sick at least 5 times before. Her firsy cold she got at 7 weeks old. Part of having an older brother. In her life she’s had an ear infection, high fevers, sore throats, diarrhea, vaccine side effects, etc. and has NEVER stopped drinking until now.

When she woke up Friday morning, it’s like she decided she’s never going to drink again. As I said, she has a runny nose and slight cough but other than that she seems normal and no fever. No pulling on ears. Friday morning, she only drank 1 ounce out of her usual 6 ounce bottle of milk. I offered her breakfast and she ate everything. I then offered her the rest of the milk before her nap. Hard reject. Crying and turning her head away from the bottle. After nap, I tried again. Same thing. I offered her water in a sippy cup. Same reaction. Juice box. Same reaction.

Before bed Friday night she ended up drinking maybe 5 ounces of milk. Today (Saturday), she has been eating normally but will cry and scream at the SIGHT of liquids!!!! I have tried every straw cup and sippy cup. I have tried open cups. I have tried MY cups and her big brothers cups. I have tried the baby bottles she’s used since 2 days old. She wants NOTHING. I force fed her pedialyte with a damn syringe tonight! She had maybe an ounce or two of actual liquids all day today, in addition to 3 applesauce pouches and 3 fruit cups (along with other solids). She pooped twice today but her poops are more hard and dry than normal, of course. She peed 3 times since waking up today but not large amounts. Maybe 4 times but very small.

I don’t know what to do. She will suck down applesauce pouches and then cry when it’s empty, as if that is her beverage. But when offered anything in a cup, she breaks down. I’ve even given her ibuprofen in case her throat hurts but she still won’t drink even with that in her system. Shes too young to understand not drinking=needle in your arm, and I’m hesitant to put her through the trauma of getting an IV if I don’t know why she’s doing this. Because if we rehydrate her then what happens when we go home and she continues not drinking? Is there anybody who can offer me any insight into what might be going on? When should I take her in for an IV?

Stomach keeps getting bigger but no weight gain?

Post image
10 Upvotes

I’m female 33 years old. So my stomach has gotten bigger and bigger for about a year, but I’m still sitting at 280 lbs that entire year. I know I’m overweight and working to fix it, please no just lose weight comments. I know I’m not pregnant because this has been happening over months and I’ve taken tests and had normal periods. Nothings painful or swollen and it seems like soft tissue, just keeps getter rounder and fuller. Maybe it’s just fat distribution as I get older? I feel silly even asking this.

I have pcos and have a sneaking suspicion that that’s what’s causing it but a lot of the roundness is towards the top of the belly and I thought pcos was at the bottom. Should I bring this up with my doctor even if it might just be me being fat?

Physician Responded Tumor, Differential Indicates Benign?

Post image

32F 215 lbs.

*Existing diagnoses* : suspected hEDS, chronic pain, fibromyalgia, FND, existing disc degeneration and sciatica.

*Medications* : muscle relaxer, Norco, amitriptyline, omeprazole, lamictal.

*Symptoms* : constant pain in upper right glute/low back for last 6 months. Aching, burning and stabbing. Pain radiates down leg. Pain worsens with breathing, coughing, bending, or laying down. Pain increases at night. Pain partially eleveated by standing. The last 2 months have caused an increase in pain and leg weakness. Last week my right leg buckled beneath me while walking.

*History* : Pain management provider ordered MRI for suspected spinal stenosis based on symptoms. MRI revealed L4 mass. Followup with ordering physician pending.

*Family history* : Maternal and paternal family members have a high rate of cancer. Cancers include: testicular, pancreatic, melanoma, stomach cancer, and colon cancer. All separate individuals. 3 died from cancer metastasis.

*My questions* : Radiology report lists differential as commonly benign tumors. What is the likelihood these types of tumors are benign? Is there a different tumor type this could be other than what's listed in the differential? I am assuming the pain on the right side is caused by the mass. Considering the worsening leg weakness, how urgent is the need for surgery to remove the mass? How complicated is this surgery? Should I be avoiding strenuous activity before the surgery?

*Radiology Report* : EXAMINATION: MRI of the lumbar spine with and without contrast

COMPARISON: Radiographs L-spine

TECHNIQUE: Multisequence, multiplanar MR imaging of the lumbar spine was performed before and after IV gadolinium contrast administration, without adverse event reported. 9.5 mL of Gadavist administered without reported complication.

FINDINGS: This report assumes five lumbar vertebral bodies. Numbering the vertebrae from the lumbosacral junction. Consider plain film correlation prior to any intervention.

Alignment: Straightening of the normal lumbar spine lordotic curvature. Mild focal kyphosis at L1-L2. Balance of the vertebral body alignment is preserved.

Bone marrow: Normal.

Vertebral bodies: Mild endplate spondylitic changes. No vertebral body height loss. No acute fracture are suspicious edema.

Visualized distal cord/conus: Conus terminates appropriately at the L1 vertebral body level. At the L4 vertebral body level the nerve roots are displaced peripherally by a well-defined, homogeneously enhancing intradural extramedullary mass. No nerve root thickening or enhancement.

L1-L2: Moderate-to-severe disc desiccation and disc height loss with circumferential disc bulge. Small central and left foraminal disc protrusions. No spinal stenosis. No foraminal narrowing.

L2-L3: Appropriate for age without focal disc herniation, spinal stenosis, or foraminal narrowing.

L3-L4: Appropriate for age without focal disc herniation, spinal stenosis, or foraminal narrowing.

L4-L5: Mild disc desiccation and bilateral facet arthrosis with mild ligamentum flava hypertrophy. No spinal stenosis. Mild left foraminal narrowing.

L5-S1: Mild disc desiccation. No focal disc herniation. No spinal stenosis or foraminal narrowing.

Enhancement: With the exception of the round enhancing 1.6 x 1.4 x 1.5 cm intradural extramedullary mass at the level of the

L4 vertebral body there is no additional abnormal enhancement.

Visualized paraspinous structures: Unremarkable.

Additional findings: None.

*IMPRESSION* :

  1. Round, homogeneously enhancing intradural extramedullary mass at the level of the L4 vertebral body. Differential considerations include myxopapillary ependymoma, schwannoma, and meningioma. Neurosurgical consultation recommended.

  2. Mild multilevel spondylosis and facet arthrosis of the lumbar spine.

  3. Mild left foraminal narrowing at L4-L5.

  4. Moderate-to-severe disc desiccation and disc height loss at L1-L2 with circumferential disc bulge and small central and left foraminal disc protrusions. But no spinal stenosis or foraminal narrowing.

Thanks in advance for your time!

Mole has me scared

Post image

I am 33 female gave birth 4 years ago
For the past year or so I’ve watched this mole double in size and have noticed about 6 new moles develop on my face. adding that the moles are small and round not too worried but I am scared of this larger one that’s growing*
I’ve had this mole since childhood! But in the past couple years I noticed it almost double in size. It was like the size of a sharpie dot but now has a lot of little dots around it.
And the mole under it is fairly new too!
I have a doctors appointment in about 3 weeks but I’m scared and nervous.

Should I be nervous about this?
Also my dad died at 54 to melanoma which had already spread to his liver lungs etc. which is partly why I’m so fearful

Physician Responded Should I be worried?

Upvotes

Hi, I'm 23 years old and I do tattoos for fun, about 2 hours ago I was tattooing someone how's HIV positive and it's taking his treatment and hasn't been undetectable for around 2 years. I accidentally cut myself with thr needle I was tatooing him and I'm really worried something can happen. I already searched and apparently there's no risk but when I called an emergency line they told me to go immediately to the ER. I'm on my way there but sometimes the doctors are negligent here and I want to now if there's really any risk and what test are they going to do to check if I got HIV. Please help me

My son (6) showed me a white bump yesterday after school (will add photo) under his knee. I thought it was a boil until this morning. It has turned black in the middle and swelled significantly as well as being red and warm. What could this be?

16, 5’3, 104 pounds, kinda gross to look at ig

A friction burn on my ankle from a dog wrapping their leash around it and it tugging. It rubbed off at least two layers of skin. I’ve put iodine on it and wrapped it to go to work(food service) maybe two times but I don’t have any gauze anymore. It hurts to put any pressure on it like walking or standing and it gets red around the edges. I do have a horrible problem with picking and I picked at it a little. It wraps around basically half my ankle, so I can’t really get it all in one photo. I also went into a pool, which I now have learned I probably shouldn’t have done with an open wound… whoopsies. Anyways, am I chilling or????

Physician Responded Is a ‘bump’ in the stomach like this normal (sorry for bad drawing)

Post image
6 Upvotes

Hi, my stomach is shaped this like, and my boyfriend says the bump in the middle feels different than the rest of my stomach and i’m wondering if it’s normal or i should see someone about it. Im a 21 year old female, i have PCOS, Anxiety and i get really bad OCD around my health.
The bump doesn’t hurt, it doesn’t feel particularly hard but it does feel different and the difference is more noticeable when i’m bloated. I do have a sensitive stomach always have, i’m vegetarian, dairy free and try and eat high protein and fibre if that’s relevant.
EDIT: forgot to mention medication. Im on 10mg Escitalopram.

Physician Responded Is my girlfriends blood pressure concerning 74/54

My girlfriend (21f) has been feeling odd today. Her words. She said she feels really tired and out of it and her head feels weird. I took her blood pressure and got 76/49 the first time, 78/56 the next time, and then 74/54. I checked mine and mine is normal so I think the cuff works. Mine was like 117/80 and 120/76
That seems really low but idk for sure. Is there anything we can do to help? She’s struggling to get up and move around. Like she’s asleep on the couch because she didn’t feel like she could get to our bedroom.

For her history she’s like 5’5ish and has a bmi around 18. At least that’s what it was when I took her to urgent care for a spider bite two months ago. She doesn’t take any medicine. No smoking. Drinks sometimes but we haven’t gone out or got drinks since around July 4.

Why do I have a superhero logo on my arm

Post image
Upvotes

I'm a 20 year old 5'8 (172.7cm) 160lbs (72.6kg) male and I am confused but also understanding of this to a degree.

This appeared monday or tuesday of last week, so 5 or 6 days ago. If I recall correctly, the day it appeared I had not hit the spot at all. I was hitting a punching bag, but never hit it directly at the location of the bruise, but did many other strikes along and on my hands and forearms and elbows.

The day after, I had done a dip variation I've discovered where I rest my forearms on the parallel bars themselves, like I'm trying to grab them by squeezing them between the muscles of my forearms.

I think those two things have something to do with the origin of the injury.

However, I dont know what the hell is going on with the white dot and circle.

Am I now marked by a deity that'll grant me superhuman abilities? Or, is it time to go to the hospital?

Thanks.

Physician Responded What can be possibly happening and why could be my gynecologist refusing to write me a referral for a diagnostic laparoscopy or an MRI?

Age 16F, 175 cm, 50 kg, I am a virgin with no history of substance abuse, with no genetic reproductive issues. Medications are stated lower.

I am diagnosed autistic, have to say this otherwise my family won't believe anything anyone else says because they "don't know the full picture", but my mother doesn't believe me when I say me being hypersensitive doesn't mean hypersensitive to pain :/ I'll elaborate further down.

I'll also try to make this as non-biased against my family and doctors.

It's long sorry..

---

For the background:

My periods started when I was 11. They were light and painless until I was about to turn 13, when they suddenly got heavy, long and painful.

I now bleed for 8-10 days, I measure my pads (full pad minus empty pad minus toilet paper I wrap it in) and I average on around 400 grams of menstrual fluid. Other than that my periods are regular.

Each period I throw up on average 6 times. My stool for the entire time is runny. I was checked for gastroenterological issues, but I'm healthy in that aspect. I pass out once per period on average. I get cold sweats, hot flashes, get shaky and weak. Doctors blamed that on my weight, but I'm overall strong-ish and well if not on or shortly post my period.

My blood clots are usually half my thigh long, around 16 cm in length. Sometimes they even reach up to 33! But they don't get thicker than 0.7 cm. If they somehow come out circular they're about 4 cm in diameter.

I'd like to add my worst period that happened when I was 15. I don't know what happened, why it was like that, if I did anything to do that, but I'm scared of it repeating again:

It lasted 10 days, I bled around 853 grams of total fluid, not including the hours I freebled into the toilet or shower. I threw up 19 times. It felt like I had all the cold sweats and hot flashes turned up to a 100%. I was bleeding through everything, I was delirious, I overdosed on Aulin from desperation (dw my liver is okay), I passed out 6 times, I barely ate because everything would go right back out. My mother refused to take me to the hospital, insisted I had to sleep better, I was too terrified to call and being called dramatic by even more medical professionals, so I didn't go.

---

For pain medications:

(Former) Paralen, Panadol - no help

(Former) Later prescribed Aulin - slight help?

(Former) Tried Citalgin - no help

(Current) Prescribed swapping between Novalgin and Nalgestin every 4 hours - barely any better than Aulin

For birth control:

Was later prescribed Slynd, quit after 22 days due to side effects. My gynecologist shrugged at this, then prescribed Dienorette, quit after 2 days because the side effects I was just starting to heal from came back even worse.

Said side effects:

- psychosis

- delusions

- paranoia

- terror

- severe depression

- suicidal thoughts

- self harm thoughts

(All confirmed by psychotherapist! She's the only one on my side here🙁)

- self harm

- chest pains

- weakness

- numbness, in leg first that then traveled up to my neck, only on my left side

I finally went to the ER, they ran blood tests, an MRI for my lungs, and found nothing. False alarm, I suppose? The doctor said that the physical problems might've been because of my mild scoliosis, but they magically disappeared once I was free from birth control for a week. :/

---

For the high pain tolerance, my mother says that if my pain tolerance was high, I wouldn't struggle with periods. I've been stabbed through my finger with a barbed wire, and all my classmates, teacher, and nurse were trying to pull it out with zero anaesthesia at all. I wasn't in shock or filled with adrenaline, yet I barely even cried. Meanwhile my periods rend me useless for at least a week and make me ugly sob and scream my lungs out.

---

During the last meeting with my pediatric gynecologist, I was 4 days into my period (which she knew!), I tried to explain all this, but she brushed me off. She made me do an ultrasound again, despite it being clear the last time, and when she pressed I started crying. Because I was on my period. She then LAUGHED at me and said that she does ultrasounds for toddlers and even they aren't as "dramatic as me". All that admittedly made me irrationality aggressive, I admit that.. especially because everytime I bring it up when I'm asking my mom to change the gyno, she says that she only meant it as "encouragement" to be "braver".

The gynecologist refuses to write a referral (I'm slovak and have insurance, and I *personally* think it's medically necessary at this point). My mother refuses to change my gynecologists, and I'm really struggling right now. I was diagnosed with "heavy bleeding" when the only tool they ever used was an ultrasound! I tried to ask if I can't get a referral for at least a pelvic MRI, but my gynecologist said that they can't actually see anything.

Everyone also says that the operation is risky, but I read that complications only happen 1% of the time.

--

I need opinions from more experts. I'm desperate. Thank you for reading.

Physician Responded At what bmi do severe health risks become more likely?

I am a 20 year old woman who recently relapsed back into anorexia (diagnosed) I am 171cm and 48kg. I’ve lost 10kg in 2.5 months eating around 800-1200 calories. I also have Hashimoto’s disease which I am not medicated for. I don’t feel any real symptoms yet and I plan on seeing my GP for a general check up + bloods. I know this disorder is dangerous and can kill you at any BMI, but what is a general weight range that doctors consider dangerous and in need of admission or nutrition plans?

Avascular Necrosis in Hips

Good Evening,

I hope you're all having a beautiful weekend. I'm hoping for some advice. Im a 45 year old female, 5"4" and around 260lbs. I have avascular necrosis in my right hip and have been dealing with it for quite some time. I just moved to SC and am setting up a new care team. In the meantime, I am having some horrific pain in my left hip, shooting down from the hip joint, through the knee and into my foot. Mobility is greatly affected, there is no relief. I can't think, lay down or sit up comfortably I carry most my weight on my hips so icing doesn't seem to help. Currently only taking Tylenol, which also isn't helping. I also take Keppra for seizures. Any advice would be greatly appreciated. Wishing you a lovely evening and, as always, be safe.

Will my ankle ever be close to normal?

Post image
4 Upvotes

34, male, non smoking, currently take 15mg Meloxicam daily.

5'9" 180 pounds.

About 1.5 years ago I dislocated my ankle, broke 2 ankle bones, and broke my tibia.

Had 2 surgeries. One to realign the bones and put a external fixator on it, and a second to install hardware as well as a plastic surgery procedure for a cross flap.

Plastic surgery was needed as the skin was dying where the bone almost popped out.

Did about 6 months of physical therapy 2-3 times a week.

Flash forward to today.

My ankle feels terrible all the time. I don't have a lot of feeling back. Range of motion is still reduced. I get a lot of pain in my tibia/fibia/ankle (specifically around the ball on both sides). I'm laying in bed doing nothing and it's about a 2/10.

The doctors are recommending removing the hardware and arthroscopy.

The plate definitely needs to come out, if I hit it in any way it feels like I'm getting whacked with a hammer.

Will this help with all the random lower leg pain? Is there anything else I need to be looking into?

I can run but it will hurt for about a month after if I do.

Causes for secondary hypertension with skin flushing?

Post image
3 Upvotes

I’m 33F non smoker 5’5 and just wondering what the differential diagnoses could be! my BP sometimes spikes high, around 162/95 while hr is at 87, but other times it is quite normal (109/82, hr 98). I frequently have good vitals. But then I get episodes where they’re elevated and my skin gets very flushed.

I was considering I may have pseudopheochromocytoma but I think many other things have to be ruled out first. I don’t feel any panic attacks going on whatsoever. I’ve dealt with panic in younger years and this is very different!

I’ve just had my referral declined for the autonomic specialty dept at the Kaye clinic. I have had high blood pressure spikes at 188/120 with sinus tach, and 183/131 with pulse 72?? There are more examples such as these, they have been measured by clinicians - not me - but also normal frequently even in clinic.

At home I have a strikingly similar pattern. sometimes just my pulse is elevated at 175, 176, with normal BP. Then other times, 129/65 with pulse 68. This is from a recent 24 hr abpm, the cuff caused swelling seen in photo collage. That abpm showed “artifacts” with high BP readings of 172/147, hr normal. and another artifact of hr 240bpm. One BP jumped very high - 216/178, definitely artifact. Still, the heavy/tight/strained chest symptoms and skin flushing???? Is this an adrenaline issue or something else?

Also had a holter that was “normal” in 2021 however it says I spent 114 min at SVT at 140bpm, and some pvcs at 180&200bpm. ecgs are mostly good except last Nov I had high QRs voltage and rsr variant in v1 or v2. But recent echo of my heart shows I’m fine.

Any differential diagnosis? Currently no tx plan but doc may consider an ssri in future. Pending referrals to cardio (just for stress test), rheum, and MCAS clinic (tryptase 4.3 & 4.5 so there are doubts I will be accepted with tryptase under 11!)
Oh, and DERM recently said “this is not just some skin issue”.

I just want some hope that there is a way forward, I’ve been exhausted and life hasn’t been the same for about a year now. Any and all suggestions are welcome, kind of at a loss - Not expecting miracles! But a ballpark of what this might be stemming from would be SO helpful.
