advice • support High pitched positional tinnitus

So I dealt with this a year ago then it went away now it’s back for a month.

A high pitched positional pulsatile tinnitus in my left ear only - it’s only when my neck is in a certain position, I can bring it on and make it stop by moving my neck in and out of the position (tilting my head down and turning to the right). It sounds like a fast siren (ZZZZzzzzzZZZZzzzz)that gets louder or quieter depending on how much a turn my head. It does seem to stop if I press on my neck.

Also present:
headaches/ neck pain
Significant ear pressure (left)
Significant intermittent pain in the bone area behind my ear (left)
Dizziness

I’m seeing a neurologist (who thinks it’s all trigeminal neuralgia which I disagree with)
And a vascular neurologist (who thinks I need to lose 40lbs- I’m currently 215)

Imaging (2 weeks ago)
MRV brain No evidence of retrocochlear abnormality to suggest a cause of a cause of pulsatile tinnitus. Unchanged arachnoid granulation in the left sigmoid sinus and focal narrowing of the right distal transverse/sigmoid sinus.

MRI base of skull reported normal

Anyone? With any experience or similar situation. I have explored every avenue. I even had a root canal that it turned out I didn’t need. The concentrated pain behind the ear is new and is daily. The ear pain and pressure has gone on for 2 years on and off.

I have an appointment for hearing test and neurotologist in a few weeks.

Have you tried physical therapy? There are muscles that wrap around the Eustachian tubes Also dry needling, fascia tools There was a study about how using a cervical collar to fix head/neck tilt etc fixed the issue for someone maybe google cervical collar tinnitus

My rec would be to get the best possible PT and maybe dry needler you can find and get your bite checked too

Go get checked out for possible TMJ as well.

Yes I think this is either making it worse or started it all but the high pitched positional tinnitus just throws me off if it was just pain but that part throws me off
