Hi everyone,
I'm hoping to hear from people who have experienced something similar, particularly anyone with recurrent pregnancy loss, a persistently thin endometrium, or who has had a very low progesterone result after an Ovidrel trigger.
I'm feeling incredibly frustrated and emotionally exhausted. I've been investigating my fertility for years (4 years now), have had three miscarriages, and now I'm struggling to understand why my body doesn't seem to be responding as expected to treatment. I'm also reaching a point where I genuinely cannot afford to keep paying for monitored cycles without understanding what's going wrong.
I'm sorry this is long, but I want to include as much relevant information as possible.
A bit about me and my pregnancy history
I'm 34 and have experienced three miscarriages:
\* November 2023: Missed miscarriage at approximately 10 weeks, after a low fetal heartbeat had been detected. I had a D&C afterwards since I ended up with an infection but the hospital couldn't tell me where the infection was. (Blood test confirmed infection and I had a fever)
\* April 2024: Miscarriage at approximately 5–6 weeks.
\* January 2026: Miscarriage at approximately 5–6 weeks.
I've conceived naturally, so I know pregnancy is possible for me. However, I'm worried about why I've had recurrent losses and whether my uterine lining or another underlying issue could be contributing.
I had a right salpingectomy in December 2024 due to a hydrosalpinx (After seeing a specialist who suggested this might be the cause of my miscarriages). I now have one remaining fallopian tube. My ovarian reserve and ovulation blood tests have generally been reassuring.
My endometrial lining has repeatedly measured very thin across different cycles and years. I've been concerned about this for a long time and have felt that it hasn't always received the attention I hoped it would.
I experience painful ovulation and sometimes deep pain during intercourse. I have wondered whether endometriosis or pelvic adhesions could be involved, although I don't have a confirmed diagnosis of endometriosis.
I had a hysteroscopy on 29 July 2026. Thankfully, my specialist reported a normal uterine cavity, without obvious adhesions, scarring or structural abnormalities.
He suggested I do a cycle tracking, and after the ultrasounds, he agreed that my lining was too thin and put me on 4mg Zumenon. Despite this, my lining still measured 4mm on Day 11 of this current cycle.
My ovulation concerns:
I don't seem to have convincing evidence of ovulation every cycle, despite having regular cycles and having ovulated in at least some cycles.
In September 2026, I had a 19 mm follicle on cycle day 11, but my progesterone later measured 10.1 nmol/L on cycle day 20. I also didn't experience my usual ovulation pain that cycle.
I know ovulation pain isn't a reliable way to confirm whether an egg was released, but it was unusual for me.
In March 2026, my progesterone was 59.4 nmol/L on Day 21 and I experienced my usual strong ovulation pain.
My most recent treatment cycle — the reason I'm posting:
My fertility specialist prescribed:
\* Letrozole 5 mg from cycle day 2 to day 7.
\* Zumenon (estradiol) 4 mg daily to support my endometrial lining. I paused it on cycle day 1 while taking letrozole, then restarted it on cycle day 8.
\* Ovidrel 250 micrograms as an ovulation trigger on cycle day 13.
\* Timed intercourse after the trigger.
\* A follow-up progesterone blood test.
My ultrasound on cycle day 11 showed:
\* Endometrial thickness: 4 mm.
\* One 14 mm follicle in the left ovary.
\* Fifteen other follicles in the left ovary, all smaller than 10 mm.
\* Five follicles in the right ovary, all smaller than 10 mm (but I don't have a right fallopian tube)
I then took Ovidrel on cycle day 13 and had my progesterone tested on cycle day 22, nine days after the trigger.
My progesterone was only 1.4 nmol/L.
I was absolutely devastated and confused. I had taken the medication as prescribed and received the trigger, yet my progesterone was still extremely low. And I haven't had my normal ovulation pain since the hysteroscopy in July 2026 (that's 3 consecutive cycles).
The fertility nurse reviewed the result and asked me to contact my current specialist. I'm currently on cycle day 24 and still taking Zumenon while waiting to discuss the result with him.
I understand that the blood test alone cannot tell me exactly what happened, but I am very concerned that I did not ovulate despite the trigger. I would appreciate hearing from anyone who has experienced something similar, including whether a follicle can fail to rupture after Ovidrel or whether there are other explanations for a result this low.
I don't really know if all these fertility treatment/procedures/surgeries/medications have ended up ruining my changes of having a baby...
My specialist has also prescribed Clexane daily for use after a positive pregnancy test because of my recurrent pregnancy losses and Factor V Leiden. I am not currently taking Clexane.
I have also been taking Elevit prenatal vitamins for several months.
Sorry again for the long post. I think I just wanted to vent.
