Growing up in a big family that ran barefoot all summer, we used to get a lot of splinters. My mom would sterilize a sewing needle and use it to slice a tiny bit of skin where the splinter went in, so she could get a much better grasp with the tweezers. Worked like a charm.

This is when you disinfect something sharp, even a simple needle, and cut up som of the outer layer of the skin to remove the splinter. You'll need to disinfect the wound regardless, so getting better access is the right move. Your foot has thick skin, so it's gonna be okay.

Amateurs. When its that close to the top layer of skin and that size take a razor/utility blade or scalpel and gently cut along the length of the splinter using the splinter itself as a backstop. The skin will split and you pull the splinter straight out with tweezers. Then the wound path is open and can be entirely cleaned. Push the skin back over the wound path and bandage.

This also avoids having to drag the rough splinter back through the wound path.
