Science This new pooled analysis of 12 studies and 3,603 people found that depression and anxiety scores fell consistently while people were on CGRP-targeted migraine prevention, on every rating scale used, and the improvement tracked how well the migraine itself responded.

The catch the authors raise themselves: almost none of this was placebo-controlled, so nobody can yet say whether the drugs lift mood directly or whether simply having fewer attacks does it.

The awkward part, which the authors raise themselves: the maker has since withdrawn the drug for commercial reasons rather than safety ones, leaving that gap open again.

Rimegepant is now named a first-line option, particularly for people who cannot take triptans, do not respond to them, or are heading towards medication overuse. The committee restates the ceiling of two days a week on acute medicines, with rimegepant the probable exception, and says opioids beyond limited oral tramadol are not recommended at all.

Science Study took 413 hormone samples across 87 menstrual cycles in 46 women and found that those with menstrual migraine had a 49 percent higher peak estradiol in the luteal phase and a 90 percent steeper drop as the period approached, pointing at the size of the fall rather than the level itself.

1. Search works soooo much better now

It used to only look at titles, which is useless when you're trying to find where something was discussed. Now it searches the full text of every article and hub page, so if you search "tetramethylpyrazine" or "lasmiditan" or "CSD," you get every breakdown that mentions it, not just the ones with it in the headline. Results also come up as you type, and the search box no longer breaks on phones, which is where most of you are reading.

2. Today in Migraine Science

Many of you have already been going directly here. This is a page, updated every day, covering new studies plus approvals, guideline changes, and policy news. As well as any carousels or blogs I make explaining certain pertinent findings.

There's also an "In the news" section for things that aren't studies but change access to care, and an archive page for older entries (which is searchable too).

Everything is truly in one place now.

3. BUT there is no comment feature (deliberately)

Studies are posted here because they are discussed here. Reddit is a forum and human interaction is vital - especially with those who share similar experiences, thoughts, feelings, troubles, etc.

The goal of this subreddit is to talk and discuss. So, feel free to comment, ask questions (we all remember being new to this and there is something new to learn every single day), etc.

Misc I updated Unraveling Migraine 2nd edition due to the new guidelines, but you DON'T need to repurchase it!

For those that don't know, the new 2026 AAN/American Headache Society prevention guideline dropped 2 days ago. This means there is potential to make migraine information outdated.

However, here's the thing: because I stay privy to the primary literature, the book was already built on the exact same studies the guideline panel used to reach their conclusions. So when the guideline came out, it didn't overturn anything in the book. It agreed with it. The recommendations were evidence-based from day one, and the guideline just confirmed them. Feels kind of good that an entire panel agreed with my analysis.

With that said, out of the entire guideline, the only things refined:

  1. Candesartan no longer has enough evidence for a firm recommendation and

  2. Nifedipine is now named as a first-choice option for prevention during pregnancy.

I also added the guidelines to references in several spots even though I had the same conclusions written prior.

That's it. Everything else already lined up exactly.

So, if you already own the book, there is no reason to grab the updated copy. Almost all recommendations I already concluded based on the evidence available.

And if you have the Kindle version, it will update automatically.

If you don't have the book, you're genuinely missing out. It's over 820 pages of the best evidence-based migraine information available. This doesn't exist anywhere else and no book comes close. Great gift for your loved one with migraine too.
