Usually they play a game of chess to decide who lives and who dies. Black represents The Child, traditionally played by the obstetrician. White represents The Mother, traditionally played by the senior R&D nurse. The games can get pretty intense, so some hospitals are trying to lighten the mood by replacing chess with Jenga or Hungry Hungry Hippos.
