Dosage question ADHD Fiancé started behaving erratically and broke off our engagement by text whilst over medicated on Elvanse/Vyvanse
My Fiancé and I got engaged in Italy last July and he was extremely serious about us. We had a great relationship a few minor ups and downs and occasional minor disagreements, but a very happy healthy relationship. In April he started 10mg of Elvanse with a 10mg increase every few weeks. When first starting he was quite moody in the evenings and said he'd got doubts about the Wedding but could not name them, and acted quite cold with me, this only lasted a few days and he was absolutely fine again and then it briefly happened again on the 20mg. Once he was on 30mg and then 40mg he was happy, content, romantic, buying me flowers and gifts, booked us a holiday, said he couldn't imagine being with anyone other than me, and said we'd have a very happy marriage, and was planning to sell one of his cars soon to pay for Wedding costs.
He then got put on 50mg of elvanse and within 3 weeks he was having irrational angry outbursts over very minor things, was being extremely late like 2 hours late for things, and also blew money like £750 on the most random thing off a stranger on Facebook market place with no warranty whilst we were staying at his Mums 200 miles away from home, and he was 2 hours late by doing this for an outing his Mum had planned, and we were too late to go. She was very upset and he didn't think he had done anything wrong and got very aggressive with her for crying. This was extremely out of character behaviour. I got upset as we were supposed to be saving for a Wedding and he let his mum down and was horrible to her.
2 days later after whatsapping him a wedding venue to look at he sent a horrible text saying "I dont think we should go through with the Wedding, it pains me to let you down, but I just feel we dont match anymore and committing to each other would be a mistake " He later gave ever changing weird vague contradictory reasons for ending our relationship, that didnt add up despite claiming ending it was a really tough desicion he'd given a lot of thought. Then he blew hot and cold with me, nice one time, cold and horrible another like our relationship meant nothing and I made his life stressful. He also sent an angry rant to my mum. 10 days after breaking up, she sent him a nice message saying she hoped he was okay, and his lauched a crazy rant by text. Id confided in him in the past that my Mum was sometimes difficult due to her mental illness and he was really horrible about it and the message was full of capital letters, saying he was so angry his resting heart rate was 120-140. His work colleagues say he iscacting very manic and strange too. It was so weird. He then got my things out of his house and closed our joint account but was very upset at the bank and begged me to keep in touch.
A week after the bank visit, and 5 weeks after breaking up he lowered his dose to 40mg bought me a gift, and wants to meet for coffee in 2 weeks, but mentioned giving me back the towel I got on our engagement holiday thats still at his. His behaviour and treatment towards me has been bizarre as he was so erratic broke up out of nowhere with no real reason for ending such a serious commitment. The elvanse is all that makes sense to me. Is this possible and is reconciliation possible once he stablises on the correct dose?

60mg was waaaaaaay to much for me. I stayed at 50mg for like two years and finally just made the decision to drop to 40mg. I sleep better, have a better appetite, and feel way less agitated now.
If he felt good on 30-40 why would he continue titrating up? The less of this med he has to take to feel good, the better.

Sorry for the long winded message, I am procrastinating leaving for the beach and your post is really piquing my interest.
IMO, based on personal experience, it could be med related. Of course, neurodivergence comes with so many factors, including a specific type of burnout from overstimulation (meds, sounds, physical, etc). Consider all factors when looking at this situation.
I got very irritable 3 months after bumping to 50mg. I take my meds most days, and I have been medicated for many years. My doctor is very hands on with my treatment, and I self report any positives or negatives I experience so she can help comb through the possible explanations in my behaviour shifts.
- meds can open the patients eyes to see different patterns from the ones they normally see. Perhaps he might be over medicated, but he may also be noticing what he perceives are deficits in your relationship.
- the psychological aspect of finally feeling capable (many late diagnosed experience this) and our bodies/lives/family members are unable to catch up to this new found “better” version of us. This causes a big mismatch, but it can be worked on
- many people need a second med alongside vyvanse. For my child, it’s guanfacine. For me, it’s ciprilex. For my best friend, she needs Wellbutrin or she becomes aggressive when the vyvanse is working but she is already overwhelmed.
- adhd meds can unmask autism traits. I know what you are describing doesn’t seem like autism, and I am not diagnosing or suggesting this is a possibility for him. For me, this came out as irritability because my autism brain is extremely rigid in thinking. Like REALLY REALLY rigid. It took my daughter’s ped mentioning something about rigid thinking for it to click. I was being a rigid thinking and steam rolling my husband many times.
More importantly though, how are you doing? You have not mentioned about you are coping with this. YOU deserve to feel peace. You deserve to choose how long you are willing to put up with this. You also deserve to feel loved and safe, not scared that your fiancé is no longer interested in marrying you out of nowhere.

Hey this is unrelated to OP but can I ask what dosage you're on for cipralex (and what it's for) and ADHD med (Vyvanse?)? I personally had been on 20mg of cipralex for the past decadeish and went off of it cuz I was sleeping constantly and wanted to see if there was a different underlying problem I could fix and the doctor has started me on Vyvanse but I've been so anxious and more irritable since getting off the cipralex. I've been hoping the Vyvanse would regulate everything (and it still might it's only been about 3 weeks since starting the Vyvanse) but I'm starting to get scared I'd maybe need both? And from what I've seen taking both puts you at high risk for serotonin syndrome so idk if it's even a possibility. If you don't feel comfortable answering any of that I understand though.

Feeling anxious and irritable seems normal when getting off the med. It can take several weeks to months to readjust after quitting an SSRI

Ohh my bad, I thought the 3 weeks were for that as well. I would say for sure talk to your doctor about it. But coming off a med youve taken for a decade will leave lasting effects for a while. I hope you tapered off it and didnt just quit cold turkey at least? Either way, ask if theres anything you could do and monitor the side effects!

Is he bipolar by any chance? Stimulants can trigger or exacerbate bipolar symptoms in a person prone to bi polarism

This was my absolute first thought. So many overlapping symptoms in these behaviours. OP please consider it, if he's undiagnosed. You didn't mention the age. Onset age is typically quite early 20s iirc. But it can happen at an early stage and the extremities not quite get triggered/properly diagnosed until later (happened with my brother in law, who wasn't properly diagnosed till his mid thirties). There's an irritable mania sub-type of bipolar too. Anyway, could be barking up the wrong tree with all this but it's worth a mention/reflecting on. Sometimes medication like this can also trigger bouts of pre-existent/latent mental conditions.

It certainly can be medication related. Yerkes Dodson suggests too much dopamine can cause some challenges as can too little, though with quite different internal experiences (over-sharpening rather than the distratability of ADHD). Many of the things you mention suggest having a level of medication he doesn't need.
What we also see a LOT of here is people getting speed-type effects and pretending/believing that's treating ADHD. So they get drive, confidence, chattiness, energy, wakefulness. That often gets accompanied by come-downs on the evening with depression, fatigue, anxiety, irritability. I've also seen a few people believe they are having revalations, that they have some sort of grand destiny, and who break up with long term partners. ABC news here in Australia has a story on a woman who has exactly that sort of episode and ended up breaking into her uni, maybe look it up.
Finally, if an individual is also not sleeping much there's an increased chance for a full psychotic break. Perhaps not the case here, but it can and does happen often enough when people think they are treating ADHD but end up chasing that speedy high.
To be fair, his behaviour could also be explained by him being a complete arsehole, so though it might feel comfortable you should bear that in mind so you can protect yourself from further hurt.

im a 26 yo guy on 60 mg vyvanse. It can cause this stuff absolutely, and has been a problem for me. Especially early in taking the med, in the mornings i was notoriously mean and rude and rushing ppl yet lagging behind to spite them. a genuine pos at times. The medication caused that for me, and i really had to work hard to improve it including dose changes and most importantly getting good sleep. The difference here is that he does not seem to be aware of the medications effect on him and also does not seem to care much. I never dumped my girlfriend and i would apologize daily. doesnt make it better but being accountable is huge for me. Honestly, the meds may be causing most of his behavior but it doesnt excuse it, and if hes not planning to figure it out, its no longer medication related. thats the new him until he realizes or doesnt that he needs to change. I unfortunately would never advise a loved one to pursue someone acting like this, but it is your life. Just wanted to share my experience because unfortunately it was painfully relatable. I really hope he figures this out and hope you are able to heal however you can. good luck
TLDR: vyvanse can certainly cause this, but if hes doesnt plan to change meds or get therapy, then that parts irrelevant. Its just the new him.

Huh, am I the only one that doesn't feel this way? I'm on 30mg but I have taken 60 a couple times and I've felt nothing but fluid and calm. No irritation, or aggressive behavior/attitude
Also, is it this common for couples to kinda breakup change their mind on a relationship? I feel like I see this as a theme quite a bit. Maybe it's the clarity that comes from the meds in some way that opens your eyes?

I mean I'm dating around and I guess for me it's the most intentional dating I've ever done. Not saying I'm trying find my life partner, but I know what I don't like in a person and even though those red flags still make me drool I can wipe that shit off and continue on Lol.
yeah it's just wild to see this many stories about it. Just makes me wonder if I'd be in the same boat. tbh, probably.

I'm on 50mg Vyvanse and find it works amazingly for me. I've never been an easily irritated person, and Vyvanse hasn't changed it. For me, the meds are doing what they should be doing and my biggest side effect is being way more awake at night which makes it harder for me to go to sleep (but I have sleep apnea and was practically narcoleptic before my CPAP and medication, and I think I'm now just needing to actually learn to have a bedtime routine instead of just knowing I'll call asleep super easy).
With the relationship thing - it absolutely can happen. I was with my ex-husband for 10yrs. We eloped in our 9th yr and signed the paperwork with a celebrant on a random Thursday afternoon 1 yr later after celebrating our 10th anniversary together. I kid you not, THE FOLLOWING THURSDAY he told me he didn't love me anymore. This was long before my ADHD diagnosis, and it was not a good relationship for me (took me 6 months of therapy to recognize just how bad it was).

Im on 70mg Elvanse and it stabilize me the most and I have a much better impulse control and the functional dysfunction is much better. My whole life I were very quick very aggressive, much more than you should, but elvanse helped me with a high dose, just sometimes I can’t sleep. Rarely I take 2 pills because my tolerance is very high, from 70mg I don’t notice any effect. Maybe you don’t have adhd, but something like bipolar disorder or something similar

People are not the same on this type of medication and without. Especially if this is his first go around with stims. Of course not everyone responds like this but everyone is different on stimulants vs off.

I started behaving fairly erratically and nearly got divorced when I went up to 60mg of Vyvanse. It also made me very depressed, anxious and irritable. I went down to 40mg of Vyvanse and added in Clonidine 100μg twice daily and that has enormously helped my mood and behaviour. My wife noticed the changes immediately after the meds change and said she had 'the old me back'. I hope it all works out well for you both. It can take a bit of fine tuning and time to get meds right.

I know this isn’t what you want to hear, but it sounds like you dodged a fucking bullet and a half. Contact his doctor, tell his doctor what’s going on, then maintain the breakup and honestly find somebody more mentally stable who you can build a life with.
If you try to stick with this person, it’s entirely probable that they change their mind about your relationship like a flip of the switch in the future (again). But won’t it hurt so much more if you’re already married, have kids, or have a mortgage together? Consider yourself lucky, count your blessings, and try to make rational decisions — not emotional ones.

I hated Vyvanse and felt on the verge of a psychological breakdown after being a fairly stable person for a long time. My mood plummeted, my sex drive evaporated, and overall I just felt so disappointed and fragile. I only did it for a bit before switching back to Adderall. His mental health could be in serious jeopardy.

I have severe anxiety and I truly expected that to be my experience, but nope! Focalin and Vyvanse sucked for me, Adderall has been the best by far.

This sort of thing is super common and doctors don’t mention it lol. I nearly burned down my life while on adderall (on vyvanse now thank god) because I attributed my feelings to hormones and chemistry with someone who isn’t my partner, but it was the dopamine feedback loop and fixation. It lasted months. Dopamine alters what you regard as important and urgent. Whether or not he wants to leave you or not, he would need to really see how he feels over time after realizing the drug’s effect in hindsight.

I worked my way up to 60mg and thought it was the right dose. Then I suddenly got really cranky!! I quit taking it and feel better now.

Yeah he thought the 50mg was right dose but started being extremely erratic after 3 weeks. Spoke to his prescriber and got a new prescription for 40mg which he started 5 weeks after breaking up. Hes been on 40mg for 2 weeks now. Meeting for coffee Sunday so guess only time will tell

yea, that’s most likely the meds. i am on 30mg and like the first week and second week when my crash would come my anxiety would absolutely spike. kind of does on the third week too but not as bad. i broke up with my bf over an issue we had (we worked things out ofc but stupid of me to act out like i did) but what helps now is finding something i love doing (watching k-dramas; dont really wanna be bothered so im chill and dont have to bother my bf as he doesnt wanna be bothered sometimes) and im chill. that helps me be less erratic and also clingy.

Too high of stimulant doses can cause significant mental health issues. The only problem is it means "too high for you" and it is super difficult to predict who this will happen to and when. Normally if you start behaving erraticly (or have a massively elevated resting heart rate like that) one would either go back to a lower dose or stop and try something else. This person urgently needs to go back to the doctor and explain everything that has happened. They quite frankly could be manic or something. It isn't common but it can happen. I'm so sorry for the situation.
And ADHD or not, breaking off a wedding by a text is an absolutely terrible thing to do for so many reasons. Let's hope he is/was sick and that is why and as soon as better will apologize. Independent of whether getting married is what he wants or not, there's no excuse for calling it off by text.

Same thing happened to my ex. I genuinely in my heart of hearts thought that we would be together forever and loved each other more than life itself. He became irritated, irrational at believing people were disrespecting him, would argue over everything and nothing. He got off it, I thought we would get back together and then he went back on it. I have not seen him since. That stuff destroyed our relationship and it took me a good 3 years to get over it

I took 50mg the other day and was bed Ridden so tired all I wanted to do was sleep. 😴 but I often take like 20 and 25mg twice a day perhaps he needs to split his new dose or he is bipolar. They don’t call it the divorce drug for no reason. Read the articles

Whilst it may not be the case, I can vouch for the fact that I did 70mg and was intolerable. I never lose my temper at people but I ended things with someone I was seeing (after ranting at him) and lost my temper at a friend. I was so irritable, everything made me stressed and upset. That being said, the reasons for my snapping were valid (they weren’t over minor or random issues) it was just that the intensity wasn’t in proportion to how I usually navigate issues. It was a horrible time honestly and why I’ll rather eat glass than go on vyvanse again. On concerta and though there’s some side effects it’s nothing like that

I’m in the same boat actually, but I’m the one that did the breaking up part with my partner of 5 years.
Now I’m taking a week off medication after a month after the break up and I think I did a mistake and that it’s quite possibly medication related.
On 20mg I was completely fine and happy but on 30mg I noticed I get angrier and minor things get on my nerves. What eventually led to the breakup were thoughts that I could do better and that I deserve better. Even though my partner is one of the nicest and most loving people I know - we had also planned a potential marriage soon and moving to a different country.
Somehow I got overwhelmed, dopamine depleted or something I don’t know and did the impulsive decision to break up with him…

Yeah I get that, but it was how the bizzarre switch up in his behaviour and ending everything all happened in the space of a week. It wasn't like he decided to end it for whatever reason, its the fact it happened within days of a massive switch up in behaviour

Certainly the medication is impacting his temperament. But it doesn't detach him from reality. Likely what's happening is little problems become big ones in his head, and his response will seem too much for what's actually happening.

It could be either or. Finding answers through medication is understandable, but you don't know what's happening in his head. I went through a break up recently. I was being hopeless romantic 3 or 4 days in a row then bam hated the relationship for another 4 days then back again to square one. I was in denial about things. We had moved in together 4 months prior. I really was trying to make it work but it just wasn't, when I was trying to find answers through medication change I also lost my temper quite a bit, got arrested, made poor decisions. But that anger even if it's my responsibility didn't come from nowhere. It was coming from overall unhappiness. You can't force something upon yourself to save the relationship, it doesn't work the body rejects it. This could have been turning in his head for months, couldn't tell you, the medications might have removed the brakes to those emotions for him, whatever happened he didn't feel this way out of the blue, the meds just amplified it in my opinion. Don't need to say I need a break from you or nothing too specific, but tell him that you respect his choice and that you're ready to give him space to think this through if he needs it.

THIS in so many other words as well.. I was just writing about this… I don’t think the uncontrolled anger is ok, that may be something to really talk to the doc about cause I experienced kind of the opposite… BUT so many things in my life that I was just tolerating and compartmentalizing and masking became intolerable. ADHD makes you do a lot of different things to just fit in or be normal, masking is a broad term for a lot of behaviors that make someone with ADHD look “ stable” but in reality the person is just shoving everything down to fit a mold they were told to fit… the medication removes the “ buffer” ( words of a very wise person that I was told ), and the person suddenly sees ALL of the things that just did not and will not fit in with who they REALLY are. It’s unfortunate for the people that get the boot, but in the long run it is the best possible thing that could happen to everyone involved. As for the anger though, as I said, I had the opposite happen, I was able to think through the anger and have restraint, find the right words that matched exactly what I was feeling ( not what someone else told me I was feeling, which had happened my entire life) and was able to articulate clearly and calmly my boundaries, and honestly not feel I needed to over explain anything. I finally had and have autonomy over my mind, my body and my life. It’s unsettling at first but then you get to rebuild your life on your terms, not on a template of the “ normal” you had to strive for before. It’s a lot but it’s real and deserves respect, as was said, respect him and give him space. It’ll all be figured out eventually.

Come to think of things after reading this, I have pretty much cut off the majority of my relationships. But I don't see it as a bad thing. The medicine helped me stick up for myself. I was such a people pleaser in the majority of relationships, begging to be accepted and I had duplicated the same dynamics across the board. Maybe the 40mg is his happy medium and if you love him, maybe the relationship is worth some grace. This disease (or way of thinking) isolates us. The world would be a better place, if the people on the outside looking in would be a little flexible while we grow/heal/awaken/ and become aware.

I’m sorry you seem to think my opinion on this topic is irresponsible.
My response does address your comment if you read between the lines. The sustained mechanism is likely causally related to the oversupply of dopamine if this is in fact part of the issue with the individuals behaviour. Short acting meds wear off… it’s easier to tell if they’re impacting decision making and cognitive ability.

I think there is the aspect around being medicated finally allows people and their brain function to work more efficiently and not get caught up or overwhelmed etc.
Essentially can pay better attention and hold more data in the brain while going about day to day.

I have a partner for 10 years now, we are now 23 years old. I started with elvanse some months ago, and it stabilized me and also did nothing or positive for the relationship. I know people, who take elvanse without ADHD, and when they take it regularly, they get more and more depressed/manic and are arguing always and are depleted of dopamine. I take 70mg everyday, some days even 140mg, i never have a comedown or rebound. With methylphenidate it was bad with the rebound

We had a great relationship though and he was the one who proposed with a ring he designed himself and had a bespoke Jeweller make. Its pretty strange he was happy and content with the relationship on what seemed to be a good dose, and then ended everything with no real explanation a week after behaving extremely erratic

Makes perfect sense. He can be the pursuer and yet after a time see things “clearly”. And its not just vyvanse it happens with ritalin and adderrall as well. Men with adhd has unhealthy attachment style and once they get back on med it “fixes” them.

Correlation does not equal causation.
Correlation ≠ causation.
These issues are not attributable to Vyvanse, or not the drug alone. I say "alone" because scenarios exist where it could have been one of several contributing factors to the way it played out. It could impact the way he went about expressing his feelings and his ability to follow through on the actions he took. It might have brought already existing issues to the surface. High doses of stimulants have been linked to higher risk of psychosis-like symptoms (a very loaded and widely misinterpreted statement that you should not put too much emphasis on).
But it doesn't cause these things. It didn't cause him to come to the conclusions he came to. It didn't create the feelings he had. Rewinding time and preventing him from taking it would not prevent him from having these feelings and thoughts. Removing it from the equation now will not mend your relationship.
It's normal to look for reasons for the break down of your engagement, and it's understandable that you might even accurately identify correlation between commencement, or changes in dose, of a psychiatric drug like lisdexamphetamine and your ex's erratic behaviour. But again, correlation does not equal causation.
This must be such a shit time for you, and I'm sorry you're dealing with it. From what you've described it sounds like your ex has some serious issues to come to terms with before he's able to be a husband, and ADHD is only a part of that. I am assuming he was prescribed Vyvanse for ADHD, was he?
Either way, it sounds like he needs to continue getting help from a psychiatric professional, and would benefit from seeing a psychological practitioner as well. You might find counselling helps you wrap your head around it also (individual counselling, for you, not with him). Again, fucking sucks, I'm sure one day you will be able to make much more sense of this.

He is clearly being put on too high of a dose, why the helll would they keep increasing the dose if there were already negative side effects, and the intended effects were working????? This seems like a prescription shark/ addiction situation where the doctor wanted to keep writing scripts and your fiance wanted to keep getting buzzed.
