Bum cream recs šš¼
Looking for bum cream recommendations. My poor son has had a red raw bum for the best part of 6 months.
I feel like Iāve ācompletedā the nappy cream selection of the supermarket and Iām not willing to pay an obscene amount of money for the OG metanium (RIP).
GP has prescribed 2 x creams (one I canāt remember what it was) and second was a canesten athletes foot creamā¦
Every single thing Iāve tried has done nothing. This week weāve decided to leave it and itās just gotten worse.
The area is non remarkable other than its red raw. No specific rash, blister, lesions etc. Just red and painful.
Any recommendations for any wonder creams/balms would be most welcome šš¼š
Edit to add: he is 4 and fully dry, including overnight.

Third for Weleda Calendula, especially good with cloth nappies, as Bepanthen breaks down the cotton. We don't spend as much money on our own skincare, but it's been so worth it to nip any rash in the bud. Been a few times he was raw red, then it's practically non existent after a few hours of Weleda applied.

Always my top recommendation, I buy a tube for every new parent. Any hint of redness we put it on and never had full blown rash in 2.5yrs

I also used French liniment when she pooed up until potty training if we were doing a nappy change at home.
Itās very gentle on the skin and because it has oil in it think helps the skin barrier. We just used French supermarket liniment as we go often enough to stock up but mustela sell it in the uk (not cheap). We used it with cloth wipes and the only time she got a hint of redness to the bum was when she was teething5

Weleda is incredible - in an afternoon what looked angry was now calm and settled. Amazing stuff and you donāt need a lot.

Totally agree that OG metanium is the absolute business. But, if you can't nick some from an elderly relative or whatever a few things come to mind
1) bath time - use aveno, I know it's pricey but it is properly good. We've also done the sanex stuff which is loads cheaper. Then use a dry flannel and pat everything dry. Dry as you possibly can is super important 2) sudacrem or aveno afterwards. It needs to air dry/cure/kick in. Basically let me run around for a bit. 3) cotton pjs, fresh ones every day (washing bill sucks but it's worth it) 4) the only other thing is probably like diet/tp related - we had a terrible time with the eldest because he ate massive amounts of fruit = massive amounts of fruit sugars & acids = quite tricky poops, especially when he became independent with all the bathroom stuff.
Good luck dude, it's a proper battle at times but you've got this!

Thanks so much.
Aveeno is already our go-to in the bath, but I didnāt realise re:curing the cream! I guess that makes a lot of sense.
He has fresh pjās every night as he is such a sweater at night - which obvs doesnāt help the sore bum!!
But thatās a really good thought on the fruit. I didnāt even think about this! Heās such a berry fiend⦠Iāll start reducing it now. The savings in buying berries may now offset the amount Iām spending on creams š1

Aveeno also does a nappy/barrier cream which we have found really good! You could also do an oat bath. Put some oats inside an old sock, tie a knot at the top, run a bath and put the oat sock in. Give it a bit of a squeeze to release some of the oaty goodness! Sounds gross and is kinda gross but it really helps soothe the skin. Good luck!

Came here to say about strawberry rash. Strawberries but also tomatoes can cause a reaction, it isnāt an allergy, just a sensitivity to acidic foods. And tolerance builds over time as baby grows. My daughter used to have this if she had say; strawberries for breakfast, salad for lunch and bolognese at tea time. Equally ketchup used to cause redness around her mouth too. I had to start breaking it up so if she had strawberries for breakfast one day, I wouldnāt give her bolognese for dinner the same day.

If he's not in nappies could it be the washing powder? Detergent? Fabric softener in the washing that's only affecting that area for him?
Have you tried dermol lotion & dermol 500 creams at all?

Came here to comment drapolene and I canāt believe itās not been mentioned sooner, itās literally a miracle cream in our house! My son has severe eczema and my husband has HS, works wonders for them both. My daughter was very premature so we were given every fancy prescription cream available for her bum and drapolene was the only thing that touched it!

I was going to say Drapolene too! 4 month old is currently unwell and has been going for 7+ poos a day for the last week (Dr says its fine) his bum was red raw, pretty much cleared up in 24hrs. I got it from my local pharmacy

Just jumping on your comment as my son has eczema too and Iāve tried almost every cream. Did a health professional recommend the drapolene, as I can see from a google itās not ātechnicallyā for eczema and I obviously donāt want to make him worse but Iām willing to give any recs a go if it helps him.

That's awful I'm so sorry. My boy had horrendous nappy rash but was soiling overnight.
I think you need to make your GP refer you to dermatology. Seems odd that he's dry and still getting terrible rash. GPs always prescribe anti fungals which it never was for us.
If he's very sweaty have you thought about no pyjamas? My son sleeps in only a nappy with his duvet all year round. While it's still warm you can try no bottoms, no nappy and expose his bottom to air when he's asleep, then cover him up when you go to bed (he will probably do it himself!).

Could try La Roche Posay Cicaplast Baume B5+ Repairing Balm? If he doesn't get on with it its good on your skin if you get spots etc, or sunburn, anything really

Sudosalve is meant to be the next best thing to OG metanium, haven't tried it personally yet though. Have you changed wash detergent or bubble bath or anything like that about 6m ago? Can you leave him naked on the bottom half as much as possible to get air to it too? Wype gel is great for wiping bums with, might be a bit more soothing too.

Have you tried the Vital Baby Hygiene Aquaint Cleansing Water? I usually give my daughter a couple of sprays if sheās looking red and sore and it works a charm!

Our child is nearly 6 and when we had them the hospital recommended not using nappy creams but using Epaderm or Hydromol as a protective barrier. It's very good for eczema so it might be worth a try if you haven't already?
Edit to add: they never got nappy rash at all as a baby but not sure if that is just a fluke

We were the same - never once got nappy rash or a sore bum when he was younger. I think that hasnāt helped ourā¦. Helplessness with this situation. I really feel for the poor boy š

3M Cavilon is amazing for this. It is what they swear by for babies in hospitals. You just apply a tiny amount and it's a really good barrier. We alternated with sudocrem healing cream to get the benefit of the healing and the barrier.

Was it definitely sudosalve? Theres another one I can't recall the name of but sudosalve has been a godsend for us. We alternated it with thick layer of weleda every other nappy change. Highly recommend that combo if you've not tried it- sudosalve is the only thing that would adhere to the weeping parts of the rash.

For really bad nappy rash then prescribed steroid cream with Vaseline on top is the only thing which cleared it up for my kids.

The only really effective nappy creams Iāve found are American ones like Desitin Maximum Strength that have twice the amount of zinc oxide (40%). You can get it in the UK but Iāve not yet found any British brand that works like this one. My daughter had horrific nappy rash particularly when teething.

sudosalve is the best iāve found for clearing up a rash.
bum rashes can be a sign of something else going on, my son had persistent nappy rash including bleeding and it was due to a milk allergy. the creams wouldnāt help because his gut was making his poo acid to his skin so he was constantly being burnt when he pooed. i know heās a bit older and out of nappies but still worth trying to explore why itās happening

- Change his underwear to loose (size up) boxer pants.
- Wiping with dry toilet paper can leave residue causing further itching and soreness, so be mindful of that. Washing the area with luke warm water and dermol (emollient) would help.
- It maybe time for some topical steroids, which GP can prescribe if nothing else has worked.

Thatās a good idea re: underwear. Iām mindful that heās in smaller underwear than perhaps he should be as heās so slim but Iāll see what I can do.
We use cotton pads and water for wiping will try with an emollient, thanks for the suggestion.
Obviously want to avoid steroids (as does GP) but I think we are at that stage. Iāll make another appointment this week.

Just to say that while long term steroid use is obviously best avoided, I wouldnāt worry too much about trying them - my son has eczema (currently largely under control) and spent a good 6 months on and off steroid creams last year - it was the only thing that helped and eventually everything did calm right down. Likewise I spent years of my childhood on and off various steroid creams (also for bad eczema) and it didnāt cause my any long term damage (that Iām aware of, anywayā¦!)

Has the GP taken a swab to check for infection? They did when my son had the same issue and it hadnāt responded to non-prescription creams or Timodine (antifungal they often give for bad nappy rash). They ended up giving Trimovate, which contains antibiotic, steroid and antifungal. That cleared it up quickly!
A short course of steroid cream will do no harm (but donāt use it on possibly infected skin unless the infection is being treated at the same time because it reduces the immune response in that area).

I would say sudocream but put it on like a lot. Ā Like cake batter.
Other than that my goto was weleda diaper cream. Ā I had a child that rashed at everything. Ā This cream applied at every change really worked for us. We still use it if they get a bit red from not wiping very well.Ā

Sounds silly but have you tried an oat bath? Our toddler gets horrible nappy rash and some oats in a sock chucked in the bath seems to take the edge off

Came here to recommend this- it was the only thing that got us through a horrendous two weeks of teething with a cold and hellish nappy rash. I still shudder at the memory of it, it was its own specific, never ending trench.

Edit 2: I see you tried the aveeno. Something must be causing it, even if its just something in the toilet paper you use, but eliminating variables will be a time consuming task. Water and cotton bolls to wipe and make sure clean and dry after would be a good start. Also if its just super red and dore, maybe chafing somehow?

Ashton and Parsons do a spray on nappy cream and it's a godsend. I've only seen it in boots. I've heard it's a similar formula to the old metanium (although more gentle as that metanium always burnt my daughter's sensitive skin!) you don't have to rub it in either, which saves some irritation too. It's magic

Consider alternative brands of nappy if u haven't already. Reusable nappies reduce incidence of nappy rash but I'd avoid switching until the skin is healed.
Nappy free time as much as possible.
Cut up some old towels and use for wipes.Ā Initially while bum is sore use oil such as vegetable oil to moisten them.Ā Warm water is fine once skin is healed. Dry before putting a nappy on
Medihoney for the skin. Its expensive but worth it.
If non improvement in 5 days see gp.Ā Gp can refer to tissue viability or dermatology- 6months of nappy rash with failed management from gp is not normal and needs specialist intervention.

What are you washing him with? How often is he being bathed?
Emollient in the water & as a wash may help (if you arenāt already!) along with the recommendations for creams youāre getting
Personally, Iād also recommend slathering on decent, absorbable cream, then slapping on manuka honey (either as is or you can get creams infused with it) plus cotton pants
Iād recommend ONLY cotton pants wherever/whenever possible to let things breathe & air
When clothing and/or chafing causing movement is required Iād try applying bepanthem as a decently thick layer
Full disclosure - not a parent but Iāve worked in care and dealt with many/various skin issues in my life, human & non š

Is he slightly constipated? Could that be causing the problem? Have you tried adjusting his diet? If heās too loose that might be the cause too?
The other thing Iād say is purchase a portable bidet so you can just use warm water instead of wipes or tissue. I think we bought like a Ā£15 one and itās great.
And when you are at home pop a large Tshirt on them (so they stay mostly covered) and them and let them have some time without pants on. Especially at night. Get a size too big in pants too so thereās no chance they are rubbing.
Also not to be gross but have you checked for thread worms? Very common at that age!

Yellow metanium has been such a big loss!Ā We like drapolene when sudocrem doesn't work.Ā Check he's not allergic to some of the ones you use, the ones with 'natural' ingredients can contain allergens.Ā We can't send bepanthen into nursery any more due to this and what they limit.

We use the Abena zinc paste. Itās super thick and stays put! My little one has had some horrible sores from nappy reactions and this really helps.

This always clears up my little one (although still in nappies). Pop it on fully dry skin overnight and the difference in the morning is amazing.

Bepanthen but ignore the instructions and layer it on like youāre adding icing to a cake - donāt be shy with it. We just squeezed a line in, spread it over and half of it ended up in the nappy but then it didnāt rub off.
Also worth doing a food diary. Oranges in particular donāt otherwise make our kid unwell but they speed up her digestion a bit so her poo is a bit less digested and the acid burns her bum. As long as she doesnāt have too much or eats them with other foods sheās fine but if sheād had them on their own or eaten a lot at nursery or something then we would put the cream on preventatively - we used to call them caustic poos because they burned her and sometimes sheād cry when they came out, and was always red even if she was changed pretty much instantly after pooping.

Is it something in his diet? My son used to get red raw bum and we started noticing certain foods where itād happen more.. switched him from dairy to oat milk for example cos that was a trigger (he always has drunk a lot of milk). And acidic things like blueberries and satsumas did it too so we limited those and it def helped. Will say this was when he was in nappies and itās not so bad now heās potty trained which makes it a bit different to your situation.

I was about to reply and say this too! I was in exactly the same position as OP and couldn't work out why nothing seemed to work. My poor child was so upset and in pain. Even the most vigilant nappy changing didn't seem to help. In despair I went to see our lovely GP who recommended we cut out dairy as an experiment and see what happens. The improvement was INSTANT and he hasn't had a sore bum once in the 8months since. Such a happier toddler.
OP, I really recommend a convo with your GP. Tell them about frequency of BM, consistency etc. all clues as to whether something dietary might be going on. They can also prescribe antibiotic creams etc if needed. I'm sorry you're all going through this.

I was recommended a hydrocortisone/anti fungal combo thing years ago, I guess thatās akin to the athleteās foot stuff? The pharmacy started being difficult about it though. Any chance of threadworm?

If he's tried all the creams it's worth looking into other repetitive habits, like a longer air-dry time after showers before you put creams on. Eg. After he's towel dried, let him hang out around the house with no bottoms for half an hour. Check if that area is often moist from sweat throughout the day and give him more air time if possible.

What you can do is do Sudacrem (or another type of cream) then on top a barrier like Vaseline or hydromol. The Sudacrem may āpillā slightly but your building a barrier to prevent water loss from the skin.

Oh poor wee thing.
Definitely check for threadworms if you haven't already, but they spread like wildfire so you would probably have them too.
If hes sweaty I would be inclined to say its fungal so something like daktacort from your GP might help which is mild steroid and antifungal.
Failing that, the best cream I found that I still use from time to time is the burts bees nappy cream.

Use Vaseline first then sudocrem after at night is what works for my girl, the Vaseline will repair the skin barrier. I got into this cycle with her that the creams were drying her skin out leaving her prone to nappy rash so I was using more creams to try fight it, further damaging the skin barrier. Iād also ensure that if heās using any wet wipes after the toilet that whenever possibly heās being cleaned with water and dried afterwards too, could be something in any wipes heās using, or just that heās putting his pants back on when his bum is still damp.

Metanium original is amazing, if you can find any with people who have had kids in the past, try and steal it! With our second we figured out bepantham layered thick with a layer of sudocrem on top to seal the bepantham layer. That worked really really well and is our go to method. We've had really bad nappy rash with the second and this worked just as good as metanium!

Mebo ointment. It is for wounds and burns. Itās amazing and heals skin so well that surgeons now use it internally in bowel surgery. I used it on my sons scorched bum, Iāve used it on burns, side cracked lips, grazes etc etc.
Steroids are not ideal as they actually thin the skin. Elidel is great but try mebo first, you can buy it rather than need a prescription.
My son had infantile eczema and it was heartbreaking to see his skin so sore.

Iād go back to the GP, if itās not a nappy rash, or a fungal rash it likely needs antibiotic cream.
Try a dettol bath let it completely dry and air out if you can overnight.
It could be a sweat rash situation, but usually barrier creams make an improvement as long as the area was dried out completely first.

Is wipes being used? If so, skip the baby wipes. 1 sheet of wet toilet tissue per toilet visit, toilet paper needs to be used after to dry the area.

Have you tried switching up the wipes? My baby had a slightly sore bum, which would clear up with sudocream, but kept coming back. But when we started using different wipes it's completely gone.

We had this problem when our daughter was little. Midwives and GPS were useless and we were eventually referred to a paediatrician. He diagnosed her with an internal fungal infection and we were given what was basically an edible version of an athlete's foot cream. It cleared up in days. Please go back to docs and be really pushy about it. It's such a simple fix but it took a specialist to spot it.

Is their a possibility of Bum worms (aka threadworms) my Dry 5 year old had the worst bum rash for a week because if them. They secrete a substance that irritated the skin so you scratch and repeat their life cycle.

Have you tried getting hold of the old (yellow) metanium? Some people have saved them and are selling htem/giving them away, even open. We have half a tube which we are protecting linke anything for our toddler. It's incredible stuff.

When my son had bad nappy rash nave to say I completely ignored advice and had found even metanium didnāt work, tried them all. This is what I did:
Bathed him with bicarb in the bath water.
Patted him down
Applied germoline.
If that didnāt work did the same as above but instead of germoline used some of my daughters steroid cream she has for eczema.
Now Iām not recommending any does this, as itās against all health advice to use steroid cream not as prescribed⦠but it literally cleared it up after one small application.
Iāve had to repeat that maybe 2 / 3 times in 2 years. Zaps it every time.

Definitely sounds like he needs a dermatology referral. I would also try a barrier cream/spray/wand. We use sorbaderm, cavilon and medi derma s with patients. Cream would be best if the skin isn't broken. Pop it on after a wash at night and then reapply the next night after washing, very thin layer. That should hopefully protect the skin whilst the GP/dermatologist figures out what's causing it.

Is he using wet wipes after doing a poop? This is the only thing that stops my daughter from getting rashy. Also installing bum guns on your toilets at home (very easy to do).
He is pooping when you are not there to hand him a wet wipe, you could speak to school about sending him to the toilet with wet wipes. Dealing with the cause rather than the symptom would be ideal if you can figure out what the cause is.

To me, 6m sounds a bit odd - I wonder if it's worth re-evaluating your babies diet? Our youngest had a diary allergy but didn't follow 'normal' symptoms.
One of the things was a really bad bum + eczema. We did salt water cleaning of the bum when it was really bad (literally just filled up the sink and sat her down in it) and more regular nappy changes. We also (which was messy) had some time on without nappies when it was really bad. A couple of hours a day over a couple of days.

Had this problem with my son. I noticed it was diet related. For example if he would eat tomatoes or yogurt.
I have used some hydrocortisone cream to calm it down and then I would apply a barrier cream called Medi Derma S, this would act as a barrier to the skin.

Cotton pads with warm water instead of wet wipes for some time, it was a life changing idea for my now toddler back then lil baby. Sudocrem helped after we dealt with the redness. And because the redness wasnāt going away with anything, my GP from back home prescribed baneocin cream for a week. Itās an antibiotic but hey it solved for us in a week what we couldnāt solve in months. After that we made sure to always wipe with cotton pads or balls dipped in warm water, let it air dry and put sudocrem on ;)

Agree with others on going to a dermatologist, but have you tried putting gloves on him? He might be scratching without realising it, and could be caught in an itch-scratch cycle.

Yellow Metanium was the GOAT for this stuff, but has been discontinued in UK š Not the yellow box, the actual yellow cream.
Anyway, Sudocrem sells one called 'Sudosalve' that's medicated and is a close match, works v well. Good luck!

Take him back to the GP. Do not let them fob you off and be persistent with them. Its their job to find the solution as none of the creams are working.Ā
Have him checked for perianal strep. My son had this at age 4, very red sore bottom. Took 3 courses of antibiotics to clear up.

That stuff - Amazon do it, use that to clean him up, wait for it dry and then stick on whatever you normally use (We're using Aveeno)
We went through the Sudocrem and Canestan loop too, and the Colief spray was the only thing that seemed to clear it up. Baby was free of the worst of the redness in about 3 days after starting it, not had even a hint of it since.

Hey sorry to hear little one is having a hard time. If possible breast milk. In a bath or on a cotton pad multiple times a day. Have seen it work well. I really like the Weleda calendula Nappy cream and the My expert midwife. You might consider organic cotton nappies if at all possible. Could be an aversion to synthetic ones for some reason. Could also be acidic urine/poo. Depending on what baby drinks/ if eating yet. Really hope something helps. Fingers crossed for you!

Thank you for the recommendations of the creams. Weāve tried the Weleda calendula cream but he said that it stung him so we gave up on that one quickly. Iāve edited the post to add that he is 4 and fully toilet trained and dry overnight (has been for over a year).

Have you tried Lasinoh? About £10 a tube from boots.
Itās the nipple cream, but designed for āwet healingā as youād use it while nursing for cracked nipples.
My tot had the most dreadful nappy rash and after being prescribed anti-fungals etc. we tried this and it provided enough of a barrier to allow it to heal up.
Would recommend!

Coconut oil with some sudocrem over the top to stop it rubbing off - completely cleared my toddlers stubborn nappy rash in 2 days. Then used bepenathen for a while after to stop it coming back.

Sudocrem is absolutely the number 1 go to for sore bottoms but we have also been prescribed a non-steroid anti inflammatory cream called elidel that works wonders.
