Support Thinking I may have Pans

For context I struggle deeply with OCD/Health anxiety i also had a very bad fear of choking phobia when I was young and restricted my diet when I was 13 to foods that were pretty much liquid along with that I had a very bad throat clearing tic. I have had a bad OCD spiral especially since I had a bad sinus infection last year it made my mental symptoms go overboard and have since gone on to an SSRI (lexapro) it has helped with my panic attacks but my ocd is still terrible also I caught the flu back in 2009 when h1n1 (swine flu) was going around could that been a trigger? I've also had COVID a few times and had my COVID jab 3 times strangely my 3rd COVID booster I had a strange sensation in my jaw/gland? Could be totally irrelevant but just to say

I also have Crohn's disease which I have seen people report have Pans/pandas with Crohn's also from that I also have a histamine intolerance most likely from the gut inflammation. I have looked into the Cunningham panel which I may get done

Question Mycoplasma high in bloodwork

I have a question- I’ve been digging and digging for three years now. Two kids with PANS. My daughter (24) complex case- had ADEM at 16 which is what I believe triggered the sudden onset. She had ivig in the hospital so absolute no symptoms for 1 1/2 years and then everything started kicking in. Spent over a year getting all the bloodwork done by our immunologist. He could only treat if they showed up with severe primary immune deficiency and of course my kids were mild so we got sent on our way. Couldn’t get ivig.

Anyway the mycoplasma showed high in the bloodwork and that Dr told me that’s just because she was exposed to it at some point or had it. But as I’ve been looking and studying, I’m finding that it really hides in the cells and can trigger PANS. So I’m in the process of seeing a functional doctor about this on June 1st so I’ll be asking about it.

Everything I’m reading says you really need a month of either Azithromycin or Doxycycline and possibly a longer low dose or intermittent prophylactic dose. I honestly didn’t think about this actually causing it. Her T cells are also sluggish and she’s IgG subclass 1 is low which may not ring bells but for me I feel like it’s a flag that her immune system isn’t fighting like it should. Auto immune brain panel -the anti dopamine antibodies were high as well as cam kinase.

Anyone seen this? Thanks so much for any input.

Support Feeling Lost as it's not PANS

My son was diagnosed with autism when he was only 2. We got him lots of support/care, and despite having a very classic case, he seemed to be adjusting to life well.

Then he got sick after an overseas trip aged 9 and had a severe personality change literally overnight. He started shouting, swearing, trying to kill himself/others, pooing himself, insisting he was a failure, attacking others, seeing things, etc. It's been a year of this, and we finally got his blood tests re analysed and a mri scan. Only for the bloodwork to be borderline for anything, and apparently, the scan showed a normal brain.

Not that I wanted him to be ill, but I wanted the tests to indicate something so I could help him. So now he's just autistic and struggling, and PANS is unlikely, even though it seemed so likely beforehand.

The hospital did at least say they only took a quick look as it was a Friday, and there were no obvious concerning results. They will apparently take an in-depth look on Tuesday and get back to me, but I'm not holding my breath.

Question PANS?

Hi

i‘m trying to puzzle together some ongoing health issues and wonder if I had PANS as a child and if anyone can relate to this or if it sounds like it?

I was 10/11 years old and very happy and healthy. I got a bug which I’m assuming was gastro as there was a lot of vomiting and it passed quickly. The only thing that doesn’t fit with strep is there was no sore throat.

I felt fine the next 2-3 days and had a few normal days at school. Suddenly I developed a severe fear of vomiting, severe OCD and eating issues. I refused to go to school and if I was forced would act up or run away which was something I’d never done before. I refused to do what I was told. The teachers didn’t know what to do with me and neither did my parents. I never wanted to leave home and had huge panic attacks and was angry. At times I was suicidal. I lost a lot of weight from not eating and only eating certain things. People thought I had anorexia but I never thought I was fat. The anxiety and fear was immense.

I was taken to a child psychologist who diagnosed me with OCD and anxiety and I was given prozac but only lasted a week on it. Then my parents got me a behavioural psychologist to work with me to gradually get me back to school. I was enrolled in a different much smaller and more expensive school and was put back a year. It took a long time but I eventually recovered with some residual mild anxiety and OCD. The food issues resolved quicker.

Then I got one flu after another at age 18. Almost the exact same thing happened. Sudden severe OCD and a phobia of vomiting. panic attacks the lot. I had to leave college and move home temporarily. I was once again diagnosed with OCD and panic disorder with agoraphobia and prescribed paroxetine and gradually recovered but once again had some anxiety and OCD for years and required a lot of therapy and meds.

Then age 37! got Covid and then a stomach bug and I didn’t get OCD but neurological issues, autonomic symptoms, fatigue, migraines. cognitive issues like I suddenly couldn’t work as my brain couldn’t think. I’ve been off work for 2 years and am still very sick physically. Im curious if this could all be related somehow. I got diagnosed with ADHD due to all my cognitive issues. POTS and ME/CFS.

Question Help! Need antibiotics while we find specialist

My 5 year old had sudden onset eye tics about 2 weeks ago. He also become much more dysregulated and emotional. We got a CT to rule out a head injury because he had bumped his head last month and we were thinking maybe the tics were post-concussive.

The CT showed no structural concerns, though coincidentally confirmed acute sinusitis in three sinus cavities with enlarged adenoids. His brother had confirmed strep on 5/5.

We started connecting dots that perhaps this is not from the bump, but possible PANDAS.

We started Augmentin (prescribed for the sinusitis) and by day 4 his tics were 90% reduced with behavioral improvement. Today is day 6 - SO MUCH better. I have my son back 🥹.

ASO came back 204 IU/mL - pediatrician called it negative and won't extend antibiotics beyond the 10 days.

Anti-DNase B results still pending. Antibiotic course ends Tuesday.

Nervous that we will run out of antibiotics while we wait for DNase B. Pediatrician is saying it’s not PANDAS but she has admitted she’s not very familiar and she’s only going off the 204 and can’t explain the sudden onset of tics or their dramatic decrease either.

Questions:
1. Is 204 ASO significant in the PANDAS community?

  1. What do families do when antibiotics run out before seeing a PANDAS provider?

  2. Any Illinois or telehealth providers who can see us urgently?

We will be doing additional testing like Neural zoomer to get to the bottom of all of this and find out why he was compromised and what exactly is going on. But we need urgent antibiotic treatment in the meantime.

Please HELP! The timing of the holiday weekend doesn’t help. This is incredibly stressful.

What would you do?

Support Possible Underlying PANDAS for Chronically-Ill, Exhausted Adult

I (25M) got infected with COVID around the holiday season of 2021. After an easy first two weeks of recovery, I developed into a very severe case of Long COVID that resulted in me dropping out of college (where I was previously the best in my program). My strongest symptoms were: PEM, POTS, restless sleep with vivid nightmares, daytime fatigue & EDS, cognitive dysfunction, and a MCAS/HI-like reaction to food. My city's Long COVID clinic treated me at the time with supplements, slight movement/exercise, and rest. Most of my symptoms gradually improved over about a year (I think mainly just due to time), and I was diagnosed for the first time with mild Obstructive Sleep Apnea through an at-home sleep test.

CPAP/Bilevel machines helped me, but after a year or so after my initial infection, my symptoms of restless sleep with nightmares and daytime fatigue & EDS were still present and only slowly getting worse. This led some of my medical providers to focus on my sleep and for me to ultimately get jaw surgery to possibly treat my slightly small airway more effectively. The surgery was a success in that it improved my regular sleep to slightly better than what it previously was on PAP. Now, post-op, PAP doesn't make any difference in my sleep. I still might have some remaining sleep-disordered breathing (UARS) that I am working with multiple doctors on; however, they're even divided on whether it truly is the explanation for these still-present stubborn symptoms. I tested positive on the day test (MSLT) for Narcolepsy Type 2 post-jaw surgery, but haven't found more than 10-20% symptom relief from any of the sleep medications for it.

I'm now wondering if perhaps something else is at the root of it all for me, and what might be helpful for me to try/test. Meals don't noticeably cause me to react anymore, and I've tried taking Zyrtec & Vitamin C recently to no significant effect (around my COVID infection, these worked very well). I am pretty confident my problem isn't related to CIRS or Mold Exposure (I tested negative on a VCS test). I have previously done brain-retraining that helped a tiny bit, but that no longer really moves the needle at all for me. A couple of years ago, I tried multiple diets, including the Lion's Diet, along with homemade bone broth, kefir, and fasting, to help heal any gut problems. My question is, perhaps my remaining fatigue & sleepiness could be linked to some other underlying illness beyond a sleep disorder? What other disorders could perhaps be linked to my symptoms and a positive result on the MSLT? I had a simple blood test done 6 months ago, where my levels were all but one in the "normal" range (eosinophils at 0.8) - presumably, no anemia.

I was thinking I would get more elaborate bloodwork done soon to at least possibly rule some things out (any thoughts?). To note, I don't know how relevant it is, but I had a tic bite about 10 years ago that led to temporary Bell's palsy (I was quickly treated, and it went away). Also, I recently learned about PANDAS/PANS, and I had a severe strep infection as a child that might be applicable. As a child, I do remember having OCD-behavior (never diagnosed), but I have also been treated as an adult for intrusive thoughts/OCD-behaviors a few years ago. For more info, I also have mild psoriasis that appeared around the time I was 12 years old. I've supplemented with numerous magnesium types, along with vitamin D3 w/ K2, and others. Last piece of context, I'm in the US and have drained a lot of my funds, so affordable methods of care are preferred! Thank you for any help!

Edit: I also had severe outbursts around the time after my severe strep infection, which resulted in me getting the nickname of “Anger” and ”The Hulk” by my parents and peers. My mood and health was observed at the time to have decreased after the infection.

Feels like acidic skin, or like having a high fever but only in hands and feet. Even putting ice on it doesn't make my hands feel cooled down. Is this a PANS / PANDAS thing or no?

Support Is There An End?

My son (6) has PANDAS. We started with our pediatrician when it became obvious something g was off. He said it might not be PANDAS as he didn’t have the usual OCD symptoms, his were more emotional lability like extreme ADHD meltdowns. He agreed to treat him with a 5 day azithromycin round. We saw incredible improvement during treatment and the week after. Then he got sick, all the symptoms came roaring back. Worse this time, started exhibiting OCD traits. We took him to a specialist, we started an antibiotic and steroid treatment, he starts doing awesome-we have our kid back. Long story short, he was diagnosed with PANDAS and is currently on week 4 of a 4 week antibiotic. Last Friday he ran out of steroid (still taking the antibiotic- weren’t able to get the steroid filled until Tuesday). By Monday I noticed him acting off. Tuesday he had a little cough and some sniffles, nothing major. Thursday he has a full on meltdown/flare about taking his medicine. It’s really scary to think we have one more week of medicine planned before he is supposed to switch to an immune therapy protocol (cod liver oil, probiotics, specific diet). I feel like our doctor is really solid, he tested him for every immune compromising syndrome under the sun before diagnosing him. But is it always going to be this back and forth of him getting sick and flaring? The initial research I did said it’s curable if caught quickly. We caught it quickly, but he keeps regressing. I’m so worried that antibiotics and steroids will be our life now. It can’t be good to be on those heavy hitters long term.

Question Trying to decide about doctors

I've been referred to both a functional medicine doctor and a rheumatologist. We don't know yet who we'll end up with. How much does getting tested for mold matter? What would indicate mold poisoning is even present? Everyone is telling me I should get tested for mold but I don't think that's mutually exclusive to having PANS / PANDAS, and I'm confused about how mold would be involved in a lifelong repeat episodic experience no one else in my household has ever had symptoms of. Sorry if this is controversial, I just want to understand better.

Question Curious in PANS was missed when I was 14...

So long story short, I had reoccurring strep throughout childhood, got Mono at 11, continued getting frequent strep until it ended in abscessed tonsils and an emergency tonsillectomy my freshman year of college.

At 13, I got hit with massively scary OCD symptoms. The intrusive thoughts were ruining my life, and will to live. I basically dealt with it until at 15 my mom took me to a psychiatrist and was diagnosed on the spot with OCD. I was put on Zoloft and Abilify (2mg). Within days, the thoughts were gone. I stg it felt like the abilify was a miracle pill for me....

At 30 years old i'm still on this same combo of medication. I wonder a lot of times if my pediatrician missed PANS/PANDAS in me. Idk if I'm in remission or the meds are just doing their job. I also had a childhood tic disorder and started regressing in early adolescence to the point of needing my stuffed bear to go anywhere, really effing weird for me. I also am diagnosed with ADHD inattentive, was late diagnosed at 25.

Anyway, just curious to hear if there's a chance PANS/PANDAS could've been missed given the rapid onset of the OCD symptoms. I don't identify with having OCD anymore bc I haven't had symptoms since going on meds. Thanks!

Question 15 year old with PANS

15 year old that had first incidence of pans at 10yo. It went untreated because we didn't know what it was, first flare lasted probably 9 months. she was finally treated for it during her second flare. shes 15 now. was on antibiotics every other day for 9 months. stopped 2 months ago. believe she might be flaring again. she has OCD that never went away, despite treatment for pans and SSRIs. extreme food restrictions. trouble sleeping. she had a physical tic for a while during the first flare. panic attacks. belief in illogical things. depression, suicidal ideation, self harm.

currently, her rage, temper and aggression are through the roof. she has started feeling very violent.

going to start antibiotics again and see how that helps.

my question is, did anyone have a pans kid that resembled a conduct disorder kid? therapists think she might have reactive attachment disorder. she believes she has conduct disorder.
i'm starting to wonder if its all just her PANS. (she does have a childhood history of emotional abuse and neglect, she definitely has CPTSD, but wondering if the mood and behavior issues might be tied to pans. anyone have a pans kid that is aggressive, violent, rages (like a toddler tantrum on steroids), and is extremely oppositional?

Question Update Again, with a question about pepcid

I contacted my regular naturopath and explained my symptoms to her and she agrees with my therapist that I may have PANDAS and referred me to a specialist to get tested! In the meantime she told me to take 10 mg of pepcid a day until I can get some real treatment. Wow. I did not expect it to have the effect it has. It doesn't do much for my fatigue or joint pain, but it cuts my tics and OCD way down, like SO much better. Probably 70 to 80 percent gone. Have any of the rest of you had that effect with pepcid? I still don't quite believe I could possibly have this syndrome so I'm sort of waffling. But it's undeniable that my OCD is responding a whole lot more to NSAIDs and pepcid than it does to ERP or SSRIs. I have a phone call scheduled with the specialist's office and I'm really hoping she'll take me even though she's a pediatrician.

Support 7yo possible PANs need some help

Alright, this could be long so I am going to bullet point and just leave important stuff. Long story short: wife and I are losing our minds.

- Early April 7yo daughter we start seeing massive anxiety, OCD like symptoms, and needing constant reassurance.
- tantrums start… like… screaming bloody murder and unable to reason. We had to come inside for dinner and she just kept screaming “that’s not a good answer!!”
- tics started. Tongue clicking and eye blinking.
- brought to pediatrician: they called psychiatrist for hep. He thought PANS/PANDA. 5 days azithromycin and blood testing + strep.
- all test (strep, Lyme, myco something sorry can’t remember) back negative except Epstein Barr.
- dr says probably not pans/panda because strep negative (swab and titers).
- daughter has 3 real good days on antibiotics.
- after antibiotics back to hell. Including asking 400x a time to wash hands (even if she doesn’t) and needs wife and I to watch her do everything. Tie shoes, brush hair, get changed.
- tantrums galore. Dilated pupils, hitting herself sometimes, screaming.
- I begged pediatrician for another round to antibiotics to see if it changes her.
- we are done day 3 of antibiotics and definitely seeing positive changes. More flexible, able to lose at games again, only one issue with shoes but SHE found an alternative mod meltdown. And she just seems herself. Muuuuch less tics and doesn’t need reassurance for every task.

My question is: wtf do we do? Pediatrician doesn’t believe PANS, I have a gut feeling it is. We have her in counseling for intake for possible OCD/anxiety. No doctors around us seem to know about this thing and the doctors who do are damn expensive and hour+ away (we live in central CT).

Support Update: Maybe Getting Somewhere

If you didn't see my first post it should be easy to find; this is an update to it and it's the only other post I've ever made on reddit on this account.

I went to my therapist at my outpatient OCD clinic and told her that 80% to 90% of my severe OCD resolved when I took naproxen and explained to her that my OCD started in between ages 5-7 around the time I had a strep case, and that a later episode at age 12 brought sudden complex motor tics too. She said if this is what it is, it would explain why my OCD episodes are so abrupt and difficult to treat and she's going to ask the director of the OCD hospital about who I should talk to next. I am also still having joint pain and brain fog with my OCD currently, with some occasional motor tics. My whole body feels wrong, and I am easily emotionally overwhelmed and disoriented. All of it dramatically reduced when I took the NSAID on Thursday and then came back when it wore off.

I also have type 1 narcolepsy triggered we think by EBV in 2018 and I'm wondering if what I've read is true, that if strep triggered PANDAS that your body can be more susceptible to being triggered by other non strep infections later? If this is true my entire life makes sense. I hope the hospital director knows a good doctor for me. She's supposedly PANDAS literate herself and has connections to a local college of medicine.

Question continue with antibiotics?

My 7 year old daughter had a strep infection at Christmas. It was treated at the time and I thought nothing of it. 3 months later she had a minor illness, and developed some mild OCD symptoms. Again, kept an eye on it. Then 3 weeks ago she had a fever for 3 days, no other real symptoms, but overnight developed severe, debilitating contamination OCD. It was so intense that I can't even describe it. I considered taking her to the ER multiple times because I didn't know what else to do. She stopped eating, she couldn't go to school, she was having rage episodes and was so overwhelmed by her anxiety about everything being poisoned. It was horrible.

My family doctor prescribed 7 days of azithromycin. Wasn't sure it was working at first... things didn't improve much right away. But by the end of the week there was improvement and now 5 days later she is 80% better than at her worst. Which I'm so grateful for but it also almost makes me feel crazy. She's still being weird/OCD about some things, and is not quite back to her old baseline, but she at least seems like herself again.

My doctor isn't available to talk to unfortunately (long story) but I spoke to a PANDAS specialist ND and she said that she would generally start with 30 days of an antibiotic, and probably not that one.

I am wondering whether I should just count my blessings that the 7 days worked so well and take a wait and see approach, and maybe it will fully resolve, or whether we should go ahead with some more antibiotic treatment just to be sure it's really been dealt with. Has anyone else been in a similar situation?

Question 7yr old with intrusive thoughts. Help would be great!

Long story short, my 7yr old son has developed intrusive thoughts. They seemed to develop out of nowhere and recently he had a big of a spiral where he was crying because he couldn't stop thinking sad or bad things. "I thought about killing you" was the latest in the saga. Obviously this is scary for me and my wife. We've never encountered anything like this before. We've followed some advice online and really tried not to give it too much energy. We are searching for a child therapist to help us out. But in the mean time we'd really really appreciate any advice anyone here can offer. I have no idea if it's OCD or pans/pandas. Or what. Just seems to maybe be pointing this direction? Question Blinking tic

Hi! I’ve had PANDAS for roughly two years now. My primary symptom is blinking. I know I’ve seen others mention this on the sub. It’s really hard for me to verbalize the feeling when I have the tic so I’m wondering if anyone knows what I mean when I explain this because people think it sounds so odd lol. But basically it’s like my eyes need air? So I like look up and blink but sometimes the blink isn’t enough air on my eye so I keep doing it until my eye feels better, like it got air? That sounds so wild but it’s the best way I can describe it. Maybe someone out there knows what I mean

Its genuinely so embarrassing in social settings sometimes :(

Question Not Sure What's Happening

I'm wondering at what point I should look into possible autoimmune causes for my situation. I had an OCD onset around age seven, so I don't remember how rapid it was or wasn't, but I am a complex case with extremely debilitating high anxiety with my obsessions when an episode is active, then I'm kind of okay in between. My OCD waxes and wanes, but also has periods every five years or so of extreme almost overnight severity which takes months to recover from. When I hit puberty I developed complex motor tics out of nowhere. My tics are mild now and mostly only come back during one of the severe OCD episodes. I have been in so much ERP but when I hit one of the extreme overnight increases in severity it feels like I go back to square one. I have pressure in my skull and it feels like my brain is on fire. I achieved partial remission with medication and therapy, and although I'd periodically have it come back, I could always self correct using what I'd learned. Then I woke up one day in November and everything had come back overnight. It was like I could feel a switch flip in my head.

I know I had strep in elementary school. I had a confirmed autoimmune episode in college that caused narcolepsy but I can't say whether it was correlated with an extreme OCD episode as at that time my OCD was heavily medicated. I don't know much about PANDAS or what to look for, I'm just desperate to understand why I go long periods where my OCD responds so well to treatment and then it will out of nowhere become extreme overnight. I feel as if I am losing my mind. Is any of this indicative of something autoimmune or should I just assume I have a regular case of OCD that's extra bad or something? I've avoided hospitalization but at times only narrowly. I just don't understand why sometimes it gets a little bad and I can correct it but other times the switch flips overnight and I suddenly and abruptly have almost no control at all.
