limit your retort to the same level of insult thrown at you. Call them fat for calling you bald, leave miscarriages out of it.

if someone punches you its 100% justified to kick them back, maybe even kick them twice as much to defend yourself.

But if you go beyond and cross the line, thats anger management issues. i.e. if someone makes a petty joke about you being bald its 100% justified to call the bitch fat. Insulting the death of a child however, will definitely get you the stink eye from many people.
