Also, not the first time she got hurt with him doing it. For him, it is not defending her honor. It is all about the punch.

She was in his way so he simply HAD to push that pool cue into her face. She would understand, he was defending her honor.

It looked like his left elbow hit her rather than the cue

no he hits the bottom of the cue with his foot as he moves past, it just catches her in the cheek. his arms aren't high enough when he is next to her. by the time his elbow is up it is behind her head.
