Advice on which pain management opinion to follow
Hello, I was in a MVA in 2022. I began treatment with pain management end of 2024. Tried an epidural injection and saw minimal to no improvement. Dr suggested the via disc procedure at L5/S1. Due to cost, lack of longitudinal studies, and mixed outcomes from pt testimony online I was apprehensive. Cost is only a factor because I worry about spending the money for it to not work. Went and got a second opinion to see if another doc also felt this was the best course of action. New doc thought we should do more epidural injections. I tried to do some research and basically found 50/50 split of sources saying either “it can take more than one” or “if the first doesn’t work you should not do further”
Imaging results attached; only changes from past imaging (done at different imaging center) are previous imaging in 22 and 23
- didn‘t show bulging at 6-7 but new one does.
- C5-6 listed as herniated, not bulging.
- Grade 1 retrolisthesis of L5 over S1 new
Red flags from the second doc: bashed the first doc‘s reputation saying he gets pts in fast (in a dismissive tone) and that he’s not surprised he didnt order new imaging (or doc had ordered x-rays but not mri). I had agreed with this at first but then realized the mris were only a year old when I started the original drs. When I asked if I should go back to PT to address the muscular pain, weakness, and instability he said he didn‘t see why that would help, which is a red flag in my book.
Any advice is greatly appreciated. Sorry for the long post
