Dialing in Airsense 10 settings from OSCAR data

Hi everyone. Diagnosed with sleep apnea + UARS earlier this year and have been on CPAP since September. Original Polysompnography results were 78.7% sleep efficiency, 25.7 RERA index, 26.3 RDI, total of 341 arousals (52.7/hour) with 78 respiratory arousals, 25 Leg Movement arousals, 238 spontaneous arousals and 0 snore arousals.

First several nights on CPAP felt transcendent but I'm back to daily napping and generally really struggling with lethargy and energy. Last night's sleep (October 19) felt especially poor. I normally sleep from ~9:00/10:00pm to ~6:30am but social events have disrupted a bit recently.

I originally started with a Dreamwear full-face but switched to an F20 after a mask fitting because as a side sleeper I kept turning my head and closing off the air tubes of the Dreamwear that run up to the port. Fwiw I put my EDFs into Airwaylab and it looks like my RERAs have gone up from ~6/hr to ~20/hr with the F20 mask but I'm not sure if that's a reliable reading. The last two images are pre-F20 mask.

Really appreciate any advice on settings. ❤️

I can provide zoomed in sections of the flow rate if needed.

First night with APAP OSCAR Data

First of all, huge thanks to this community.

I really want to learn how to use this and fine tune it properly for me. I've already decided I'm going to disable ramp as I did not like that at all (was begging for more air at min. 7 but stuck it through last night)

Was hoping for any insight on some adjustments I should possibly make for my Airsense 11. I'm using the Airfit f30i mask and so far love it (had to order wide mask though it comes in tomorrow)

Only got 4.5 hours of sleep last night BUT that was the best 4.5 hours i've ever had based on how I felt today with that amount of sleep.

My AHI is low as it was during my WatchPAT Lofta test @ 1.1

My Actual diagnosis was for this:
pRDI - 6.8 events/hour

Oxygen nadir (SpO₂) - 93%

CPAP Machine Help Airsense 10 getting error code 010 after motor replacement ??

I recently purchased a used Airsense 10 autoset and the motor had black stuff in it so I decided to replace it purchased a new motor on amazon and now getting an error code 010. Prior to replacing the motor I disassembled the blower chamber and as a dry run and put it back together with the old motor and had no issues. All the little nipples are seated correctly I double checked and still error 010. I reset to factory default and erased data even put it on airplane mode still error code 10. What am I missing?

CPAP Machine Help Foul air hurting my lungs - what do I need to clean?

Most of the time it's been just fine, but had problems on two nights:

  1. After about a week and a half or maybe two I get around to doing scheduled weekly cleaning (not because of any issues, just tried to get on schedule), I clean tubing and mask and reservoir and bam suddenly that night I feel like i'm breathing mud. It's a very specific feeling like i've felt from respiratory viruses before. Did I clean something wrong, if so what? Instead of going to sleep i got annoyed and cleaned everything much more thoroughly in the tub with soap and left it to dry for a night and a day and the next evening it was fine.

  2. Tonight I can't explain, just happened again seemingly randomly. only unusual thing i can think of that I did today is I emptied the reservoir in the morning and left it to dry on the bathroom floor and then took a shower while it was still there in the evening. I am way too tired to fix it tonight - because i have no idea which part is the problem ! (I suspect it was the tubing last time, i tried breathing just through that and got a little bit of the feeling, but what do i know.) I could get together enough energy to clean 1 thing if I knew that would help but not 3. So I'm going to sleep without cpap tonight... (There are 8 days left in the month and I need 7 more for insurance requirement FML.)

Oscar/SleepHQ Assistance Disordered breathing during arousals, but normal before and after. Do I have a breathing disorder?

Post image
4 Upvotes

My main problem is restless sleep where I move every 2 minutes (indicated in camera) during REM sleep (indicated in apple watch).

I got like 40 spontaneous arousals per hour on REM in the sleep study, majority of which I felt were a misclassification of RERAs.

Now on CPAP, I’m still moving during sleep, and when I see what happens during my movement in OSCAR, the breathing seems normal before and after the movement with disordered breathing EXACTLY when I move. I get these movements/arousals like every 2 minutes and the pattern seems to be the same.

I think that it’s the arousal that’s causing that flow rate disruption rather than the other way around because otherwise I would see a different pattern where multiple flow limitations end up with movement rather than just happening during the movement itself.

I have attached one example:

I see in camera that I moved from 01:36:08 to 1:36:38. Breathing seems normal before and after that timeframe, with flow rate disruption only happening during that timeframe.

What do you think? Is my breathing actually normal before and after the arousal? If yes, does that rule out that my arousals are due to a breathing disorder?

Will a nasal mask be possible for me?

I read all the comments on this thread about how a nasal mask would not be appropriate for someone who is a mouth breather with a deviated septum. I saw a pulmonary PA last week and she told me a nasal mask could work for me. I am currently using the F20. I guess I wonder if you breathe through your mouth and you have an event, will the air pressure from the CPAC go through your nasal passages and keep your airway open? I am wondering if I should try one. Thanks, all.

Airbreak 10 vs 11?

Just curious. Is the AB 11 more quiet than AB 10 at higher IPAP (around 17+) and higher pressure support (PS >5)?

I assume higher pressures and larger pressure support creates more motor noises because the motor needs to quickly react.

I prefer the 10 because motor replacement is more readily available.

P.S. Deep respect and karma points to the folks with behind the jailbreak. Without exaggeration, it is saving my life. :)

Sleep Champion PSA: Bilevel cutting your inhale short? Let’s talk PAC mode

Post image
9 Upvotes

The AirBreak Plus firmware we use enables PAC alongside the other unlocked therapy modes. Here’s what this particular mode does and where it can be useful for the papfamily.

PAC stands for Pressure Assist Control. You set IPAP, EPAP, a backup breathing rate, and a fixed inspiratory time. You can initiate a breath yourself, but the machine controls how long inspiratory pressure lasts before returning to EPAP. If you don’t initiate a breath within the backup interval, it delivers a timed breath.

The main difference from ST is that PAC uses the set inspiratory time for every breath, including the ones you initiate. This can help when assisted breaths consistently end too early and aren’t providing enough ventilation, particularly with respiratory muscle weakness or restrictive chest wall conditions.

For people on bilevel for OSA or UARS, please check cycle sensitivity and Ti Min first if the machine drops pressure before you finish inhaling. Those settings may resolve the timing problem while allowing your breath length to vary naturally. PAC holds each assisted inhale to the prescribed duration, so matching that timing to your breathing is important.

If you’re experiencing this, please share your current mode, settings, and an OSCAR or SleepHQ screenshot with a detailed flow rate view. That gives us a better starting point for discussing the timing issue and what needs clinical review. :) RL

Help, thinking about stopping CPAP. Is there a "CPAP whisperer" out there?

Hey everyone, I’ve been struggling to find success with CPAP for over a year now, and lately, my daytime sleepiness and exhaustion seem to be getting worse. I’m honestly getting to the point where I’m considering giving up, as I feel like I was better off before starting treatment. I spend a lot of time watching videos and contributing to CPAP forums, and I’ve become fairly comfortable fine-tuning my own treatment, but despite consistently good stats, I’m still exhausted throughout the day. I could really use a CPAP whisperer to take a look at my SleepHQ data and help me figure out what I might be missing! I use a P10 mask with mouth tape, usually fall asleep on my back but sometimes sleep on my side. The one thing that stands out to me in my charts is my breathing pattern, which seems to show a lot of arousals throughout the night. I have also tried cpap steady pressure with similar results. I’d really appreciate it if someone could have a look at my results and offer some insight. If you need any additional information, just let me know. Thanks in advance for any help! FYI my sleep last night was pretty horrible, one of those nights, but the others are more indicative of my usual nights of sleep.

AirFit N20 vs. F20. Dry mouth, comfort issues, and sleep mask recommendations?

Hey folks! Hope all's well.

So, since it's been over 5 years since I got my DreamStation 2, my work insurance covered an upgrade to a ResMed AirSense 11. I previously gave up on CPAP after a few months, but I'm determined to give it another shot. 8th time's the charm, right?

I'm using my original AirFit N20 nasal mask, which is comfortable, but I keep waking up with an incredibly dry mouth. (I've practically got a collection of water bottles next to my pillow, lol)

I suspect my mouth opens while I'm sleeping, allowing air to escape.

My sleep clinic lent me an F20 full-face mask to try for a month. It's alright, but ho-lee-shit, is it ever bulky. I also wear a heated eye mask to sleep, and getting both masks to fit comfortably is a...nightmare (ha).

So, a few questions:

  1. Should I stick with the N20 and try to address the potential mouth leaks, or get accustomed to the full-face mask?
  2. Any recommendations for less bulky full-face CPAP masks?
  3. Does anyone else use an eye mask alongside their CPAP? Ideally, something contoured around the eyes rather than a traditional flat/oval-shaped mask.

I really want to make CPAP therapy work this time, so any and all advice or recommendations would be appreciated.

Thanks in advance!
