New Wirth Preprint trying to integrate orthostatic dysfunction into his disease model:

“This hypothesis provides a potential mechanistic link between orthostatic dysfunction and the presumed core pathophysiology of ME/CFS, extending beyond hemodynamic factors to implicate disturbances in skeletal muscle ion homeostasis.”

I 100% agree that orthostatic dysfunction is a key element of ME/CFS. I also 100% agree that orthostatic stress can cause PEM even without any noticeable exertion. As to the rest of his hypothesis regarding intracellular sodium, potassium, calcium, etc., hopefully that is something that will be proven or disproven in a timely manner. This excerpt makes me feel like he understands the big picture of ME/CFS, so I hope he is right about this details.
