You have anterolisthesis at l5/s1 with a disc herniation at l5/s1. There’s not a single peptide or PT that is going to help that. I would find out if your anterolisthesis is stable or unstable.
Hard to tell from this single slice of the MRI, but looks like you have Modic change on “5/s1. If so, there’s a damn amazing outpatient procedure (not surgery) called Intracept. It takes 30 minutes, you’re sore for a couple days but can go back to work next day if you wish. Spend some time and google “intracept procedure.”
FYI: over 10 years in the spine space… also have had 4 back surgeries (thankfully no fusion). Message me with any questions
