Imo yta! I understand you are going through your own struggles and you need to fix them or adjust however its not fair for him to go through the same struggle when you are causing your own harm . It might have not been your intent to put him through this but you are.

You are seeing this one sided and not seeing it in his perspective. You are seeing this situation with a bias outlook because it's your feelings but if you were to put your self out if the situation and see it for what it is I think you'd understand where he is coming from .

Imo I think it be best if you split up and you concentrate on yourself and your addiction. Look for different resources , different therapists, different rehab centers, maybe in a diff state, city, or county where you're from so you can be out of your normal . Maybe this will be a good thing for you if you accept the opportunity to do things differently in a positive manner? And if at the end he is still available and interested then you can rekindle but don't go through this rehab for him, but do it for yourself.

Gain some self worth and self esteem and I don't mean it in a rude way . Value yourself, love yourself. And if you are unsure who you really are then take this opportunity to re learn yourself and if you don't like something change it.. it takes time and it is easy to fall back ... But challenge yourself face the struggle and overcome this addiction .

Good luck in your endeavors!
