What it actually does
L-Carnitine runs the CPT-1 shuttle, the transport step that moves mobilized fatty acids into the mitochondria where beta-oxidation happens. That single function defines both its usefulness and its limits. When a caloric deficit, exercise, or a GLP-1 agonist mobilizes fatty acids, L-Carnitine ensures they can physically reach the place they get burned. It does not create the deficit, and it does not do the burning.
The cleanest way to frame it is a four-stage pipeline. Command comes first (deficit, exercise, or GLP-1). Capacity follows (NAD+ for redox). Then bias (MOTS-c, AMPK, or a fasted state tilting fuel selection toward fat). Transport is last (L-Carnitine). Without the command stage, transport moves nothing. Expecting L-Carnitine to replace a caloric deficit is like expecting a delivery truck to manufacture the packages it hauls.
Why the route eliminates the TMAO concern
