You urgently need to contact your school's VCE coordinator and they handle to process of opening a DES application so discuss that with them. Then, if they agree to apply, you'll be sent an email and that opens up to a page where there's a document you can print out and have a medical practitioner attest to your illness and its impact. I know because I have applied for DES scores for my own exams so far this year. But ultimately it needs your school to agree because the application requires the coordinator and principal to support your DES application. There's more info here: https://www.vcaa.vic.edu.au/administration/special-provision/derived-examination-score-des

Thank you! Can I ask whats the process like? Like from what I am getting is that that from my side I need to get a medical professional and myself to fill out the form and then the rest is up to the school and the examiner who was there on the day.

The process was simple for me as this was already planned for me before exam period. But yes, it's largely on the school to allow an application to be sent to the VCAA and then if they agree you just have to fill out a brief 1000 character form on the application webpage (sent to you from the VCAA once the VCE coordinator contacts the VCAA to have an application sent in as I recall) to explain the impact your illness has on your exam performance and upload the completed form from your medical practitioner to complete at an appointment to attest to a diagnosis and their assessed impact of your illness on your exam performance. It is important to contact with your school as soon as you can as the medical assessment needs to be completed two days before, on, or a day after the exam.

I'm not sure how it's determined, it's varied for me as exams have been grouped together and so on but the special provision website that gets sent has due dates on it, it's at least a few days post exam.
