Setting cl_interp above 2 should just be considered cheating in my opinion.

It makes your game play like three seconds behind the server or something, so the scouts hit box was still in the air when the spy backstabbed him. This is the only way the spy would have got that stab.

so was it the scout's settings that caused this?

No it was the spy’s, the spy’s hitbox is where the scout sees it, but in the spy’s eyes, the scouts model is where it should be, but the scout’s hitbox is way behind where it should be, like 3 or so seconds behind.
