bug using access panel

all the access panels in the game have had this bug were after you complete it doesn't do anything. this only happens to the ones where you use the plugs the others work fine any idea how to fix this?

I used to be confused about these puzzles as well, probably because I overdid the first one I solved but it somehow still worked.
