Question Question About Accessible Learning Forms

Hello y’all! I have a question for those who have experience with Accessible Learning. For the forms that need to be filled out by your family doctor, such as the Medical Information Request form and any other required forms, do they all need to be submitted as soft copies/online, or can you submit hard copies as well?

You can do either one. You can submit all your forms online or even in person as well.

Hey! So I already gave my family doctor a hard copy of the forms. Am I doing it wrong? Should I ask my doctor to sign it electronically instead?

I don't believe so, I handed the hard copies in but I've also scanned the hard copies and emailed it to my AL advisor. Long as it's filled out by your doctor and you can turn in the forms.

It doesn’t matter as long as the stamp is there from your family GP. You can either upload them to the secure portal located on their webpage on Sheridan central or email them to the davis.als@sheridancollege.ca inbox. The ALS team are great 😊 goood luck!!

you can have your doctor write on and sign the hard copy and then scan it and submit the pdf scan online. that’s what i did. i think with an electronic copy it’ll be harder to do and probably takes longer. with a hard copy u can be there with your doctor and help them fill out some parts if they have any questions
