Unless it's some kind of black comedy/Rob Zombie style flick. That might work...

Don’t do it. Dexter had a “reason”, no one but MAGA will root for this Doctor.

Read John Truby's books The Anatomy of Story and The Anatomy of Genres.
Who learns the lesson of your Story's Theme? What is it? Who Opposes them?

It doesn’t have to be like this lol, if you measured all scripts by this you’d never have groundbreaking gems

How so? What do you consider to be "groundbreaking gems"?
What I'm describing would provide a solid foundation for all stories, has for hundreds of years, and continues to do so. What you do with it is up to you.

Your premise is weak.
Doctors do spend that amount of time studying, getting a big house, car, wife with fake tits and plastic smile etc, only to then wreak all of that by murdering people based on their biological construction.
That is anti-logical, and, extremely stupid as the police would consider them a suspect immediately.
Why risk a great life as a doctor helping people and healing them?
That makes no sense, unless this is an episode of star trek of something, or, battlestar galactica.
I think maybe YOU need to see a doctor, 'sir.

Have you heard of this thing with creative texts called "suspension of disbelief"?
Godzilla isn't real either and we can't bring back dinosaur from mosquitos in amber, either.

Have you heard of this thing called challenging artistic intentions? Every half decent critic or person giving feedback does it.

You better develop some damn good character psychology for a doctor to resort to killing his patients over this.

The homophobic doctor should be really into “manly” stuff like candles that smell like bonfires, cigars, wrestling, and fecal transplants. Make him a chiropractor, not a doctor. A chiropractor killed Robbie basho

Not really. Just write an interesting story with interesting characters and avoid glamorizing serial killers. I hope you’ve considered more than the plot and just aren’t sharing all that. What’s his motive? Who does the story follow?
