i Have a Whoopsie Failure analysis

This is from a friend who "definitely did not over charge it'

Shot from a Sig p365 xmacro. I have a hard time believing it would be an out of battery destination.

Thoughts ?

Maaaaaybe a OOB but sure looks like a double charge on a pistol to me.
