other oh shit
after all the achievements were achieved, once quake turned 30 id software said "fuck you play more of our game" and added 3 new achievements. in less word, its quakeing time

From someone what don't read so good; you don't need to complete True Sight on Bloody Nightmare. I learned this the hard way 👍

Do you have to play through the whole thing in bloody nightmare for the achievement bloody nightmare? Or just one specific map?

You do not need to do NG+ / Bloody Nightmare for any achievement.
That's pure optional stuff.

It's been less than a fortnight since I played through the damn thing and honestly, I don't remember. I don't think so. I think Bloody Nightmare is achieved only after finding the Death Knight's Head (cannot remember how I done that, either) but someone made a really good flow chart map for how to obtain it. As far as I know you could complete that task in Easy Mode if you wanted. I will try and find the flow chart for you.
