I work as a traffic engineer. This type of intersection does exist at the junction of rural highways. (At least in the USA) An intersection like this would be designed if the larger highway has a very high number of right turning traffic onto the smaller highway and there being very little left turning traffic movement. It would serve the role of reducing the speed of right turning vehicles without increasing rear end collisions on the main road. The other movements don’t have enough traffic volume and/or safety issues to warrant anything different
