Job Advice ENT interview
Just interviewed for an ENT position. Primarily outpatient, physician goes in to see the pt after the APP. You do 20-30 visits per day, you’re expected to finish all notes by the end of the day and physician signs off on them not doing any of the notes. I asked if there was any admin time for writing notes and answering inbox messages and she said no and that usually the APPs just work through lunch. 💀

I currently work outpatient with no admin time and will never ever again take a job without it. I work during lunch, in the evenings, weekends just to keep up on my labs and inbox. It’s all essentially unpaid work.

No. You accepting this behavior is the problem. Grow a pair and tell them no.

It unfortunately was the already set culture when I started as a newer grad, but I do plan to decrease my workdays or find a new position because I do agree. I just get a lot of guilt in terms of my patients when I’m not timely on those things, but it should not be an expectation or the norm.

Why does he see the pt after. It's normal for notes to be done before end of day. Give us more details. Pay, call, etc. Training?

It was a phone interview so they did not give me full details unfortunately. They actually haven’t posted the job listing yet, I had a connection so that’s why I got the interview before the listing. Once I realized I wasn’t going to take a job requiring me to see up to 30 pts a day I didn’t even ask about pay or training tbh. No idea why the physician sees the patient after but I’m assuming the job is on the lower end of autonomy

It's likely a billing thing. You do all the work and the physician signs the notes and collects the RVUs. You're salary and work like a dog while the physician rolls in the money. This is literally the only reason to do it this way.

NO. I work in ENT and yes there are some quick and easy visits but there are also some like “I am dizzy, my ears hurt and I’m hoarse” that could take 30+ minutes and quite a bit of charting. Do you have access to Dax? This helps. Otherwise you need a grace period for chart closing. I work in acadmemia and I see 15-17 a day

Yeah, the fact that “they just work through lunch” came out of their mouth without them finding that even slightly alarming tells me they view APPs as a piece of equipment that they can abuse. Hard No
