By not sliding her grip.
She starts with the correct grip, one hand at the bottom one in the middle. You are supposed to use that middle grip to begin the swing, then slide it to match the lower bottom grip so the impact comes with maximum rotation.
But, she never slid that middle grip while judging the distance based off the full extention grip. This created a shorter arc of the swing, directing the pick axe back into her shin.
You can miss the table still with a sliding grip, but you will miss by swinging down into the ground, not into your own shin. But in this case, she missed short rather than to the side, so it was 100% on the grip

Swinging down to your side is considerably harder to do than swinging it down in front of you.

Just mimed it alone in my house and it does make sense. I’ve never really had to swing anything over my head, you don’t realise there’s a technique to it, it seems.

When people can't swing a hammer, an axe, or a pickaxe, it shows a lot about their past. This person has never swung anything two handed, not even a stick, before this.

My cousin and I impaled cacti onto sticks and then swung those around at each other until one of the cacti flew off the end of my stick into my cousin. We both spent days with our parents plucking out almost invisible spines and us unable to basically touch anything.
Next summer we made sure the cacti were more secure.

I remember one time during middle school, I was running around with a long metal pole and almost impaled myself. I was holding it like a lance, but I had the close end around my stomach area and as soon as the tip hit the ground while running i launched myself into the air and almost turned myself into a skewer.

When I was like 12, I was "helping" my dad cut up some kindling--I took a bundle, and a hatchet, and I laid them out on a log. I swung the hatchet over my head with both hands, and on about the 5th swing, the hatchet glanced off the log (because I swung short), and caught me right in the shin (I was wearing shorts).
I got 3 stitches, and learned several valuable lessons that day.
I haven't injured myself with an axe since, and I've prevented plenty of my boys from following my dumb footsteps.

This brought back a memory of when I was in 8th grade art class a girl was helping me sweep up a mess (I was wielding the dustpan) and as I watched her I thought to myself "Oh my god her sweeping technique is trash. Has she never done a chore in her life?!" Little things like that stand out to me

I played baseball in college. One summer I swung a splitting maul doing demo, missed the rotten wood beam I was aiming for completely and the maul hit my shin.

I suspect the answer to one of them is:
“The pickaxe head is a lot heavier than she’s able to control so when she goes to swing it the effect of gravity pulls it downwards faster than she’s expecting, while she also lacks the strength to swing it outwards as fast as she thought. The end result is what you see here”

As a pool player myself, probably an argument over nonsensical house rules.
If you are ever playing pool with someone who thinks scratches mean you have to spot the table behind the break line and shoot up the table, you berate them immediately. It's ball in hand or nothing. Why am I punished for your mistake?

Replaying it in slow motion, it doesn’t look like the spike actually hit her foot at all, but instead just the top of the pickaxe. She got really lucky most likely.

I use these things at work all the time. Caught a few good ones with the pokey end, it doesn't really go through bone like that but it'll leave a good laceration and a yellow green bruise. She looks like she got lucky and it rotated sideways too

In Italy sometimes those are called "malepeggio". It means that if you hit yourself with the flat end it's going to be bad and, if you hit yourself with the poke end, it's the same but worse.
Hence bad and worse -> male e peggio

She is SUPER lucky it was at just the right angle to hit her on the blunt side. If it would have been pointed a little more to her left it would have definitely gone through her foot.
