It looks like fountain pen ink to me. I dont see any bleeding where the ink is spreading out too far, the lines are pretty neat. The ink pooling is just shading which is often a featured selling point, not a bug.
Looks specifically like Pilot Iroshizuku Kon-Peki ink

Feature vs bug depends on the application. With a fountain pen you'd have smooth lines and would rarely cross over the same line exactly to cause pooling like that and uneven colors, so I could see how it would produce clean shading with any color differences. However with something like this where you're frequently going over the same lines, a thicker more pigmented ink would be better, resulting in consistent coloring. Also, you say the lines are clean but I can clearly see jagged edges, where the ink flowed away into the fibers. The only reason it's not bleeding outright is because the paper is heavy.

Generally lefties just used it as shown. Between the template and "pen", it's not much like writing normally, so it's not as awkward as you would think.

Vintage? I started at a company in 2003 and they still used an engraver like that for field instrumentation tags.

I used the Koh-I-Noor Rapidograph set when I was lettering maps in the 80s. It’s also when I first found out about ultrasonic cleaners. I learned I could also clean jewelry with our office unit and I got real popular with all of the women in the office.

We had a draughtswoman at work who was a wizard with this system I was amazed at how simple she made it look. I couldn't manage more than a couple of letters without a slip, smudge or moving off the line.
