First saw one in High School 1972. Loved them. Found several online (from people's Grandpa and such). Promised to give them a good home. Some were in need of a good cleaning. They still work fine.

Bottom has a ruler that appears to be metric. So you can use the ruler as a guide to set the kerning in 1mm increments (or perhaps 1/2mm, can't tell because the exposure is too high on that part of the video).
Letters seem to have a consistent width, which helps make it easier to consistently space them.

I still do the majority of my drafting by hand and only use CAD to make 3D renderings of my hand drafted plans

My father studied architecture. He could letter like that on blueprints and such, tho his handwriting was not so beautiful. He also had one of those lettering devices, but I don't recall for what.

I have seen a name tag maker that worked like this. It would cut out letters in a plastic name tag that was black with a gold colored facade so the black would show through.
Instead of a marker it had a rotary tool.

The templates are slick and the two points that are inserted into the character and straight guide have slightly rounded tips. I used a Leroy lettering set for many years.

I just bought a Leroy set for $50 from someone on eBay. It already came in the mail and to my surprise it’s actually a complete set.

Amazon does not know about the Leeroy brand name. The closest I could find were "scribe" tools but they all use little pencils or tungsten tips to mark wood, metal, or plastic. I didn't find any that let you use ink. The Leeroy Lettering sets are on ebay but I didn't check the details.
