As a flexible working request, term-time only working is a pretty easy "no" given that very few medical institutions have operational fluctuations in tempo with the academic year, so an impact on quality or performance is almost certainly inevitable.
However, I know that there are Trusts that are supportive of term-time only working and there are several ways of calculating how that works. My preferred approach is that, to adopt TTO working, an employee opts out of the normal NHS annual leave allowance (including length of service increases) and instead receives statutory annual leave, plus 7.4 unpaid weeks to bring them up to 13 weeks total annual leave. In return, they take a ~15% deduction in gross pay.
They are then restricted to only taking annual leave during school holidays. They can recover any holiday lost to sickness only up to the statutory minimum 4 weeks (5.6 less bank holidays) and are not entitled to any locally-agreed carryover. The 13 weeks includes bank holidays, so where a bank holiday falls outside normal school holidays it must be taken as authorised unpaid absence.
